FOR HIGH SCHOOL
1. New and transferee students

Note: Old students with failed subjects are not allowed to enroll without
completion.

B. Enrolment Procedures
NEW ENROLEES
1. Present the complete requirements for enrolment.
2. Enrolment requirements will be evaluated, enrolment form will be issued for filling
up.
3. Pay the enrolment fee to the finance officer after filling up the enrolment form.
4. Official receipt will be issued for the payment.
5. An interview will be given to transferees only with their parent/guardian before
they are allowed to enroll.

OLD STUDENTS
1. Submit the Form 138 or Report Card
2. Students with failed subjects in the previous year are required to submit their
grades on failed subjects taken during summer class.
3. Follow the procedures for new enrollees.

C. Rules Governing Enrolment


1. Enrolment requirements submitted upon enrolment cannot be withdrawn. It shall
be filed at the Record Section.
2. Enrollee who cannot present the complete requirements may be admitted as
temporary student/pupil with the approval of the principal. The enrollee becomes
permanent student/pupil as soon as the enrolment credentials will be completed
within the given period.
3. Withdrawal/ cancellation of the enrolment can be granted with valid reasons and
the parents/ guardians shall be the requesting party. It can be granted only within
the period of enrolment.

D. Identification Card
All students are required to have ID card (school ID/ library ID). It serves as the
passport of the students in entering the school campus and in order for them to enjoy the
facilities of the school.
All students are required to submit two (2) pieces of 1x1 recent colored picture in
school uniform for the school ID at the beginning of the school year for freshmen and
transferees.
Lost school ID means securing another ID within a week. Strictly no ID no entry.

E. Rules on Special Examination Period


1. Special examination will be granted only to students who failed to take the
scheduled examination given by the school for failure to pay the tuition and/ or
sickness.
2. The following two (2) days after the scheduled examination will be the only period
for the special exam. If the student fails to take the special exam, a grade 70 will
be given for the periodical test. (other schedule is on case to case basis)
3. Promissory note will be granted/ accepted only one week before the examination
day/s by the school director and/ or representative to those with valid reasons and
the parents/ guardians shall be the requesting party for any promissory note.
Strictly no promissory will be entertained during the examination period.
4. Payment during examination day/s will be allowed only when done by parent/

guardian. This is to keep the students ready for the examination and to be free
from any inconvenience.

F. Issuance of Transfer of Credentials


1. Any student who wishes to transfer to another school is granted with transfer
credentials with the following requirements:
a. must be cleared of all school obligations
b. comply the school clearance slip
2. Submit the clearance after complying to the record office for evaluation.

G. Graduation
1. Graduating students must have passed all of his/ her subjects.
2. Have complied completely the school clearance.
3. Have fully completed the graduation requirements.

4. Must have attended religiously the graduation rehearsals. Graduating students with
three consecutive absences during rehearsals will not be allowed to attend the
graduation rites.
5. All cleared graduating students are required to attend the graduation exercises and
claim their credentials after the graduation program.

H. Procedure for Determining Honors


1. The class advisers and the subject teachers determine the honor students in every
year level for every grading period and to be approved by the principal.
2. In the first grading to the third grading period, only the academic grade will be
computed to determine the top ten students.
3. Except for the year-end recognition, honor students for every grading period
awarded on convocation program will be determined using a grade-cut-off system
as follows:

With Highest Honors 98% - 100 %

An honor student recognized on convocation program for every grading period is someone
who not only excel in his/ her class but also achieves the highest standards of academic
excellence as set by the school.

•Honor students shall have no grade below 90%.

•The ranking committee for the graduating honor students shall be the
principal as the chairman, the class adviser, and the subject teachers as
the members to be approved by the school director.

•The ranking committee for non-graduating students shall be the class


advisers and the subject teachers and to be approved by the principal.

I. Scholarship Grants for Deserving Students

1. With highest honor, With high honor, With honor, Athletes in selected events of at
least Provincial Level.

• 1,500.00 is discounts to tuition fees.

