Subject: Weekly Accomplishment Report

Introduction:

As a community health worker, we provide basic community health care


services for promotion of health, prevention of illnesses, simple treatment and
rehabilitation. The services utilize the philosophy, content, method and skills of
public health care. While most nurses care for one patient at a time, public
health nurses care for entire population. By working with whole communities,
public health nurses are able to educate people about health issues, improve
community health and safety and increase access to care.
Being a NDP is far more than a job or even a career. It is an adventure, a
continuous learning process that I embark on bravely each day, in search of
life changing events and miracles. Touching the lives of every family in the
community is a treasure chest overflowing with the metamorphical gold,
whether the help provided is of great measure or remains unnoticed. Nursing is
more than a profession of integrity and compassion; it is a privilege that
measures out ones value as a human being and the responsibilities granted to
us NDPs with the precious gift of life.

RESULT:

 Total of 16 households profiled on two (2) barangays, Barangay Centro and Sta.
Maria consists of individuals.
 As of the household interviewed, there were no newborns assessed.
 There are 3 infants profiled, there were no defaulters in immunization and there
were no AEFI reported. were exclusively breastfed and the other is currently
utilizing breastfeeding as of the time of interview. There were no infants classified
as underweight or obese. has no Vitamin A taken; referred to RHM for Vitamin
A.
 12-59 months were identified and profiled. 100% has completed their
vaccinations appropriate to their age. There were also no noted underweight and
stunted among the under 5 children.
 There were 22 adolescents (10-19 y/o) profiled. No teenage pregnancy noted.
Health education regarding disadvantages of teenage pregnancy was done and
adolescent health programs were identified and explained to them. There was 1
postpartum mother identified. Postpartum mother had stopped taking FESO4
since delivery; health education was done and given FESO4 tablets (90tabs). PP
mother has complete postpartum visit and had no danger signs.
 Furthermore, there were 3 of adolescents were drinking alcohol and 1 smoking,
health education regarding lifestyle modification and effects of the vices to their
health and their family members.
 Aged 15-49 were 34. 17 were identified WRA, 3 (17%) of the WRA were unmet
need. The 3 unmet need WRAs had their health education regarding other FP
methods that they can be used; referred to RHM for counselling. There were 7
WRA current users.
 There were 18 clients identified having Non-communicable diseases, 6 of them
were undiagnosed hypertensive clienteles; Clients were referred to RHU for
further assessment, diagnosis and management. Requested the BHWs to
monitor their blood pressure regularly. There was 1 client currently taking
maintenance for HPN; the client was encouraged to have follow-up check-up at
RHU for assessment and to acquire free medicine for HPN patients.
 Other significant data gathered:
o Owns a toilet 9 households (69%)
o Non-member of PHILHEALTH ≥21y/o: 3
o Individuals with ≥ 25 BMI : 1
 Integrated Health Education on Different Health Programs was done to different
barangays. In coordination to Department of Social Welfare and Development we
were able to educate the 4Ps during their Family Development Session (FDS).
 Barangay Medical Consultation in Sta. Isabel and Calamegatan was done and
different health services were rendered to them, like BP taking, height and weight
monitoring, health education and physical assessment.
