ON SITE EMERGENCY DRILL

1. Nature of incident: Medical emergency at TLF, Trainee engineer Mr. Sajin

Varghese collapsed due to heat stress.

2. Incident first noticed by: Mr. Elayaraja (Utility Operator)

At Control room: Mr. Manoharan Balachandran (CRO)
At field location: Mr. Ezhilarasan Tamilarasan (Shift Supervisor)

3. Time when incident first noticed: 14:55 hrs

4. Time when incident first reported to control room: 14:56 hrs

5. Emergency services reported to affected site at: 14:57 hrs

After getting communication through: Handset (Walkie Talkie)

6. Mr. Ezhilarasan Tamilarasan reached affected area and took charge as incident
controller (IC) at: 14:56 hrs, he informed control room officer immediately about the
emergency and advised to declare emergency call using PA system.

7. Emergency call declared : YES

Time of activation : 14:56 hrs

8. Mr. Manoharan Balachandran (CRO) informed site emergency controller (SEC)

Mr. Unnikrishnan Nair about the emergency: 14:56 hrs
SEC took charge at Central Control Room (CCR) at: 14:56 hrs

9. Time when incident brought fully under control: 15:00 hrs.

10. Functioning of Incident Controller:

• Mr. Ezhilarasan Tamilarasan took charge of Incident at 14:56 hrs after

receiving the information from Mr. Elayaraja.
• Emergency declared by using handset at 14.57 hrs
• Wind direction announced on walkie talkie - NA
• Inform neighbouring industries & request them to be standby for assistance –
NA
• Loading operations at all tanker filling bays stopped and area was evacuated.
• All non essential staff was evacuated to assembly point where headcount was
done by security guard Mr. Dilip Dhuria.
• As per IC instructions, field operators Mr. Sheriff Mohammed and Mr.
Jeyakumar reached at site to attend medical emergency and were supported
by Mr. Arun Gopi and Mr. Raghu. Casualty was shifted to OHSE room by
performing a two man lift.
• Emergency called off and normalcy announced by security guard Mr. Dilip
Dhuria from exit security room.
• All clear call announced from exit security room at15:00 hrs.
• Problems if any: NIL
• Emergency was controlled within short time. Communications were made
only up to Terminal Head Level Mr. Satyabrata Mukherjee

• Communication between IC & CCR : Satisfactory

• Communication between CCR & Corporate : Satisfactory

Observer’s Remarks:-

 Mock drill was conducted as per Emergency plan.

 People took appropriate actions in short time to tackle the emergency.
 All non essential staff members were evacuated to assembly point & head
count done and was found matching.
 Emergency medical help kept ready.
 Training on “ROLES & RESPONSIBILITIES” to be given to First
responders & Incident Controllers.
 Use of PA system should be optimised.
 First Aid training to be imparted.