2. Student Wards of the School Director/Employees

• 1,500.00 is discounted to tuition fees. Employees with sons and


daughters, brothers and sisters and the convent boys for the school
director. There would be a minimum of one ward and maximum of two.
Excess wards will be paid as in the usual rates. Other Fees/Misc. will be
charged

III. FINANCE POLICIES

A. Enrolment
When a student registers in school, it is understood that he/ she is enrolling for the
whole year. A student who wishes to transfer or withdraw and has paid the pertinent tuition
and other fees in full maybe charged with the following:
1. 10% of the total amount if he/ she withdraws within the first week of classes.
2. No discount if she/he withdraws after the opening of the class.

Tuition fees paid for the first month during the enrolment period together with the
other school fees and etc. WILL NOT BE REFUNDED. (Source: Manual of Regulations
for Private Schools, 1970 Edition, p.35 and p. 137).

Old students with outstanding balance must pay first said balance to be accepted in
the following school year.

Transferring and dropping-out of students:


1. If it is on the third month of the semester, the whole semester shall be charged.
2. If it is on second semester, the whole year will be charged.

One semester is equivalent to five months.


The first semester is from June to October while the second
semester is from November to March.

B. Mode of Payments
1. Every student is obliged to pay monthly or on installment basis for the whole year
obligation on tuition fee, miscellaneous fees and others, for a lighter and easy
way of paying the total amount due for the whole school year.
2. Account Card or the statement of account card should be presented when paying
the monthly obligation to the Bursar.
3. Without the account card or statement of account card, no student is allowed to
take the examination or (No permit No Exam). They will have a promissory letter
to be addressed to the Principal and he approved by the School Director, a week
prior to the examination.
4. Reissuance of the account card shall be charged Php50.00
only.
5. Scheduled of payments during examination day is two days before the
examination.

IV. LIBRARY RULES AND REGULATIONS

The Library is the storage of knowledge. Students come for mind nourishment and
advancement. Printed and non-printed materials are made available for student services and
to all bonafide MOMANIANS.

Its service is available at designated time; all MOMANIANS are part in the
preservation of the school library and must follow religiously the guiding rules and
regulations for a quality services and performance.

A. Library Hours
•Monday to Friday, 7:00—6:00 P.M.

B. Identification and Borrower’s Card


1. At the beginning of the school year, each student is issued with a library ID which
will be presented when borrowing or returning a library material.
2. The Library ID is needed when using any material placed on the reserved section in
the library.
3. The student who is not in uniform must present his/ her Library ID to the checker
at the entrance.

C. Behavior in the Library


1. Observe silence. Idle conversation, loud laughter and other unnecessary noises
disturb others so it must be avoided. If it is necessary to talk, it should be done in
subdued tone.
2. The librarian has the right to send out students who do not behave. Habitual
offenders will be reported to the school principal for disciplinary actions.
3. The library should be kept clean. Wastebaskets are provided for waste papers and
scraps.

4. To prioritize the readers, doing individual projects and the like in the library is
strictly prohibited. Eating and sleeping are not allowed.
5. Books and other library materials should be handled with extra care. They are
intended to a number of users.
6. Things in the library must be kept in order after using. Chairs must be returned to
their proper places.
7. Students should never commit such acts like hiding or stealing books or other
library property, tearing/ writing on pages of books and periodicals. Stiff penalties
will be imposed on violators of this rule.
8. Big bags, envelopes, etc., must be left at the deposit counter. They should be
properly inspected before leaving the library.
9. Responsible Internet is advised—hence, opening of Facebook, YouTube and other
unnecessary site is not allowed

D. Circulation Regulations
1. BOOKS AND OTHER MATERIALS NOT FOR HOME USE.
The following must not be taken out in the library:

GENERAL REFERENCES. These include encyclopedias, dictionaries,


almanacs, atlases, handbooks, and other books found in the general reference
section.
PERIODICALS and NEWSPAPERS. Newspapers and journals are to be
read in the library. For special reasons, a periodical maybe checked out with a note
of the teacher of a particular subject or a special permission from the librarian.

2. RESERVED BOOKS

Materials placed on the reserved section at the counter include basic reference for the
five learning areas. They are for overnight use only or more depending upon the number of
copies available. Overnight book is out at 5:30 P.M. and due at 8:00 A.M. of the following
day.
If more copies are available, a borrower can request for a minimum of three nights
only.

