ABSTRACT

Prescribing error is an error of medication process that leads or has potency

to bring disadvantage and harm toward patient safety who is under the control
and responsibility of the healtcare workers which is actually it can be prevented.
The aim of this study is to know the occurrence of medication error case which is
in the form of prescribing error, the profile of prescribing error case, and the
precentage of prescribing error case in some pharmacies in Samarinda Ulu
region at the January to Juny 2017 period. This study was using semiquantitative
descriptive research, and the data are collected by retrospective study which is
based on prescription sample from some pharmacies in Samarinda Ulu at the
January to Juny 2017 period. The results of the study showed that medication
error which in the form of prescribing error in the administrative analysis such as
there are no patient’s name in the amount of 0,39%, no patient’s age in the
amount of 16,77%, no patient’s gender in the amount 56,46%, no patient’s weight
in the amount of 94,42%, no patient’s address in the amount of 52,03%, no
doctor’s name in the amount of 10,99%, no doctor’s SIP in the amount of 15,23%,
no doctor’s address in the amount of 22,74%, no doctor’s contact number in the
amount of 52,61%, no doctor’s autograph in the amount of 90,75%, and no
prescribing date in the amount of 0,58%. In the pharmaceutical section, there are
no dosage forms in the amount of 61,47% and no dosage strength in the amount
of 72,26%. In the clinical section, there are no directions in the amount of 4,82%,
dosage mistakes in the amount of 12,71%. There are also 2,12 drug duplication
and 17,73% drug interaction (5,39% minor interaction, 9,45 moderate interaction
and 2,89% major interaction). Based on the research, the medication error in the
form of prescribing error is still potential for having medication error case in
some pharmacies in Samarinda Ulu region at January to Juny 2017 period.

ABSTRAK

Kesalahan peresepan (prescribing error) merupakan kesalahan dalam proses

pengobatan yang mengarah atau berpotensi mengakibatkan kerugian dan
membahayakan keselamatan pasien yang masih berada dalam pengawasan dan
tanggung jawab profesi kesehatan, yang sebenarnya dapat dicegah. Penelitian ini
dilakukan dengan tujuan untuk mengetahui adanya kejadian medication error
berupa prescribing error, mengetahui profil kejadian prescribing error dan
mengetahui persentase kejadian prescribing error dibeberapa Apotek wilayah
Samarinda Ulu periode bulan Januari-Juni 2017. Penelitian ini merupakan
penelitian deskriptif semikuantitatif dengan pengambilan data secara retrospektif
yang didasarkan pada sampel resep obat pasien di beberapa Apotek wilayah
Samarinda Ulu periode bulan Januari-Juni 2017. Hasil penelitian menunjukkan
bahwa medication error berupa prescribing error pada kajian administratif yaitu
tidak ada nama pasien sebesar 0,39%, tidak ada usia pasien sebesar 16,77%, tidak
ada jenis kelamin pasien sebesar 56,46%, tidak ada berat badan pasien sebesar
94,42%, tidak ada alamat pasien sebesar 52,03%, tidak ada nama dokter sebesar
10,99%, tidak ada SIP dokter sebesar 15,23%, tidak ada alamat dokter sebesar
22,74%, tidak ada nomor telepon dokter sebesar 52,61%, tidak ada paraf dokter
sebesar 90,75% dan tidak ada tanggal penulisan resep sebesar 0,58%. Pada bagian
farmasetik meliputi tidak ada bentuk sediaan sebesar 61,47%, tidak ada kekuatan
sediaan sebesar 72,26%. Pada bagian klinis meliputi tidak ada aturan pakai sebesar
4,82%, tidak tepat dosis sediaan sebesar 12,71% ada duplikasi obat sebesar 2,12%
dan ada interaksi obat sebesar 17,73% (interaksi minor 5,39%; interaksi moderat
9,45% dan interaksi mayor 2,89%). Berdasarkan penelitian yang dilakukan dapat
disimpulkan bahwa medication error berupa prescribing error di beberapa Apotek
wilayah Samarinda Ulu periode bulan Januari-Juni 2017 masih berpotensi
mengalami kejadian medication error.
