1 Safety guidelines
1.1 Requirements on the operator

1.1.1 Clinical experience and relativ e physiology knowledge on JHM-2028A Hemodialysis.

1.1.2 Fully understand the contents of this user’s manual.


1.1.3 Have receiv ed training on the correct operation and utilization of the JHM-2028A Hemodialysis

device. Able to distinguish the abnormality of the devic e.

1.2 Safety notices

1.2.1 The operator should carefully read through the user’s manual before the operation of the

JHM-2028A Hemodialysis device.

1.2.2 The operator should verify the parameter that he or she inputs, that’s to say, the operator should

check whether the input parameter value is correct or not. If there is discrepancy betw een the

optimal parameter value and the displayed parameter value, the setting value should be

adjusted before running the devic e.

1.2.3 Make sure the power supply system (cir cuit, voltage and cable) meets the electricity load

requirement of the device.

1.2.4 JHM-2028A Hemodialysis Device is a Category I and Type b devic e, please plug the power cable

into a three core socket with grounding wire, making sure the grounding is appropriate.

1.2.5 During the dialysis period, if the protective system alarms, the operator should find out the alarm

reason timely and solv e the abnormality quickly before the dialysis continues. In case of alarms

that fail to resume normal in short time, or serious abnormality, the dialysis treatment should be

terminated.

1.2.6 It is recommended to select the syringe w ith same brand and same model when changing the

syringe of heparin pump

1.2.7 The exposed live parts should be earthed by means of the protective earth for the individual

safety and the prevention from indirect electric shock. The good protectiv e earth may ensure the

personal safety in the case of the electric leakage against the injury or death caused by the

electric shock.

1.2.8 No item unrelated to the devic e is allowed to be placed on the top of devic e for the sake of the

normal operation of the devic e.


1.3 The device could only be used for one patient during the hemodialysis treatment,
1.4 Contradiction: uncontrollable coagulation abnormality.

1.5 Only the manufacture or those personnel authoriz ed by the manufacturer can assemble, debug or

repair the device.

1.6 In case of malfunctions, you can try the normal troubleshooting methods, In case of failure, please

contact the company. Non-professional personnel are not allowed to dis assemble the device.

1.7 It is the responsibility of the users to comply with the regulations related to the waste fluid discharge

in his/her country or region.

1.8 The surface of waste pipe should not be contacted with that of the wastewater treatment system or

the bacteria may enter the hemodialysis devic e along the waste pipe. The use of the waste pipe

with strong corrosion resistance feature is recommended.

1.9 The abnormal substance, whic h reaches the blood chamber of dialyzer from the dialysate chamber,

may result in the risk that the blood of patient is contaminated by bacteria and endotoxin.

1.10 The protection system w ith the measurement of venous pressure against the loss of blood in vitro

can only reduce in part the hazardousness which may remain such as the ris k condition of the

blood loss of the patient in vitro. The measures to minimize the risk: the blood pump stops and the

ultrafiltration is lowered to the min value when the preset value of alarm is exceeded.

1.11 Any narrow part of the external tube (such as the small blood tubing set or intubation) may lead to

the risk of hemolysis whic h might not be detected by the protection system.

1.12 The failure in the function of ultrasonic air detector may be resulted from the ultrasonic glue or clot.

1.13 The air may enter the external tube through the joint at the bottom of air detector; the occurrence of

such event may be caused by the use of single needle or central venous catheter in the case of

negative pressure.

1.14 When the pre-pump arterial pressure is low, the blood flow and the efficacy of treatment may be

compromised, thus the blood flow and the pressure range of pre-pump arterial pressure should be

maintained.

1.15 Contraindic ations: patient w ith the contraindic ations of hemodialysis such as shock or hypotension,

and severe hemorrhage trend; severe heart-brain complications such as cardiac enlargement

accompanied with cardiac hypofunction or cerebral hemorrhage, and severe arrhythmia, etc.

1.16 Only one type of potential equalization conductor can be used in the device.

1.17 There are certain interferences during the operation of the devic e, such as high-power motor,

solenoid valv e, induction heating equipment, high frequency welder, external radio, X-ray device,

high frequency physiotherapy device and ignition system of internal combustion engine, whic h may

affect the safe operation of the devic e. Do not place the dialy zer in the environment w ith

electromagnetic interference.

