WAIVER

The undersigned parent/guardian of __________________ hereby voluntarily and

willingly allow and permit him/her to join the company visit scheduled by Mr. Raymund
E. Dilan of the College of Information Technology and Computer Science, University of
the Cordilleras. It will be held on October 4, 2010 at PLDT, Session Rd., Baguio City.

I am aware of the possible risks involved in the above mentioned. For this reason,
I hereby expressly and voluntarily WAIVE any or all possible actions against the
University of the Cordilleras and its representative, and shall not hold the later
responsible for any damages as a result of accidents and other unforeseeable events.
