any terms and conditions that may be laid down by AG's Branch (W&R Dte) GHQ Rawalpindi.
e. In case of my selection on open merit in NUMS (AM College) in any med discipline (MBBS or BDS) as indicated in my
choices of discipline in NUMS, I will become ineligible for PC Scheme and will not object/contest cancellation of my
admission at any stage.
f. Choice and priority of discipline given by me in this form is irrevocable and I will not seek a change in my choice or
priority of medical discipline given in this application form at any stage hereafter.
g. I fully understand the up-gradation policy given in Para 13f.
h. In case of unwillingness for BDS seat opted as second choice during initial selection process, I will not be considered
for MBBS seat also.
i. I will immediately inform W&R Dte about my UNWILLINGNESS if any, to avail Army reserved quota seat (PC) through
FAX/Email after declaration of NUMS entry test result.
j. I hereby agree for 3 years Voluntary Military Service (VMS) and I will be bound to submit a surety bond at the time of
getting admission. In case I fail to do so my nomination will stand cancelled automatically.

INSTRUCTIONS
(To be detached before despatch of the form)
13. Special Instructions
a. It is mandatory for candidate applying for PC Scheme to appear in NUMS entry test.
After declaration of NUMS entry test result, candidates are required to apply in
NUMS for admission on open merit. Respective institutions website must be visited to fulfill
their latest eligibility criteria/clarification, if any.
b. Wards of Shuhada/War wounded will be nominated in CMH Lahore Medical College,
CMH Institute of Medical Sciences Multan, Quetta Institute of Medical Sciences, Quetta,
Foundation University Medical College (FUMC) Islamabad, Wah Medical College, Jinnah
Medical College Peshawar and Avicenna Medical College, Lahore for which respective
provincial Med entry test is also mandatory (ref Para 20b). The candidates must secure
good mks in entry test to qualify for admission.
c. Allocation of discipline and college for wards of Shuhada/War wounded is done in order
of merit/domicile (place of residence) irrespective of priority of choices.
d. PC seats are allocated by W&R Dte based on the merit of a candidate in NUMS Entry
Test merit position.
e. After declaration of NUMS open merit position, priority of discipline already given by the
candidate in PC form will not be changed. Therefore, a candidate who is NOT serious in
pursuing a career in BDS MUST NOT give it as a second choice, ESPECIALLY for the
Open Merit Seat (s) of NUMS (AM College). If selected for such a discipline on open
merit, he/she will not be considered for PC Scheme by W&R.
f. Up-gradation of discipline as per given order of priority against the vacant seats shall be
undertaken without prior information. The process shall continue till finalization of the
admissions. Refusal to accept upgraded discipline at a later stage will result in
cancellation of nomination. However, option to withdraw from willingness of up-gradation
can be exercised once only on receipt of first nomination letter. To exercise this option
candidate shall forward UNWILLING CERTIFICATE as per specimen to be received
with nomination letter in writing within due date specified in nomination letter to W&R
Dte which will be irrevocable.
g. A surety bond for voluntary military service in the Army for three years on stamp paper of
Rs 100.00 will be submitted by nominated candidate only at the time of admission to
AM College in the light of PA Dte existing policy.
h. Form should be filled without any overwriting/cutting, usage of fresh form be resorted to
in case of any such error. Therefore, ensure proper filling of all columns with utmost
care/legible handwriting.
i. Please read the instructions carefully before filling the form. You may like to retain a
photo copy of the form for future reference.
14. Last Date for Submission of Application Form
a. Last date for receipt of application forms in AG’s Branch W&R Dte (WR-4), GHQ
Rawalpindi is 31 Aug 2018. A candidate who has appeared or is appearing in his/her
annual FSc or equivalent examination during 2018 can apply for admission. Candidate is
required to forward one attested photo copy of their FSc marks sheet or Equivalent
to GHQ, W&R Dte (WR-4) and also to NUMS on their website within seven days on
announcement of their annual result, otherwise their cases will not be processed
further.
b. All candidates will submit a hard copy of application form. Application received
through OAS or Emailed will not be processed. Application should be submitted
through Pak Post Office Registered Mail (UMS). Application mailed through private
courier is not received at GHQ.

15. Availability of Application Form. GHQ's prescribed application forms are available with all
Corps Headquarters, Station Headquarters, Army School of Instructions, District Armed Services
Boards (DASBs) and W&R Dte (WR-4), GHQ Facilitation Centre, Hilal Road, Rawalpindi and on OAS
(W&R Portal). Photo copy of the application form is also acceptable.
16. Eligibility Conditions. Following are eligible to apply:-
a. Educational Qualifications. FSc (Pre Medical) or an equivalent with Biology,
Chemistry and Physics. A minimum of 60% aggregate marks, each in Matric and FSc
separately. Candidates of A level or equivalent qualification can apply on the basis of O
Level (SSC) equivalence certificate obtained from Inter Board Committee of Chairmen
(IBCC).

b. Age Limit. 17-25 years on date of the commencement of the course.

