Aug. 01,2018- Went to Laua-an National Highschool to give School Based Immunization to Grade 7 students.

Aug.02,2018- Went to San Ramon primary school to give School based Immunization to grade 1 and grade 4 Pupils.

Aug.03,2018- Admitted Patients for consultation,Taking Vital signs and dispensed medicines to MDR clients.

Aug.06,2018- Admitted Patients for consultation,Taking Vital signs,and cleaning of wounds of the patients,dispensed
Oral Contraceptive Pill to the family Planning acceptor.

Aug.07,2018- Admitted Patients for consultation,Taking Vital signs,complete patients information,wound

dressing,dusting and cleaning before and after duty hours.

Aug.08,2018-Handle and Assisted Delivery,Cleaned the Delivery Room,Made Progress notes of both Mother and Baby.

Aug.09,2018-Taking of Vital Signs of Pregnants who went for their Pre-Natal check-up ,Immunize the Newborn and
Infants and Dispensed Oral Contraceptive Pill to the Family Planning Acceptor.

Aug.10,2018-Went to Casit-an Elementary School to give School Based Immunization to Grade 1 and 4 pupils.

Aug.13, 2018-Admitting patients for consultation, taking vital signs, and dispensed medicines in the pharmacy.

Aug. 14, 2018- Went to Eastern Laua-an National Highschool to give School Based Immunization to Grade 7 students.

Aug.15, 2018- Taking vital signs of pregnant women who went for their pre-natal check-up. Immunize the newborn
babies and infants, inject DMPA to the FP acceptor.
