1. The Organization

Sahara Air-Conditioning Co. W.L.L. was established in 2006 in Kuwait and today it is one of the successes
full company in Kuwait Over the years, the scope of business has also diversified into several works such as:

Installation
Maintenance
Material Supply
Manpower
Projects
Operations

Apart from the above mentioned works, SACC also pursues privately financed projects under Build-
Operate-Transfer (BOT) schemes.

PART B: HSE MANUAL

1. SCOPE

The HSE system is applicable to all operations within the Organization and it is a commitment to improving
health and safety performance. This HSE Manual defines how the Organization shall establish and maintain
an HSE Management System.

2. REFERENCES

3. DEFINITIONS

The terms and definitions as given shall apply.

3.1 Incident - Undesired work-related event(s) that resulted in or could have resulted in death, illness, injury,
damage or other loss (regardless of severity).

3.2 Risk - Combination of the likelihood of an occurrence of a hazardous event or exposure(s) and the
severity of injury or ill health that can be caused by the event of exposure(s).

3.3 Acceptable Risk - Risk that has been reduced to a level that can be tolerated having regard to the HSE
Policy and legal obligations.

3.4 Hazard Identification- Process of recognizing that a hazard exists and defining its characteristics.

3.5 Risk Assessment - Process of evaluating risk(s) arising from a hazard(s) taking into account the adequacy
of any existing controls, and deciding whether or not the risk(s) is acceptable.

3.6 Non-conformance - Any deviation from relevant work standards, practices, procedures, regulations,
management system performance, etc. that could either directly or indirectly lead to an injury or illness,
property damage, damage to the workplace.

3.7 Corrective Action - Action to eliminate the cause of a detected nonconformity.

3.8 Preventive Action - Action to eliminate the cause of a potential nonconformity.

3.9 Procedure - Specified way to carry out an activity or a process

3.10 HSE Record - A document that demonstrates compliance to the requirements of the HSE Management
System. It is the output of a work instruction, providing objective evidence of compliance to the
requirements specified therein.

3.11 HSE - Health, Safety & Environment

4. HSE MANAGEMENT SYSTEM ELEMENTS

4.1 General Requirements

The HSE Management System established by the Organization shall be implemented, maintained and
continually improved.

The HSE management system places emphasis on the prevention of risk rather than dependence on detection
of risk or non-conformances.

4.2 Health Safety & Environmental Policy

4.2.1 The HSE policy shall be made known to all Organization employees through open access to system
documentation (electronic media) in all operations.

The Organization's HSE system shall be subject to ongoing audit, review and monitoring to ensure its
continued effectiveness. The HSE policy shall be reviewed by top Management for continuing suitability.

The Management considers no phase of operation or administration as being of greater importance than a
safe and healthy working environment. The Management also believes that all employees are very valuable,
and must return home safely; therefore the Management actively encourages total personal commitment, and
a responsible safe attitude to achieve a safe working and healthy environment for its employees and its
Clients.

4.3 Planning

4.3.1 Planning for Hazard Identification, Risk Assessment and Risk Control

The Organization shall ensure that planning for the ongoing identification of hazards, the assessment of risks
and the implementation of necessary control measures are established, maintained and kept up to date.

The Planning for Hazard Identification, Risk Assessment and Risk Control shall include routine and non-
routine activities of all persons having access to the work place, facilities and temporary sites.

The Organization shall ensure that the results of risk assessment and the effect of risk controls are considered
when establishing the HSE objectives.

All activities and operations that have potential to cause harm, including injury or ill health to people, and
damage or loss to assets including neighbours, shall be formally evaluated in accordance with the
requirements of this manual. This evaluation shall identify and assess the risks associated with these
activities and operations, and what controls need to be implemented to ensure that risks are eliminated or are
as low as reasonably practicable. Human behaviour, capabilities and other human factors will also be
considered part of Hazard Identification process and Risk Assessment.

Management of risks involves management of the hazards and effects. The essential steps of hazard
management are as follows:

a) Hazard Identification
b) Evaluation of Risk
c) Identify and evaluate risk reduction measures
d) Implement selected risk reduction measures

4.3.1.1 Hazard Identification & Evaluation Hazards and effects shall be identified as early as possible and
shall be tracked throughout the activity or operation.

Staff at all levels of the organization shall be actively involved in the hazard identification process. For all
projects, it is important that key persons involved in the proposed activities are involved and aware of the
hazards specific to that project.

HSE hazards shall be evaluated mainly according to inspection and experience. A Risk Assessment form
shall be prepared for each operation, listing hazards and describing the controls to be applied to reduce risks
to acceptable levels.

