Observations and Recommendations for change, if adverse condition exists

Adverse Condition: There shall be a five-step or fifteen foot walk off mat at all entry points into the building. If the area
cannot accommodate a fifteen foot mat, the mat should be as long as the area will accommodate.

Inspector Comments: The mat at the cafeteria door to the playground and the mat to the exterior door near room 188 need to
be replaced or repaired. Both of these mats or rugs present a tripping hazard

Adverse Condition: All food in break rooms shall be properly stored so as to limit attraction of pests. Refrigerators in break
rooms shall not be used for storage of chemicals or specimens.

Inspector Comments: School wide all food items must be put in plastic containers to reduce the risk of mice and to remove
food supply. Everything from crackers, to pre-packaged mac and cheese, fruit snacks, candy and all fod
items must be kept in a plastic protective container. At the time of inspection, the director of food service
delivered approximately 20 large plastic containers to the kitchen to get all cereals, crackers out of
cardboard boxes and into plastic containers

Adverse Condition: All buildings shall be maintained in general good repair.

Inspector Comments: Observed there is a 1/2 inch gap between the exterior brick wall and the concrete. Please seal this gap
to avoid pest entering the building

Adverse Condition: Doors shall be properly installed and maintained to fit tightly in their frame. Exterior doors shall have no
cracks, gaps, or other visible openings that allow the entry of pests.

Inspector Comments: Please check all exterior doors to make sure they are tightly sealed. Add weather stripping to door that
show any type of gap. The smallest gap can be an entrance for mice and other pests. Please do this
before the weather gets cold and mice are looking for a warmer place to nest and live.

Adverse Condition: Written procedures for responding to vomiting or diarrheal events were not available.

Inspector Comments: I gave copies of vomit clean up to the nurse (Tonya) and the PIC (Peggy) in the kitchen

Adverse Condition: Furniture and toys shall be cleanable, clean, and in good repair.

Inspector Comments: There are stuffed animals in room 105, bean bag chairs in room 171 and some large reading pillows in
other rooms. All of these fabric material items must be easily cleanable and disinfected to avoid
spreading germs, bed bugs and head lice. Please develop a plan for cleaning and disinfecting items

Observations and Recommendations for change, if adverse condition exists

Adverse Condition: Televisions and heavy equipment, that may pose a tipping or falling hazard, shall be securely anchored
to a cart, wall, or the floor. Heavy equipment shall be moved only by authorized personnel.

Inspector Comments: In the library there are many books stacked up high on the book cases. Please make sure books and
other items are not stacked high These present a tipping hazard if students reach up high or bump
these items and can lead to injury. Please locate these books somewhere that is more safe

Adverse Condition: Floors shall be maintained in a dry, sanitary condition without tripping hazards.

Inspector Comments: There are a few mats by the cafeteria exterior door and by room 188 exterior door that are worn and
may present a tripping hazard

Adverse Condition: Drinking fountain streams shall crest high enough above the mouth guard of the fountain so that user's
mouth does not contaminate the spout.

Inspector Comments: The drinking fountain right inside the restroom area across from the art room needs to be repaired

Adverse Condition: Walls and windows shall be maintained in good repair. There shall be no signs of cracking, damaged
paint, signs of condensation, or water intrusion.

Inspector Comments: The wall below the window in room 141 needs to be repaired to avoid water damage or other structural
problems.

Adverse Condition: There shall be no evidence of pests or obvious food sources for pests.

Inspector Comments: There is an infestation of mice in rooms 141, 136, 105 and the office area. There is some activity in the
school kitchen and in other classrooms.
Room 141 is a pre-K room and they have seen mice run under the napping cots when kids are sleeping
and in their blankets.
They have seen mice on counter tops and there has been dropping throughout the room including near
the backpack areas
The teacher in this room has put food in large plastic bins and we discussed other items that should be
in bins
The exterior doors in room 139,141 and 149 will be re-keyed so they are not held open by a piece of
wood during the recess time. This can also lead to pests entering the building. Ms Jones has already
put in a work order for the doors to be re-keyed, the exterior area be sealed, the wall in room 141 fixed
and the weather stripping

Misc. PIC: Ms Jones (Principal) Lester Barnett (custodian) Peggy McBride (school kitchen) Sean Marshall (IA)
Met with Ms Jones to discuss the mice problem. She called in work orders to have the exterior wall sealed, new
weather stripping added to exterior doors, continue exterminator treatments, and other recommendations in this
report
Peggy McBride called the food services supervisor and ordered large plastic containers for all cereal, crackers,
cookie type food items. Please keep all these plastic containers at least 6 inches off the floor. Keeping food in large
plastic containers will reduce the risk of mice in the food. These plastic containers arrived at the time of inspection
from the food service supervisor.
Met with Nurse Tonya about nursing safety and disinfecting cots etc. There were no violations or concerns with the
nursing area
The playground had no problems at the time of inspection.
The exterior of the building needs to be sealed to avoid the entering of pests into the building
All staff must keep their food in plastic containers. Please have every teacher and staff member do an inventory of
their work space and remove any food items including boxes of crackers, pre-packaged noodles or rice and candy.
All of these items attract mice. Any food kept in a classroom must be kept in a plastic container-no exceptions
Columbus Public Schools had an exterminator out this morning working on bait boxes and sticky traps. Teachers
and staff shared photos of evidence of mice infestation. There must be a continuous treatment of the infestation
and removal of ways for mice to enter the building and removal of food sources. Ms Jones is dedicated to
improving this problem with Columbus City Schools.
We will monitor this situation and return for a follow up inspection and keep track of the treatment of the pest
problem
Some teachers and staff have shared photos with me to show mouse activity.
Please continue to take photos and document activity to help with treatment.
Please DISINFECT any surface where mouse drooping are found and keep a log of mouse activity to help with
further treatment.
