TUTORIAL 1

1. General Learning Objective:


After completing the tutorial activity, Students will able to understand and explain about nursing care
to client with placenta previa

SKENARIO 1
“What happen with me?”

A 37-year-old woman, G1P0A0, was 32 weeks pregnant came to the hospital because of
contraction and vaginal bleeding without pain. Patient say there is no prior history of fall.
She said the bleeding occur after sexual intercourse. Ultrasound examination results:
placenta covering ostium edge, DJJ= 130 x/min. Nurse encourages the patient to bed rest
and collaboration tocolytics administration. The patient is afraid of the baby condition
because this is the first child. Nurse supports that the child is a gift from God.

SKENARIO 2

“…Pain…”
A 25-year-old woman, G1P0A0, was taken to the hospital by the family for complaining
that she had been contraction since 6 hours ago, regular, and out of bloody mucus. The
results of vaginal toucher: soft palpable portio, opening 3 cm, rupture membrane, head in
hodge 2 position, with head presentation. Contraction 4 times in 10 minutes, with duration
<40 seconds. After 10 hours, the patient feels the urge to straining, pressure on the anus,
prominent perineum and open vulva. Nurse helps the delivery process. Nurse performs
episiotomy because of stiff perineum. 2 hours later the baby born with male gender. Nurse
assist the mother to do early breastfeeding initiation. Nurse encourage breastfeeding until
the age of 2 years.

“Am I dying?”

A 25-year-old woman, P1A0, gave birth 2 hours ago. Physical examination results:
blood pressure 80/60 mmHg, pulse 120 x/min, weak pulse, temperature 37 0 C,
under pad seen full of blood, weak uterine contraction, TFU 3 fingers below the
umbilicus. Her husband said that before giving birth his wife drinking immersed
Fatimah grass. The patient looked worried. Nurse performs internal bimanual
compression to stop bleeding, monitor TTV every 15 minutes, and collaboration
of oxytocin administration. Nurse motivate patient and families to dhikr.

to client with uterine myoma

SKENARIO 4
“Irregular spot bleeding...”

A 48-year-old woman, P1A0 came to the hospital with her husband because of vaginal
bleeding outside the menstrual cycle. Patient said every day she had to replace 4-5 times
pads because the pads were full. Patient also complained of lower abdominal pain. The
result of physical examinations: TD 90/60 mmHg, N 88 x/m, R 20 x/m, temperature 38.40
C, and mass palpable in the lower abdomen. The ultrasound results show a solid mass near
the Uterine Internal Ostium with the conclusion of uterine myoma. Laboratory results
showed Hb: 6.7 gr %. The doctor gave a blood transfusion and planned a hysterectomy.
Patient say confused how to pray because of her condition. Nurse provide education about
Istihadah.
