SUBJECT : Request for Supply Room and Bookshelves

1. Reference: DepEd Educational Facilities Manual (2010).

2. This pertains to the mandate in the Revised Deped Educational Facilities Manual
wherein Chapter 5 (Eduational Facilities Management) , Paragraph E (Property Responsibility)
provides that, “All educational facilities procured/purchased by the Department or donated by
LGUs/NGOs and private individuals shall be considered property of the recipient school. School
Heads/Principals are responsible to the Schools Division/City Schools Superintendent for the
educational facilities issued to them” explicitly imposing upon our school the stringent duty to
enforce the rules on the proper care of educational facilities. Subparagraph 1, number 4 of the
same provision likewise provides that, “Instructional tools/equipment and supplies such as
computers, books, magazines, periodicals, journals, bulletins and other publications issued by
the Department or procured from various sources shall be displayed and kept in safe
shelves/storage areas”. Also, the fact that a crime of Robbery has already been committed
against us in the past, taking our school properties, indicates that our school properties are indeed
vurnerable from being stolen or taken from our custody.

3. In this connection, in my official capacity as the Property Custodian of the school,

I respectfully request for the prompt construction or allocation of a safe and secured Property and
Supply Room which is designated solely to keep and store all our school properties such as tools,
equipments, and supplies. The said supply room should be secured by grills or any physical
defenses against any theft and robbers and the access of which should be limited only to those
authorized personnel. Bookshelves are also requested to prevent our books and other documents
from being consumed by termites, given that most of our books has already been destoyed by
these pests.
