JOINT AFFIDAVIT OF COHABITATION

WE, EUGENE GLENN BACO ACHURRA and GINALYN TUAZON OPERIANO,

both of legal age, Filipino, Single (living together) and residents of Purok 4-A, Bingag,
Dauis, Bohol / # 55 Zamora St., Ilo-ilo City Proper, Ilo-ilo City, Ilo-ilo, Philippines,
after having duly sworn to in accordance with law, do hereby depose and state,
THAT:

1. We are the above-named affiants as well as the per circumstances to

whom they pertain respectively;

2. We have been living together as husband and wife on the same roof,
freely, continuously and without interruption, since August of 2012
or more than five (5) years now;

3. During our cohabitation and even until present, there exist no legal
impediment or any obstacle whatsoever, which might prevent us
from marrying each other inconsistent to Philippine laws, its pertinent
rules and regulations, or its customs and traditions;

4. Our union were blessed with a beautiful daughter, and named

PRINCESS EULYN OPERIANO ACHURRA who was born on January 28,
2015, running four (4) years old early next year;

5. On account above, we look forward to enter that special and solemn

contract of marriage to finally legalize said union ,and somehow,
allow our only daughter to have the right to acquire the status of
legitimacy, and more so, of availing the privilege exception as
therein provided, pursuant to Art.34 of E.O. 209, otherwise known
as the Family Code of the Philippines, concerning marriage license
and its issuances as a requisite necessary for validity of marriage ;
and

6. We are freely, willingly and voluntarily executing this JOINT

AFFIDAVIT OF FREE COHABITATION to attest as to the truth and
veracity of the foregoing statements, and for whatever legal purposes
this may serve best.

IN WITNESS WHEREOF, we have hereunto set our hands and affix our