3. GENERAL CIRCULATION BOOKS


Fiction, biographies, novels, and other general reading materials are issued
for one week and renewed for another week.
Other books are issued out for students for three days and may be renewed
only once.
The Librarian may recall any book anytime the need arises.

4. TEXTBOOKS
Textbooks are compulsory requirement for all students. (Please refer to the
Finance In charge for the terms and conditions of acquiring.)

E. Responsibility
The borrower is held responsible for all books, materials he/ she borrowed. They
must be returned before or on the due date.
Borrowed books should be returned properly, i.e., to the circulation counter.
Returning the books elsewhere does not relieve the borrower of his/ her responsibility.

F. Circulation Procedure
1. Open-shelf system borrowers are free to go directly to the shelves and withdraw
books needed except those found in the special and reserved section.
2. Books are arranged numerically by class numbers from 000– 900 (Dewey Decimal
System)
3. The borrower’s card is needed to check out any library material.

4. A student may borrow a maximum of two books or two general circulation books
but never two reserved books.
5. Any student using library card of other student for borrowing books or any library
material is held liable and is subject for disciplinary actions.

G. Fines and Other Penalties


1. A fine of one peso a day will be imposed for overdue home-reading reference and
fiction books. Sundays and holidays are included.
2. A fine of five pesos a day will be imposed for overdue reserved books.
3. A book lost must be reported immediately to avoid accumulation of fines.
4. A book reported lost must be paid for or be replaced with a new one. Damaged
book must be paid for or be replaced with the latest edition of the same title.

V. NON-ACADEMIC POLICIES

W. School Uniform
Every MOMA student is expected to wear proper prescribed uniform inside and
outside the campus during the school days. Everyone should be proud wearing the school
uniform.

1. Students should come to school properly dressed and well-groomed.


2. Only students wearing the prescribe uniform are allowed to enter the school
premises and to be admitted in the classes. Strictly no entry to students not in
proper uniform or not wearing the uniform.
3. Uniform must be worn within the school premises and during other school
functions as maybe specified by the administration. Students should not wear
their uniforms in public places such as movie houses, disco joints, and the like.
They should respect their uniform for it symbolizes the school.
4. Written request for non-wearing of uniform must be filed at the principal’s office
with the recommendation of the teacher adviser. Students granted uniform
excuse slip must present the said slip to the security officer and to other subject
teachers.
5. Wearing the bakya/ step-in, sandals and slippers are not allowed. Students should
wear only black shoes with white socks for girls and boys.
6. On Wednesdays and other non-uniform days, all are required to dress decently
which means no wearing of the following: 1. see-troughs 2. plunging neck lines
3. spaghetti straps 4. very short skirts 5. short pants 6. pedal/ purontong 7.
sleeveless shirts 8. sleeveless blouses, 9. ripped jeans and the like.

B. Student Behavior

ON-CAMPUS BEHAVIOR
1. All students are expected to respect all authorities and show courtesy in their
dealing with their co-students, the administration, faculty members, school staff,
maintenance and other school personnel. Visitors are likewise to be shown respect
and be offered assistance if necessary.
2. Public manifestations of affection, contrary to the Christian moral standards will
not be tolerated. Students are expected to exercise self-discipline and control over
matters pertaining to their relationship with the opposite sex.
3. Loitering along the corridors or outside the classroom during the class hours should
be avoided.
4. Without permission from proper authorities, students are not allowed to loiter in
the following areas: laboratories, administrative offices, and other restricted areas.
5. As the school year ends, offices or rooms assigned to student organizations/ clubs
are off-limits to students. Keys to these rooms should be turned over to proper
authorities.
6. All classrooms are off-limits to students during noon break, after the last period in
the afternoon and during weekends unless with permission from proper authority
is secured for their use.
7. Students are expected to value and handle all school equipment and other facilities
with extra care. Blackboards and other classroom equipment must be properly
used. In case of unclean rooms or defective furniture/ facilities, students and
faculty members and other school personnel are enjoined to inform immediately
the proper school authority for immediate fixture, repair and maintenance and to
prevent maximum damage.
8. Students will pay for any damage they cause to any school property.
9. Teachers/ students must see to it that lights are turned-off after using the rooms
especially after the last period.
10. Smoking is strictly prohibited inside the school campus even to teachers, other
school personnel and visitors.
11. Gambling in any form is strictly prohibited inside the school campus.
12. Bringing of alcoholic drinks into the campus is strictly prohibited. Students
caught under the influence of liquor inside the campus will be subjected to
disciplinary action.
13. Any student caught possessing, using or disturbing prohibited or dangerous
narcotics/ drugs will be subjected to stiff punishment.
14. Deadly weapons of any kind are strictly prohibited inside the school campus.

