Dear MAYOR,

Greetings!

As we approach the end of the fiscal year, the Department of Health - National Capital Regional
Office must warrant that all programs and activities that will be prioritized and carried over to
the following fiscal year are geared towards the attainment of the goals and targets under the
Fourmula One Plus, the National Developmental Framework, the Philippine Developmental
Plan and the Sustainable Development Goals. Further, it is imperative that the local government
units define clear strategies on how the goals and targets can be achieved.

In the light of this, the District Health Team 2 of the Field Operations Cluster, Local Health
Support Division, would like to invite six of your program managers (Immunization, Family
Planning, Dengue, AIDS, Service Delivery Network, and Health Facility Enhancement
Program) to a Program Evaluation and Planning Workshop on September 11-14, 2018 at
INSERT VENUE HERE. The intention is to provide an intimate venue for the LGU program
managers, partners and stakeholders to review and evaluate the overall health systems
performance of District Two, as well as listen and share the different gains and gaps in health
programs.

We are hoping that you will accord this matter with the most favorable response and we thank