c. Marital Status. Must be unmarried.
d. Medical Exam. Candidates are required to undergo medical examination on receipt of
nomination letter as required by NUMS.
17. Ineligibility. Following are ineligible:-
a. Candidate who is selected for admission on open merit (based on NUMS Entry Test or
SAT) in any medical discipline (MBBS/BDS) in NUMS (AM College) shall remain
ineligible for PC Scheme during the year of applying.
b. In case of unwillingness for BDS seat opted as second choice during initial selection
process, the candidate will not be considered for MBBS seat also.
c. Candidate nominated as PC in any medical discipline (MBBS/BDS) in NUMS
(AM College) is ineligible for the subsequent years’ admission. However, cancellation by
GHQ due to selection on open merit will not affect eligibility for admission in PC Scheme.
d. Candidate selected/offered as Med Cadet by PA Dte.
e. A candidate who is NOT serious in pursuing a career in other NUMS affiliated med
colleges MUST NOT give it as 2nd, 3rd choice.
f. Candidate released/withdrawn on disciplinary/academic/medical grounds/own request
from any Defence Forces Training Institution.
g. Candidate declared medically unfit (as per NUMS laid down requirement – for detail ref
to NUMS website/prospectus).
h. Children of those Army personnel who have been weeded out from service on
disciplinary grounds (due to fault).
i. Children of those ex-servicemen who have not rendered pensionable service/not
drawing pension. This condition is not applicable to those who died while in service or
medically boarded out.
j. Children of those Army personnel whose retirement period exceeds 25 years on the
closing date of applications by GHQ.
k. Adopted/dependent children and other relatives of serving/retired Army pers.
l. Candidate selected for any defence forces institution as a regular cadet.
m. Candidate not fulfilling the basic eligibility criteria of NUMS/concerned institution.
n. Wards of de-activated Mujahid Battalions.
o. Candidate selected for Engineering PC scheme/UET on the basis of A level will be
ineligible for admission in PC Medical.
p. Candidate not fulfilling aforementioned eligibility criteria and special instructions.

18. Suppression/Falsification of Information. In case any information is suppressed or falsified

and it is revealed later, at any stage, it will make the candidate liable for necessary disciplinary action/
withdrawal etc at any stage of their studies. No representation at any forum will be acceptable.
19. Category of Parents. Candidates will indicate a relevant category applicable out of following in
Para 3:-
a. Army Personnel Categories
(1) Shaheed (ref Para 11)
(2) War Wounded (ref Para 12)
(3) Serving
(4) Retired (ref Para 22 i)
b. Civilian Paid out of Defence Estimates. Only 1 x seat in MBBS is reserved for wards
of Serving Personnel in AM College.
20. Distribution of Seats. Seats will be distributed at the ratio of 60% and 40% between serving
and retired Army personnel respectively. Disciplines/College wise available seats are as under:-

b. Seats Reserved for Wards of Shuhada/War Wounded Other Than AM College. Some
seats are also reserved in the following medical colleges for the wards of Shuhada/War
Wounded. Wards of subject categories can be adjusted in these colleges other than AM
College as per NUMS merit for which selection/nomination will be made by W&R Dte
against these seats and financial assistance will be provided by the institution concerned:-

21. Documents to be attached with the Application Form. Attested photo copies of following
documents are required to be attached with the application form:-
a. Matric marks sheet.
b. FSc marks sheet Part-I and (Part-II if applicable) or equivalent.
c. Discharge Certificate/Release Order of parents.
d. NOC from respective Military Colleges (Military College students only).
e. CNIC/Form ‘B’.
22. Miscellaneous
a. Equivalence Certificate. The candidates with ‘O’/’A’ level/Senior Cambridge
Examination etc must attach equivalence certificate with marks obtained from Inter
Board Committee of Chairmen (IBCC) with the application form. Application form
received without equivalence certificate with marks will not be entertained.
b. No amendment/change of any kind will be made in the application form at any stage
once submitted to GHQ and change of subject/discipline will not be acceptable at any
stage even after the admission. Hence clarity of opting disciplines is solicited before
submission.
c. Candidate will only be considered and up-graded for discipline for which he/she has
applied.
d. Selection on Open Merit. The candidates are required to intimate AG’s Branch, W&R
Dte (WR-4) GHQ about their selection on open merit as soon as possible after
declaration of Open Merit Results of NUMS (AM College)/Regular Medical Cadets,
failing which case for admission will not be considered.
e. All eligible wards of Shuhada and War wounded (ref Para 11 & 12) are given admission,
subject to availability of seats against Army reserved seats, on merit.
f. All terms & conditions spelled out in SPAO 3/86 will be applicable for allocation of seats
in PC Scheme.
g. Students of Military Colleges can only apply for PC Scheme after obtaining NOC from
respective college before submission of application form.
h. No application form will be entertained without documents mentioned in Para 21.
i. Wards of those individuals whose LPR terminates before the date of inviting applications
(last date) will be considered in retired category and same category be reflected by the
applicant in Para 3 of the form.
23. Selection Criteria. NUMS merit will be followed for selection/nomination in PC Scheme as
well as other NUMS affiliated med colleges.
24. Fee Concession/Subsidy/Scholarship. Fee concession/subsidy/scholarship to Army wards
(Lt Cols & below) selected on open merit/PC/Additional Selected Cadet is provided only in AM
College. However, candidates nominated by W&R Dte in NUMS affiliated med colleges as PCs will
pay same tuition fee in line with AM College to concerned institutions.
25. Change in postal Address and Contact Numbers. The responsibility of intimation of change
in postal address/contact numbers to W&R Dte lies with the candidate. Non intimation of such
changes may deprive the candidate from seeking admission in the PC Scheme for which this
Directorate will not be responsible.