1.18 The operator should pay attention to the safety ris k related to the connection/dis connection with

patient; as well as the safety risk caused by the incorrect connection of external pipeline.

When the outdoor temperature is below 0℃, the device should
be laid aside for one hour before installation after moving it in from

2 Brief introductions
2.1 Introductions on the HD device

2.1.1 Product name: hemodialysis device

2.1.2 Model: JHM-2028A

2.1.3 Standard No. of product: YZB/National 1305-2013 Hemodialysis Devic e

2.1.4 Application: JHM-2028A Hemodialysis Device is used for Hemodialysis on patients w ith acute or
chronic kidney failure.

2.1.5 Performance, structure and components of the product: the product consists of dialysate supply

system (dialysis pump, ultrafiltration pump, proportion preparation unit, liquid constant

temperature unit, transmembrane pressure monitoring and conductivity monitoring) and blood

circulation system (blood pump, heparin pump, arterial-venous pressure monitoring, air bubble

monitoring and blood leakage monitoring), without the dis posal sterile consumables used with

the device included.

2.1.6 JHM-2028A Hemodialysis Device can be used in various medical centers and for clinical use.
When JHM-2028A Hemodialysis Devic e is conducting HD treatment, it runs and monitors the
dialysate cir culation and extracorporeal blood circulation.
2.1.7 All the operation menu and running status of JHM-2028A Hemodialysis Device is displayed on the
color LCD screen.
2.1.8 JHM-2028A Hemodialysis Device uses hydrogen carbonate and acetate for HD treatment.
2.1.9 JHM-2028A Hemodialysis Device With single needle dialysis function.
2.1.9 JHM-2028A Hemodialysis Device should be connected to the RO w ater treatment
2.1.10 In the circulation, after heating and air removing, the dialysate is conveyed to the dialyzer, whic h
keeps the flow-in fluid and flow-out fluid equal in volume. According to the selected UF rate, the
necessary UF volume for treatment can be achieved.
2.1.11 In the extracorporeal blood circulation, the blood goes through the dialy zer. The air inspector can
prevent the air entering. The blood leakage inspector and venous pressure inspector can prevent
the blood loss.
2.1.12 The mixing proportion of hydrogen carbonate concentrate and RO water is 1:1.225:32.775
(A:B:water). The manufacturer can customize the mixing proportion according to the user’s
demand.

2.1.13Based on the requirement of treatment, the concentration of Na can be adjusted in the range of
136mmol/L~145 mmol/L.
2.1.14 JHM-2028A Hemodialysis Devic e can conduct pure UF (ISO UF)/ single needle treatment/double

2.1.15 JHM-2028A Hemodialysis Devic e can conduct rinse, hot disinfection, and dis infection procedures
to complete automatic cleaning and disinfection.
2.1.16 The flux of dialy sate can be adjusted within the range of 300 ml/min~800 ml/min.
2.1.17JHM-2028A Hemodialysis Device is equipped with a safety monitor system that ensures the safety
of patients. Extracorporeal blood-losing protection system meets with GB9706.2 “The part 2~16 for
Medical Electronic Device: safety requirement for hemodialysis , hemodiafiltration and
hemofiltration”.

3 Definition and terms


Please find below some explanation on the special terms that are used in this manual.

Extracorporeal circulation: Blood circulation conducted outside the human body.

Air trap: Air device installed in the blood line system to separate the gas that is

irresolvable in the blood, including blood level inspector and air bubble

inspector.

Bypass: Bypass the dialysate from the dialyzer.

Conductivity: The electricity conductiv ity of the dialysate.

Venous line clamp: The device that automatically clamps the blood line system.

Blood measurement line: Line that connects the blood way system and the pressure monitor device.

Rotation pin: Pump head of the blood pump and spare pump, w hic h is used to rollingly

press pump tube.

4.3 Introduction on extracorporeal blood circulation

Introduction on extracorporeal blood circulation:

Connect the artery and vein of the patients w ith the blood pump, dialyzer and monitoring device, to

establis h a close cir cuit of external circulation.

