AFFIDAVIT

I, YOLANDA A. RUANTO, of legal age, married, Filipino, with residence

address at 946-A, Adelina Street, Sampaloc, Manila, after having been duly
sworn in accordance with law hereby depose and state:

1. I married MR. RUBEN M. RUANTO on 29 July 1992 at

the Manila City Hall. A copy of my Marriage Contract is attached
herein and made an integral part hereof as Annex “A”.

2. During our marriage, we begot two (2) children namely,

RUBY ABIGAEL A. RUANTO who was born on 01 February 1900
and ROY FRANCISCO A. RUANTO who was born on 03 July 1991.
Copies of their Certificates of Live Birth are attached herein and made
integral parts hereof as Annexes “B” and “C”, respectively.

3. Sometime in 2003, my husband, Ruben M. Ruanto, left

me and my children and decided to go home in Quezon Province.
Since then up to present, Ruben M. Ruanto neither communicated
his whereabouts nor visited me or my children and even failed to
extend support to me and my children. In other words, Mr. Ruben
M. Ruanto has abandoned us for fifteen (15) years now.

4. I am executing this Affidavit to attest to the truth of the

foregoing and for whatever legal purposes this Affidavit may serve.

of Manila by the affiant, exhibiting to me her Social Security System I.D. No. 33-
2149147-6, a competent evidence of her identity under Section 12 of the Rules on
