In line with the present thrust of Laguna University (LU) in providing its students with a well-
balanced academic program in Bachelor of Science in Accountancy, students are required to undergo
400 hours of practicum within the 1st Semester of A.Y 2018-2019 starting August 13, outside the
campus. This is to prepare them as accounting professionals and likewise, to ensure that their
educational formation shall satisfy the needs of the business sectors.

We are pursuing this program in the belief that what is learned in school becomes more effective if it
is supplemented with on-the-job training, thus making students adapt easily to the actual work after
graduation.

In this connection, we would like to recommend Mr. Jaybee M. Rodriguez, a 4th year Bachelor of
Science in Accountancy student of Laguna University with good scholastic records and good moral
character. Attached herewith are his application letter and resumé as initial requirement for your
perusal.

Rest assured that the knowledge and skills that he will learn in his stay as a trainee in your office shall
be applied and used in his future endeavors in the professional field.

WAIVER
I, Jaybee M. Rodriguez, a Bachelor of Science in Accountancy student of Laguna University, will
have my internship Laguna Provincial Capitol, Sta. Cruz, Laguna in order to acquire professional or work
experience related to the academic training I learned from the classroom.
And in addition, I shall also work with the following terms and conditions:
 That I shall abide the rules and regulations of the host company and shall comply with what is
stipulated for in the memorandum of agreement.
 That I shall be responsible for whatever duties or task assigned to me, particularly those
indicated in the recommendation and acceptance letters.
 That I shall be reasonably liable for all the damages of properties or facilities I will incur
while performing my duties.
 That I will not hold Laguna University liable for any damage or injury that I will suffer due to
my own negligence in connection with my internship in the company.
 That the company shall not be liable for the injury I will cause myself due to my negligence
while performing my duties.
 That I will not demand that the company hires me after my internship.
 That I will not require the company any compensation, salaries, allowances and any form of
incentives as payment for my service as an intern.

That the company has the right to withdraw or terminate my training, if I fail to comply with the
company’s rules and regulations and the aforementioned terms and conditions.