During the dialysis , the heparin pump injects heparin into the blood way on a regular basis to prevent

the blood from coagulation. The heparin flow rate can be adjusted according to the treatment

requirement. The range of heparin flow rate is 0.1 ml/h-10 ml/h.

The patient blood was led out of the body by the blood pump and enters the venous pot after going

through the dialy zer. In the venous pot, the venous pressure is measured and strictly monitored w ithin the

preset range. The venous pot is located in the ultrasonic fluid level inspector to prevent the introduction of

air. Once the fluid level is lowered and bubble blood appears in the venous pot, the blood alarm is

triggered. The blood pump stops immediately, the venous line clamp automatically clamps the venous

blood way to block blood reflux, alarm w ith light and sound begins, and the UF ends.

After going through the venous pot, the blood flows through the air bubble inspector, whic h detects the

existence of air bubble in the blood circuit. Once the air bubble is found, the alarm w ith light and sound is

triggered. The blood pump and the UF pump stop immediately, the venous line clamp automatically clap

the blood way to prevent blood reflux.

Eventually, the blood is conveyed back into the patient body.

4.4 Introduction on the water way system

The following tw o RO water supply methods are available for selection:

1. Supply by the RO water tube, the adapter (W1) connects to the RO w ater tube.

2. Supply by the container, the RO water container supplies the fluid, the adapter(W1) connects to

the RO w ater container.

The RO w ater enters the water way through the adapter W1, and it’s heated in the heater system (W5)

after getting through W2 ,W3 and W4, meanwhile, air will be removed in W5 by the air-removing device

(W6).

The heat exchanger (W3) is to transfer the heat from the used dialy sate that runs out of the balance

system to the RO water.

After the air is removed, the RO w ater gets to the confecting system (W7), and the concentrate is

pumped up into the confecting system by the ceramic pump at the same time, and then they get mixed

into dialysate. The dialysate is driv en by the pre-dialysis pump (W8), and runs through the mix

temperature sensor (W9) and the mix conductivity sensor (W10) into the balance system (W11), then it

runs through another temperature sensor (W12) and conductiv ity sensor (W13), if the temperature or the

conductiv ity doesn’t meet the preset values, the pre-dialysis valve (W14) and the post-dialysis valve

(W22) w ill be closed and the bypass valv e (W23) will be opened to get the unqualif ied dialysate around

the dialyzer (W18). If the temperature and the conductiv ity meet the preset values, the bypass valve

(W23) w ill be closed, and the pre-dialysis valv e (W14) and the post-dialysis valv e (W22) will be opened

to lead the dialysate into the dialy zer.

Note:When bic art cartridge is loaded, tube B should connect back to the connector Block of

Concentrate B.

The used dialysate that come out from the dialyzer runs through a pressure sensor (W21), the

pressure measured here can be regarded as the value for TMP calculation.

The used dialy sate runs through the air-removing device (W24) again in order to ensure the volume of

the fresh dialysate is exactly the same as the used dialysate that is replaced all along. Then it is driven

by the post-dialysis pump (W28), and runs through the balance system (W11), the heat exchanger (W3)

and the blood leakage inspector (W35). The blood leakage inspector is to monitor if there is blood

infiltrating the dialysate cir cuit due to the damage of dialysis membrane. After that, the used dialysate

runs through evacuation valv e (W36) and gets out of the water way from outlet of evacuation (W37).

The UF pump (W34) draws out the dialysate from the fluid infusion system at the preset rate. As the

system is airtight, the ultrafiltrate can be drawn out at the same volume from the blood through the

dialyzer.

4.5 Introduction on hydrogen carbonate and acetate dialysis

4.5.1 Connection of concentrate tube

Tw o types of concentrates used in hydrogen carbonate dialysis :


——Acidic concentrate (concentrate A)
——Hydrogen carbonates concentrate (concentrate B)

The concentrate is sucked by two pumps, and is conveyed to the HD devic e to mix w ith the RO water.
The suction tube of concentrate is connected to the container accordingly.
To minimize misusing of concentration, concentration container should have special mark.
To avoid the misconnection with container, sucking pump should als o have special mark.

Warning:
You must use the container with specif ic mark and matched suction
tube, to avoid wrong connection.