Quantification that risks are as low as reasonably practicable (ALARP) shall be included.

4.3.1.2 Risk Assessment and Risk Control

The assessment that risks are ALARP (As Low As Reasonably Practicable) shall be made by reasoned
judgment and consideration of alternative or additional controls on hazards. For ALARP to be demonstrated,
risks must be judged to be at levels that are considered both “Acceptable” and for which the cost or effort of
additional/ alternative controls are considered disproportionate to the risk reduction obtained. The following
guideline for hierarchy of risk control shall be considered while evolving the control measures to the
identified risk.

b) Substitution
c) Isolation
c) Engineering
d) Administrative
e) Personal Protective Equipment

Barriers shall be put in place to reduce the probability of the release of the hazard (control) or to reduce the
consequences (recover).

Tasks that need to be carried out to avoid major consequences, which may lead to unacceptable risk hazards,
are defined as health & safety critical tasks.

Barriers need to be linked to the health & safety critical tasks carried out prior to or during operations. It is
only by conducting these tasks that assurance is provided that the major hazards will be managed.

4.3.2 Legal and Other Requirements

The organization shall ensure that the applicable legal and other HSE requirements are identified and are
accessible to the projects and departments. The organization shall communicate relevant information on legal
and other HSE requirements to its employees and other relevant interested parties.

4.3.2.1 The organization shall be responsible for checking any new laws and reporting them to HSE to take
the necessary actions e.g. providing training, if required, within the organization.

4.3.2.2 The company shall establish a list of legal and other HSE requirements. The list shall be updated and
distributed annually. Should any new requirements become available before the next update, then necessary
action shall be taken to distribute the new requirements to those affected by the changes.

4.3.3 Objectives

The HSE objectives shall be set by the organization at each relevant function and level within the
organization consistent with the HSE policy. The HSE objectives shall be measurable in order to facilitate an
effective and efficient review and to enable HSE policy to be achieved.

The HSE objectives shall be established considering the following:

Relevant to the organization’s business as a whole.


HSE policy, including the commitment to continual improvement.
Results of Hazard Identification, Risk Assessment and Risk Control.

Legal and other requirements.
Technology options.
Views of employees and interested parties.
Records of HSE non-conformances, incidents, and property damage.
Results of Management reviews.

The HSE objectives shall be reviewed and revised as necessary.

4.3.4 HSE Management Program

The Organization shall establish and maintain an HSE management program for achieving its
objectives.
The HSE management program shall include responsibilities and authorities for achievement of the
objectives at relevant functions and levels of the organization.
The HSE management program shall include the means and time scale by which the objectives are
to be achieved.
The HSE management program shall be reviewed annually and amended as necessary.

4.4 Implementation and Operation

4.4.1 Structure and Responsibility

4.4.1.1 The organization shall ensure that roles, responsibilities and authorities are defined, documented and
communicated to persons who manage, perform and verify activities having an effect on the HSE risks of the
organization’s activities, facilities and processes.

Top Management has delegated certain authorities and responsibilities and these are communicated within
the Organization in order to implement and maintain an effective and efficient HSE Management System.

Persons throughout the Organization are assigned responsibilities and authorities to enable them to
contribute to the achievement of the HSE objectives and to establish their involvement, motivation and
commitment.

The HSE responsibilities and authorities of key operational entities within the Organizations are as under:

i) Managing Director

 Define and document the commitment to HSE on behalf of the Organization.


 Authorize and provide adequate resources for the implementation, maintenance and
verification of the HSE system.

 Delegate authority and responsibility to General Manager, Operations Managers, and
Department Managers concerned in the management, performance and verification of this
HSE system.
 Appoint a Management Representative with the authority and responsibility of ensuring that
this HSE system is implemented maintained and verified.
 Participate in Management Reviews of the HSE systems.

ii) General Manager

 Advice top management regarding the corporate policies / procedures related to HSE.
 Ensure that all Operations Managers remain committed towards achieving HSE objectives.
 Participate in periodic management reviews of the HSE system as required.

iii) Manager of Projects

 Authorize and provide adequate resources for the establishment, maintenance and
verification of the HSE system for the project.
 Ensure that the project's HSE system properly and efficiently satisfies the requirements of
the Contract.
 Ensure compliance with Company’s Occupational Health & Safety Management System and
Quality Management System.
 Shall be accountable with regards to applying control measures, taking corrective/
preventive action and conducting safety meetings in the project(s) assigned.
 Ensure Customer Satisfaction

iv) Department Managers/ Unit Heads

Verify that the HSE system for the project is implemented and maintained.
Implement HSE related training for all Organization persons.
Coordinate HSE non-conformance identification, disposition and corrective action follow-
up.

