1. As part of your Group assignment pick any four (4) cases from the
above list. The condition for the class is that ALL of the above cases
must be covered by groups in the class.

2. Each group must address the following areas in their presentation:-

a. Explain the factors that led to the accident/incident.

b. How was the accident/incident investigation carried out.
c. What lessons can be learned from the accident/incident.
d. If the main factor being the Human Factor, elaborate using
any of the human factor models learned.

3. Group will be required to present this assignment in the next class

as scheduled.