The mixing process is based on volume and proportion. That is , the concentrate pump mix certain
volume of concentrate into the water of fix ed volume.
Only by using the concentrate matching the mixing system, you can get a correct ion concentration in
the dialysate.
By measurement on the conductiv ity and the bypass function, the system can prevent the dialysate
with wrong concentration from entering the dialyzer.
In case of existence of wrong components in the dialysate, the conductiv ity will exceed the alarm range
and triggers the alarm. The HD device then opens the bypass, accompanied w ith light and sound alarm.
The condensate of dialy sate is required to comply with the standard YY 0598-2006.

The components of dialy sate include: sodium chloride, potassium chloride, calcium chloride,

magnesium chloride, sodium bicarbonate andglacial acetic acid.

The dialytic powder should be diluted as per the ratio specif ied in the instruction. The sample of

dialysate is collected from the device and the resultant ionic concentration of dialysate should be

consistent with the labeled amount as shown below:

The sodium (Na) is 97.5%-102.5% of labeled amount;acetic acid (or acetate) is 90%-110% of

labeled amount;

Calcium (Ca), kalium (K), Magnesium (Mg), Chlorine (Cl) and bicarbonate (HCO3)should be

95%-105% of labeled amount.

Specific measurement method: When the device is running in Hemodialysis mode, collect the

sample of dialy sate as the devic e become stable. The test is conduced according to the description in

Item 6.5.1~6.5.4 on Page 4 of YY0598-2006 and the result should be consistent w ith the requirements

described above.

Warning:
The hydrogen carbonate concentrate and acetate concentrate cannot
be stored again once opened.

Warning:
Please make sure the container has sufficient hydrogen carbonate
concentrate and acetate concentrate to complete the treatment.

The single fault condition defined based on the alarm limit of protection system:

Upper limit alarm of mixed conductivity: the error among each component of dialy sate is ±0.5%;

low er limit alarm of mixed conductivity: the error among each component of dialy sate is ±0.5%.

Upper limit of outflow conductiv ity: the error among each component of dialysate is ±0.5%; lower

limit alarm of outflow conductivity: the error among each component of dialysate is ±0.5%.

the content of citric acid is measured with high-performance liquid chromatography (refer to appendix B)

and the content of peracetic acid is determined w ith quantitative test paper, the result should be

consistent with the requirement in the table above.

Follow ing the dis infection, the sampling is done at the supply port, and the content of citric acid is

measured w ith high-performance liquid chromatography (refer to appendix B) and the content of

Press button “sampling port 1 of UF fluid”,


when UF sampling tube is bounced out, it means
the test the test has been started.. Please put
measure container on place of “sampling port 2
of UF fluid” and collect the liquid from UF
sampling tube.
After the test, comparing the UF dialysate (flow
out from sampling inlet) measure result w ith the
set UF volume to get the UF precis ion.

5 Treatment
5.1 Preparation work

5.1.1 Start to run JHM-2028A Hemodialysis device

Note:
Please make sure the follow ing preparation work has been done
before using the HD device.
1) The connection betw een the HD device and the RO water supply
system has been done.
2) The connection between the HD device and the drainage system
has been done.
3) The connection betw een the HD device and the power supply
system has been done.
When the hemodialysis machine is running, don't run the other device
whic h will transmit electromagnetic wave or radio wave as this kind of
device will bring work malfunction to hemodialysis machine.

5.1.1.1 A+B common dialysis mode

Put A/B concentration absorbed tube into


corresponding container, then connect the
dialysate supply and return tube onto dialyzer
respectively.

5.1.5 The installation of the blood pipe system

Open the cover of the pump, put the entry of the blood pump pipe into the fix ed trough, press the
switch of blood pump, the rotor will move slow ly, the blood pump w ill enter into the space of two poles.
When the rotor move, the pump pipe will move into the pipe way, and fix the trough at last.

Adjust the elastic press of roll pole


Adjust the ball bearing of the rotor to adjust the screw, to adjust the degree of the tightness of the
pump pipeline.
If choosing other siz e of the pump pipe, do adjust the degree of the tightness of pump pipe.
Adjust ways: adjust the screw of the ball bearing of the pipe, increase or reduce the length. After the
pump pipe is installed, fill in a bag of saline water to the entry of the pipeline, hang in the place of 1 meter,
adjust the ball bearing pole and the screws, so as to make the ball bearing to press the pump pipeline
until the saline water won’t pour out, whic h is the best situation.

injection bracket, take hold of the adjust knob of
the bracket, push it upw ards to adjust the height
of the injection height.

