3. Elimination Pattern
Patient has no difficulties of urinating with slight yellow to clear urine and voided at
minimal of 2- 4 times day. In terms of defecating, he usually defecates everyday with
stool color of brown , soft in consistency with minimal odor depending on what he eats.
4. Activity-Exercise Pattern
The patient usually can sit and ambulate but with assistance by his warcters due to his
post operative pain site and discomfort. In general, the patient requires help and
assistance when it comes to self-care and other functional activities.
5. Sleep-Rest Pattern
He finds difficulty in sleeping the hospital due to he is accustomed to sleep in the house
and finds the place bothering due to the noises of other patient watchers and usually
sleeps around 5-7 hours.
6. Cognitive-Perceptual Pattern
He has no signs of cognitive problems and recalls past events in his life. He aware of the
date, time, place and his name and age. He has no difficulties answering questions ang
takes time answering it.
7. Self-Perception Pattern
The patient describes himself as a jolly person but with current state he is anxious about
post- op site wound and concerns about his self esteem being operated already .
8. Role-Relationship Pattern
He is a happy guy and he has a brother and a sister. He is grateful that his mom and dad is
there for him specially that he is admitted in the hospital and being operated and said that
he loves his family soo much.
9. Sexuality-Relationship Pattern
He has no reproductive dysfunctions and sexually transmitted infections. And currently
he has a girlfriend