15. Posters and announcements may posted only on designated areas after the
approval of the school head.
16. Tampering of notices or documents posted for dissemination is a serious offense.
Offenders will be dealt with accordingly.
17. School ID for students and employees should be worn upon entry of the school
gate. The security officer will check the proper wearing of the school ID.
Visitors, parents/ guardians and others are not allowed inside the campus unless
on official business transaction and should register themselves in the logbook.
18. Parents/ guardians, relatives, friends, others with important transaction with a
student inside the campus are allowed only with proper presentation/ agreement
to the security officer and with the approval of proper school authority and such
transaction should be discussed at the faculty office and the requesting party
should observe proper conduct and dress decently inside the school campus.
19. Theft, extortion and other unauthorized collections or solicitations would be
charged with heavy penalties.
20. Assaulting, coercing, disgracing and other similar cases to a teacher, a school
official, any school personnel, or a student would mean a severe penalty.
21. Using of cellular phones/ mobile phones is strictly prohibited during class hours,
programs, mass, and other school activities. Battery charging of cellular phones
inside the school is not allowed.

OFF-CAMPUS BEHAVIOR
The school cannot be held responsible for the behavior and activities of students
outside the school campus. However, any complaint of scandalous behavior or misconduct
outside the campus especially those affecting the reputation of the school will bring
investigation of the student concerned.
1. Students have the responsibility to safeguard the name of the school and conduct
themselves with dignity and decency even if they are not acting in any official
capacity for the school.
2. Students on official business for the school must abide by the instructions of the
school authorities.
3. Students are encourage to perform their civic duties and cooperate with the
government.
4. Students are enjoined to keep away from the indecent places, gambling joints,
drinking bars, and other places of ill reputation.
5. For the activities that the students plan to outside the school, e.g., field trips, socials
and the like, a written permission must be obtained from the school director
observing the proper procedures.
6. The use of the school in any activity or public affair must have the prior permission
of the school head.

7. The name of the school, school authorities, and others must not be used without
prior approval from the proper school authorities in any undertaking such as
invitations, programs, printing of tickets, etc.

CONDUCT INSIDE THE CLASSROOM


1. The classroom is the academic forum in the course of instruction. It is the right of
the teacher to refuse admittance of non-bonafide student of the class.
2. Students are expected to be always in the classroom during class hours. Going in
and out of the classroom should be avoided. Unnecessary excuses of students
should not be allowed. Students are expected to do personal necessity during break
time.
3. Students are allowed only to eat at the canteen during recess time. Strictly no
eating inside the classroom.
4. Attendance of each student should be checked daily by the class monitor and by the
subject teachers.
5. Students who are inside the school campus but do not attend classes or school
functions/ activities will be marked absent and dealt with accordingly.
6. Students who are absent for medical reasons should be present a medical certificate
to support his/ her excuse letter.
7. Students who are absent have responsibility to make up for the lessons they have
missed. No special quizzes will be given. Only special examination for the
periodical test will be granted.
8. Students must not leave the room unless with valid reasons and with the permission
of the teacher. Students summoned by the school official may leave the classroom
only if an official notice or call slip is presented to the teacher. Visitors/ relatives
of students may inquire from the principal’s office or other representative with prior
arrangement with the security officer.
9. Students must sit properly at all times with feet off the furniture and the wall.
Sitting on the table and rocking of the chair must be avoided.
10. It is just proper for a student to stand up when reciting/ answering.
11. It is prohibited to write or carve on walls, tables, chairs, chalkboards, etc.
12. Chairs must be arranged properly before and after classes and litters must be
deposited into the trashcan.
13. During examinations, students must refrain from communicating with each other
and looking at their neighbor’s paper. If they do so, they are at risk of being
suspected. Cheating is extremely prohibited.
It maybe any of the following:
a. Getting or giving out information from or to another students about the
examination.

b. Use of sign language to convey or acquire information.


c. Plagiarism or copying the work of another students.