5.1.7 The installation of the adjust- liquor connection

The hydronium in the dialysate is the main factor to decide the penetrativ e pressure of the dialysate,

whic h effects directly the concentration of the blood serum of the patient and the penetrative pressure. In
the process of the haemodialysis, adjust the concentration of the hydronium of the dialy sate according to

the situation of the patient, to reach up to the excellent result.

5.2.1 Dialysis treatment
The patient can only be treated when the temperature of dialysate is maintained exactly within the

set temperature ±0.5℃ of dialysate and the conductivity within the target conductivity ±0.3Sm/cm prior to

the dialysis treatment.

6 Cleanness
6.1 Introduction

Cleanness includes exterior cleanness and interior cleanness.

Exterior cleanness is the cleanness of the outer part of the hemodialysis device after treatment.

Interior cleanness includes hot dis infection, dis infection and rinse, that is the cleanness of the

waterway in the hemodialysis device.

The process of rinse: the system w ill rinse the waterway of the hemodialysis device.

The process of hot disinfection: the system will remove the calc ium and rinse with hot water to the

waterway of hemodialysis device.

The process of the disinfection: the system w ill perform the dis infection and iron-removing to the

waterway of the hemodialysis device.

Warning:
The system should be undergoing the interior cleanness after the
dialysis treatment.

Note:
If the system is left unused for more than 72 hours, it is proposed to
under go the interior cleanness before use.

Note:
If the hemodialysis devic e is left unused as long as 5 minutes after the
process of the cleanness, the system w ill be power-off automatically.

Note:
After the completion of rinse procedure, the hemodialysis device
may be powered off automatically if it idles for 15 minutes.

6.2 Exterior cleanness

Every time after the treatment, use the pledget to clean the water and stain on the surface of the

hemodialysis devic e with the alc ohol or the hypochlorite. Keep the cleanness and the dryness of the

hemodialysis devic e. If there is the phenomenon of leaking water or dialysate, it should be cleanup and

deal w ith as soon as possible.

Do examine the colander web on the connector termly. If with the phenomena of jam, the web

should be cleaned or changed. The discarded filtering web should be dis posed properly to prevent

against pollution of the environment.

6.3 The basic condition to undergo the process of interior cleanness

7.7 Disposal of unusual thing

7.7.1 The disposal principle for the unusual thing of hemodialysis device

Before the dialysis:


1. If there is malfunction when start the hemodialysis device, restart the device to examine and check.

2. If there is the malfunction about the conductivity, perform the “rinse” in the allowed short time, then
examine and check.

3. If the hemodialysis device can’t resume, please transfer the dialysis patient to another good
hemodialysis devic e to receive the treatment.

4. Don’t use the devic e to do any dialysis treatment before it is repaired.

During the dialysis:


1. If there exis ts the warning voic e which w ill not affect the patient’s safety and can be solved at spot,
after the warning voic e, it is ok to do the dialysis treatment for the patient. After the treatment,
examine and check it at length.

2. If the warning voic e can’t be eliminated at once, do reinput the blood to the patient’s body and pause
the treatment, then undergo the examination and check. If it is resumed, continue the treatment,
otherwis e change the hemodialysis device or continue the treatment after the device is resumed.

3. During the treatment of dialysis , if there is malfunction that it is impossible to use the dialysis device
to return the blood for the patient, do turn the spanner on clockw is e by hand to make the blood
transport to the patient. Continue the treatment after the devic e is resumed or change another
device.

4.If the reverse ultrafiltration occurs during dialysis treatment, the water whic h enters into the body may

increase the blood pressure and weight of the patient. When confirm that this is not caused by the

malfunction of devic e, you can increase the blood flow or the ultrafiltration volume such that the

transmembrane pressure increases to the normal range.

7.7.2 The disposal of unusual things of hemodialysis device

7.7.2.1 The disposal of warning

Note:
With the alarm voice, press the key of “alarm mute” to eliminate the
warning voic e,but the warning indicator and warning words will still
display on the screen. At this time, operator should inspect the
parameter of protection system which the alarming is cancelled.