Proposals Department

Ensure that safety requirements have been considered in any proposals that are submitted.

Human Resources Department

Responsible for getting the right people in the right jobs and develop them in the right way through
training and regular Performance Evaluations especially persons, who manage, perform and verify
activities having an effect on the HSE risks of the Organization's activities, facilities and processes.

Procurement Department

Check the safety requirements for purchased material and tools in coordination with Health & Safety
Manager.

· Logistics & Procurement Processing Units

Check that safety requirements are addressed on all orders for purchased material and tools.

· Material Control

Ensure that the necessary safety storage conditions are fulfilled for all materials.

· Equipment Division

Ensure that safety items for all equipment and lifting gears are checked and maintained.

4.4.1.3 Management Representative

The appointed HSE Management Representative shall have responsibility and authority as defined
below:
Ensure that the HSE Management System is properly implemented and performs to
requirements at all locations and spheres of operation within the Organization.
Verify that the HSE Management System is established, implemented and maintained
Ensure that reports on the performance of the HSE Management System are presented to top
management for review and as a basis for improvement of the HSE Management System.

The currently appointed HSE Management Representative is Mr. Mohammed Abdul Rehman.

The management shall ensure that adequate resources are available for the maintenance of safe
workplaces, including equipment, human resources, expertise and training.

The Management Representative shall be accountable with regards to Communication and


conducting internal safety meetings from the corporate health and safety level.

4.4.2 Training, awareness and competence

4.4.2.1 General

The employees, whose work may impact on HSE in the workplace, shall be recruited on the basis of
appropriate education, training, skills and experience.

All levels of management and employees shall be trained to understand their respective responsibilities to
maintain HSE in the workplace.

Each new and/ or transferred employee shall attend an HSE training induction meeting after arriving on the
project.

4.4.2.2 Identification of Health & Safety Training Needs The HSE Department in coordination with the Cost
Center Managers shall jointly define the HSE training needs, based on the nature of the job and the
competencies required for performing tasks that may impact HSE in the work place.

The HSE Department shall keep a list of health & safety trainings using the Employees Health & Safety
training matrix.

4.4.2.3 Training & Induction Areas The HSE trainings and inductions given to employees shall include an
explanation of the following:

The importance of conformance to the HSE policy and procedures, and to the requirements
of the HSE Management System.
The HSE consequences, actual or potential, of their work activities and the HSE benefits of
improved personal performance.
Their roles and responsibilities in achieving conformance to the HSE policy and procedures,
and the requirements of HSE Management System.
The awareness of the relevance and importance of the employees activities, and their roles
and responsibilities in achieving conformance to the HSE policy, procedure and the
requirements of HSE Management System

Appropriate records of education, training, skills and experience of the employees shall be maintained.

The effectiveness of training and inductions given to employees shall be recorded.

4.4.3 Consultation, Participation and Communication

The HSE Management System and information shall be communicated to and from employees and other
interested parties (contractors and visitors).

The employees shall be:

Involved in the development of polices and procedures to manage risks


Consulted where there are any changes that affect workplace health and safety
Represented on health and safety matters, and
Informed who is their HSE representative and management appointee.
Involved in hazard identification, risk assessment and determination of controls.
Involved in incident investigation as appropriate.

This will be done through regular and planned HSE meetings, induction courses, trainings, newsletters and
toolbox talks.

An HSE meeting will be held with Subcontractors/ Suppliers (as appropriate) to ensure their commitment
and cooperation to the organization’s HSE Management System.

4.4.4 Documentation

The information related to the HSE Management System shall be maintained as part of the Organization’s
management system.

The HSE policy and objectives of the Organization, translating the requirements into terms specific to it’s
method of work, organization and resources. This manual shall form the basis for the development of all
other HSE work instructions.

The establishment, maintenance and control of this HSE Manual shall be the responsibility of the Safety
Management Representative and the Safety Manager.

4.4.5 Document and Data Control

The purpose of document and data control is to provide identification and traceability to the current revision
of documents, and eliminate the use of obsolete documents.

All documentation and data relating to the requirements of the HSE system described in this manual shall be
subject to review and approval for adequacy by authorized persons prior to issue. Access to this
documentation shall be available at all worksite locations.

Obsolete documents shall be disposed off, or clearly identified as such, or stored in a separate location. If
however it is necessary to retain obsolete documents for any specific reason, they shall be clearly marked
and identified to prevent unintended use.

