SOCIAL SECURITY SYSTEM

APPLICATION FOR SOCIAL SECURITY NUMBER

Step by Step Guide

1. Accomplish all fields of the online form correctly.


2. A link shall be sent to the registrant's email which will enable them to continue with their
SS Number application.
Note: The link shall expire in 5 days and once it expires, the registrant will have to
repeat the first step of the registration process.
3. Once the link is clicked, the registrant must supply all the required information from
Basic Information up to Beneficiaries Information correctly.
4. Prior to generation of SS number, the registrant may review and correct their
electronically filled out registration form. Once an SSS Number is already generated,
applicant will need to go to the SSS Branch to correct his/her information.
5. After clicking the "Generate SS Number" button, the system will display the SSS
number of the registrant and an option to print their ePersonal Record and SS Number
Slip. The registrant will also receive an email confirmation which indicates his/her SS
number, a copy of his/her SS Number Slip and other instruction to complete his/her SS
Number application.
6. Print the Personal Record Form, SS Number Slip including the SSS Number
Application Confirmation email body sent to you by the SS Number Issuance
System.
7. Go to your nearest SSS Branch and submit the required documentary requirements
together with the SSS Number slip.
8. If married, bring a copy of your Marriage Contract
9. If has child/ren, also bring a copy of the child/ren's Birth Certificate
10. For Non-Working Spouse, your working spouse must sign before his/her name on the
printed Personal Record Form. The signature of your working spouse signifies that
he/she agrees with your SSS Membership.

Reminders
1. Registrant is advised to go to the nearest SSS Branch for submission of the required
supporting documents.
2. Registrant's membership status with SSS shall remain "Temporary" until submission of
the required supporting documents. This means that the SS Number can only be used
for:
Contribution payment and employee Submission of required documents for
reporting by the employer. conversion of Membership Status from
"Temporary" to "Permanent".
3. Availment of SSS loans and benefits (subject to qualifying conditions) and UMID Card
Application (provided applicant has one (1) posted contribution) shall be allowed only
for Permanent Membership Status.

Required ID Cards and/or Documents for the Issuance of SS Number


1. Birth Certificate

2. In the absence of the Birth Certificate, any of the following documents:


Baptismal Certificate Professional Regulation Commission
Driver's License (PRC) card
Passport Seaman's Book(Seafarer's
Identification and Record Book)

3. In the absence of the above ID card(s) and/or documents, any two (2) of the
following, both with the correct name and at least one(1) with date of birth:
(GSIS) card/Member's ID card issued by (LGUs) (e.g
Record/Certificate of Membership Barangay/Municipality/City)
ATM Card (with cardholder's name) ID card issued by professional
Alien Certificate of Registration association recognized by PRC
Bank Account Passbook Life Insurance Policy
Baptismal Certificate of child/ren Marriage Contract/Certificate of
Birth Certificate of child/ren Marriage
Cert. of Licensure/Qualification Membership card issued by Private
Document from Maritime Industry Co.
Auth. National Bureau of Investigation (NBI)
Certificate from Office of Muslim Affairs Clearance
Certificate from Office of Northern Overseas Worker Welfare
Cultural Community Administration (OWWA) card
Certificate from Office of Southern Philippine Health Insurance
Cultural Community Corporation (PHIC) ID card/Member
Certificate of Naturalization from the Data Record
Bureau of Immigration Police Clearance
Company ID card Postal ID card
Company Representative School ID card
Authorization Card(ACR) issued by Seafarer's Registration Certificate
SSS issued by (POEA)
Credit card Senior Citizen card
Firearm License card issued by Student Permit issued by Land
Philippine National Police (PNP) Transportation Office (LTO)
Fisherman's card issued by Bureau of Taxpayer's Identification Number (TIN)
Fisheries and Aquatic Resources card
(BFAR) Transcript of Records
Health or Medical Card Voter's Identification card or
Home Development Mutual Fund Affidavit/Certificate of Registration
(Pag-IBIG) Member's Data Form