Warning:
The power supplied by the backup battery may maintained for 15
min after the power supply of the alarm system is failed; and the log will
be retained in this period.

Note:
The record will be retained even if the power of alarm system fails (mains
and internal supply ).

Note:
The power w ill be supplied by backup battery after the mains is
failed, and the alarm settings will not be changed during the period of the
power-off and recovery.

Warning:
If the device can’t be resume by the outer adjustment, do report to the
Engineers to repair the device.

Warning:
If the same or similar devices are used in any separate area such as
ICU or cardiac operating room, there may be potential hazardness when
the different alarm preset is applied.

8 Maintenance and storage

8.1 Termly check the device whether the supply of water (such as the water purify unit) and electric ity

(such as the manostat) is good or not. And check whether those devices work properly or not, the

voltage is stable or not. For the area whic h the voltage is alw ays higher or lower, it is better to use the
automatic manostat.

8.2 Check the precision of the ultrafiltration per half a year or a year, have a look of the operation 4.12 for

reference.

8.3 Termly check the filtration web in the hemodialysis device, it there exists the situation of blocks, clear

up or change it in time.

Note:
After clearing up or changing the web, take care the airproof when
installing to avoid the connector of web w ill leak air.

8.4 If with the built-in power supply, the user should charge the battery according to follow ing ways when

they use the device for the first time:

Connect the hemodialysis device with the electricity source system by the electricity wire.
Connect with the electric ity source, the devic e should be charged for about 20 hours.

If the device have not used for a long time, repeat the process every 6 months.

8.5 The device can’t be impacted to avoid the damage to the parts in it.

8.6 The device should be put upright in the house w ith the relative humidity not more than 80%, no

causticity, and good ventilation.

9.1.1 You should use the dedicated arm band.


9.1.2 While the recovery time following the defibrillation voltage to be applied via cuff is at least 2-3

minutes, the cuff can only function as expected when such period is expired.

9.1.3 Do not fold tightly the arm band or air pipe.

9.1.4 There is potential risk when the balloon is maintained in over-inflation state.

9.1.5 For the occurrence of the general arrhythmia (such as APB, VPB and AF), the measured value
may not be correct or the blood pressure cannot be measured.

9.1.6 Although the precision of pressure sensor of cuff has been tested, it is recommended to check and
calibrate the sensor annually in order to ensure the normal function and precis e measurement of
the hemomanometer. Use of pressure calibration instrument for verif ic ation is suggested.

9.1.7 Range of temperature/humidity

Operation temperature/humidity: temperature 10℃-38℃, relative humidity ≤80%, atmospheric pressure

70kPa-106kPa.

Storage temperature/humidity: relative humidity ≤80%, in the room without caustic gas and w ith good

ventilation.

9.1.8 The blood pressure value measured w ith this hemomanometer is equiv alent to the measured value
of stethoscopy w ith the error consistent with the requirement of YY0670.

B.1 Chromatogram conditions and suitability test

Use of sulfonic acid anion exchange resin (Rezex ROA- Organic Acid H+ ( 8%,150 mm ×7.8

mm, 8µm), or column w ith similar nature) as filler, and 0.015mol/L sulphuric acid solutionas the

mobile phase, flow rate 0.6ml/min, detection w avelength 210nm, column temperature 40℃. The

chromatogram conditions and suitability test should be consistent w ith the requirements of Appendix

VC High-performance Liquid Chromatography in Chinese Phar macopoeia ( Part 2, 2010 edition): R

B.2 Deter mination of liquid chromatogram

1.0 g citr ic acid control w eighted precisely is dissolved w ith dialysis w ater to volume in the

volumetric flask until 10.0g/L citric acid control is obtained. Take and pour 20μL of such control in the

liquid chromatography for determination and the calculation of the peak area. Take the test solution

and dilute based on the appropriate dilution ratio w ith the dialysis w ater as the solution to be tested.

This solution is filtered w ith 0.45μm membrane and 20μL of w hich is taken and poured in the liquid

chromatography for determination and the calculation of the peak area. The concentration of citric

acid in the test article is calculated w ith the external standard method.