Any changes to the document and data shall be reviewed and approved by those persons responsible for the
original review and approval, or by any other designated personnel having the authority and pertinent
background information. The nature of changes shall be identified either on the concerned document or
appropriate attachments.

All efforts shall be made to ensure that documents remain legible and readily identifiable.

4.4.6 Operational Control

4.4.6.1 General

The organization shall ensure that:

All operations and activities associated with identified risks are identified, planned and
controlled.
All HSE risks of goods, equipment and services received from subcontractors or used by the
organization are identified.
Control measures and procedures are developed for operations and activities associated with
identified risks.
Safe Operating Procedures are established consistently, which shall be applied to the
maximum extent they are applicable in all operations.

The Safe Operating Procedures shall form the basis for operational control at projects and work facilities.

4.4.6.2 Instructions

The Project Specific Safety Manual shall provide the minimum acceptable level of safety standard to be
applied for areas and applications appropriate to that area; i.e. for SAC, JO, KOC, KNPC and EQUATE
Projects, etc.

4.4.7 Emergency preparedness and response

4.4.7.1 General

Each work location within the organization’s main office and project sites shall prepare an
emergency plan that shall be reviewed, tested and improved whenever new activities with
risks, hazards and emergencies are identified.
The emergency plan shall describe the potential and response to hazards and emergency
situations.
All employees shall be informed and trained on emergency preparedness and response plans.
Managers, Area Heads, senior staff, supervisors, foremen, etc. are responsible to train their
subordinates in emergency preparedness and response plans. They are also responsible for
conducting emergency trials to ensure the right understanding and response.
The emergency preparedness and response plans shall be available at all working areas as
guidelines, to help protect people, assets and reputation.

4.4.7.2 Emergency Procedures

Each working site shall initiate their own emergency plan that defines the following:

Responsible persons for evacuation, firefighting and first aid.


Instructions of evacuation, rescue and medical treatment.

Instructions for communications with authorities and relatives.
4.5 Checking and Corrective Action

4.5.1 Performance Measurement and Monitoring

4.5.1.1 The HSE performance shall be measured and monitored using both qualitative and quantitative
measures. Both Proactive and Reactive measures of performance shall be established to ensure that the HSE
objectives are achieved.

The following methods can be used for HSE performance measurement:

Conducting workplace safety inspection and recording on daily Safety Log/Concern/


Observations form
Evaluation of new worksites and equipment
Survey to determine employee’s attitudes on HSE
Analysis of documentation and records
Control of Monitoring and Measuring Equipment
Control and maintenance of equipment

The records of calibration and maintenance of equipment shall be retained.

4.5.1.2 On a monthly basis each project shall submit a Statistics Report including the number of incidents,
property damage, fire, number of violations, number of health & safety inspections, number of trainings
taken, etc.

4.5.1.3 The HSE Department shall analyze the data and prepare an annual safety report for top management.

4.5.2 Evaluation of Compliance to Legal and other HSE Requirements

4.5.2.1 The HSE Department shall evaluate the compliance of legal and other HSE requirements annually
and record the results of the evaluation for further improvement (e.g. conducting workplace safety inspection
and recording on daily Safety Log/Concern/ Observations form, safety audit).

4.5.3 Incident, Non-Conformance and Corrective & Preventive Action

4.5.3.1 General

a) The Cost Center Manager and / or Safety Officer shall have the primary responsibility for handling
investigations and taking necessary action to mitigate any consequences arising from incidents and
nonconformities.

b) The Cost Center Manager in cooperation with the HSE Department shall be responsible for the initiation
and completion of corrective and preventive actions, and for confirming the effectiveness of the corrective
and preventive actions taken.

c) All proposed corrective and preventive actions shall be reviewed through a risk assessment process prior
to implementation.

d) The corrective and preventive actions taken to eliminate hazards that can cause incidents, and eliminate
the root causes of actual and potential non-conformities shall be appropriate to the magnitude of problems
and commensurate with the HSE risk encountered.

e) Any corrective / preventive action leading to new hazards or changes in controls should also be considered
in hazard identification.

f) The results of any corrective / preventive actions that are implemented shall be documented as appropriate.

g) The Health & Safety Manager shall analyze the HSE incidents and nonconformities, evaluate the
significance of certain trends and may recommend corrective and preventive actions. The Health & Safety
Manager shall also be involved with the concerned operations and departments' staff to participate in the
corrective and preventive actions process.

h) All Department Managers, Manager of Projects and persons as may be assigned by them, shall be
responsible not only for identifying the areas for potential corrective and preventive actions to be taken, but
also in taking the lead in implementing any recommended corrective and preventive actions.

4.5.3.2 Incident Records/ Incident Investigation

The incident records and investigations shall be executed as described in the Safety Manual.

An investigation shall be conducted as soon as possible after the incident using the Incident Investigation
Report. The investigation must be fact finding not fault finding.

4.5.3.3 Corrective Actions

4.5.3.3.1 The corrective actions shall focus on eliminating the causes of incidents and nonconformities in
order to avoid recurrence. The sources of information for corrective action may include:

Incident reports.
Nonconformity reports.
Internal audit reports.
Outputs from HSE management reviews.
Outputs from risk assessments.
Relevant HSE Management System records.

4.5.3.3.2 The Health & Safety Manager shall analyze at regular intervals the incidents and nonconformities
raised through incident reports, non-conformance reports, internal audits findings, and other sources to
identify the basic cause of problems. The appropriate corrective actions shall be identified with responsibility
assigned to implement the corrective actions. Risk assessment should be done before the implementation of
proposed corrective actions.

4.5.3.3.3 The corrective actions may be:

Changes to HSE Documentation.


Additional resources.
Training of persons.
Modification of the work processes.
Risk control instructions or processes.

These actions shall be verified through HSE internal audits.

4.5.3.3.4 The effectiveness of the corrective actions shall be reviewed by the Health & Safety Manager and
reported in the Management Reviews.

4.5.3.4 Preventive Actions

4.5.3.4.1 Information obtained through hazards identification and risk assessments, incident reports, non-
conformance reports, internal HSE audit results, and customer consultations shall be reviewed for consistent
trends to detect and eliminate potential causes of nonconformity. The steps to be taken to prevent occurrence
shall be identified and progress shall be reported in the Management Review meetings.

4.5.3.4.2 The preventive actions may be:

Changes to HSE Documentation.


Additional resources.
Training of persons.
Modification of the work processes.
Risk control instructions or processes.

These actions shall be verified through internal HSE audits.

4.5.3.4.3 The effectiveness of the preventive actions shall be reviewed by the Health & Safety Manager and
reported in the Management Reviews.

4.5.4 Records and Records Management

Records shall be established and maintained to provide evidence of conformity to requirements and of the
effective operation of the HSE Management System.

The controls needed for the identification, traceability, storage, protection, retrieval, retention time and
disposition of records shall be established. All efforts shall be made to ensure that records remain legible and
readily identifiable.

4.5.5 Internal Audit

Internal audits shall be conducted at planned intervals to determine whether the HSE Management System

Conforms to the planned arrangements and to the HSE Management System requirements
established by the Organization,
Is effectively implemented and maintained, and
Is effective in meeting the Organization's policy and objectives.

Follow-up audits shall be conducted to review the results of previous audits. The information on the results
of audits shall be provided to management.

An audit program shall be planned, taking into consideration the result of risk assessments of the
Organization's activities, as well as the results of the previous audits.

The audit criteria, scope, frequency and methods shall be defined. The selection of auditors and conduct of
audits shall ensure objectivity and impartiality of the audit process. The auditors shall not audit their own
work.

The responsibilities and requirements for planning and conducting audits, and for reporting results and
maintaining records are defined in a documented procedure.

The management responsible for the area being audited shall ensure that actions are taken without undue
delay to eliminate detected nonconformities and their causes.

Follow-up activities shall include the verification of the actions taken and the reporting of verification
results.

4.6 Management Review

The Organization’s HSE Management System shall be reviewed annually by the Safety Steering Committee,
to ensure its continuing suitability, adequacy and effectiveness. Management reviews shall be the platforms
for the exchange of new ideas, with open discussion and evaluation of the inputs being stimulated by the

leadership of Top Management. The reviews shall include assessing opportunities for improvement and the
need for changes to the HSE Management System, including the HSE policy and HSE objectives.

The agenda for the management review may include the following:

Any remedial measures carried out to the system since the previous HSE management
review.
Incident statistics.
Internal and / or external HSE Management System audit results.
Status of incident investigations, corrective and preventive actions.
Review of HSE policy and objectives.
Evaluating suitability and effectiveness of the HSE Management System;

Meeting current needs or any changes required, including legal and other requirements.

Reports of hazard identification, risk assessment and risk control processes.


Results of participation and consultation
Relevant communication(s) from external interested parties, including complaints.
Recommendations for improvement.

All Management Review Meetings shall be recorded and maintained.