C. Student Discipline

MOMA students are expected to be cooperative and supportive individuals towards


rules and regulations of the school. They must feel that violation of rules and regulations is
not a personal matter but should be taken as a means to have order for the welfare of all
concerned.
Education is for all. It aims to develop an individual to be an authentic Christian of
his/ her community. Self-discipline is an important element to ensure of everybody. Success
is good, requires seeing the good, feeling a desire for it and being able to do it.

1. PROCEDURES OF DISCIPLINARY SANCTIONS


A. Offenses must be reported in writing to the chairman of the disciplinary
committee. The disciplinary committee shall be composed of the school head/
representative, class adviser, and the subject teacher. The Compliant must be
signed by the offended party and countersigned by the class adviser and subject
teacher. In case the misconduct or offense happened not in the presence of the
subject teacher, the signature of the class adviser and the offended student can be
accepted.
B. The chairman of the Disciplinary Committee informs the student/s concerned in
the complaint before a conference and requests a written statement of his/ her act.
C. The committee is not a court of Justice and so does not deem it proper to the
student/s concerned to be represented by a legal counsel. Parents of both the
complainant and the offender however may be invited.
D. The Disciplinary Committee imposes the proper sanction/s after due process. Due
penalty will be given and decisions will be made in writing and all concerned will
be notified.
E. In case the complainant or the respondent fails to appear at the time and place set
for the conference after the notice has been served and without sufficient
justifications, these facts shall be noted and the conference shall proceed without
prejudice to the party’s right of appearance in subsequent conference.
F. A written request for the postponement of the conference may be granted for good
cause for such period, as the end of justice and the rights of the parents to a
speedy conference require. No more than two postponements per party to the
investigation shall be allowed.

2. DISCIPLINARY SANCTIONS
3. Censure—an oral or written reprimand for violation of any specific rule or
regulation.
4. Warning
a. First warning—for all offenses minor in nature, the student is given an oral
or written warning for him/ her to amend his/ her behavior.
b. Second warning—a written notice to parents/ guardian informing them of
the misconduct of their child and consequences of it. A warning sheet
will be signed by the student, parents/ guardian and the school
authorities.
c. Third warning—the student’s parents/ guardians is invited to visit the
school to inform them of the habitual offenses of their child. The student
is subject to suspension depending on the severity of misconduct to be
implemented by the school director.
3. Suspension—a penalty for serious or habitual violation of school rules and
regulations. The parent/ guardian will be notified accordingly. The suspended
student will not attend all his/ her classes but will do school work at home, at his/
her pace to cover up the lessons supposed to be taken within the suspension
period. Suspension from five days to not more than 20% of the total number of
school days is given to the student who is proven to have committed a serious or
habitual offense.
4. Dismissal—a student is terminated from the roll and transfer credentials are issued
expect for good conduct certification. A student dismissed from MOMA will not
be readmitted. After due investigation, the offender who is found to have
committed a serious offense even for the first time, or due to incorrigible attitude
which subjected the student through all stages of status in the school is dropped
from the roll.
5. Expulsion—the penalty is an extreme form of administrative sanction which
debars the student from all public and private schools. To be valid and effective,
the penalty of expulsion requires the approval of the Department of Education.
Expulsion is usually considered proper punishment for gross misconduct or
dishonesty and or much offenses as hazing, carrying deadly weapons, immorality,
drunkenness, vandalism, hooliganism, assaulting a teacher or any other authority
of his/ her agent or student instigating, engaging in activities resulting to damage
of school facilities/ properties or injury to persons preventing or threatening
students or faculty members or school authorities from discharging their duties
from attending classes or entering the school premises, forging or tampering
school records or transfer credentials or securing forged transfer credentials. (See
Manual of Regulations for Private Schools Revised Edition 1998, Article XIV,
Section 77, p390.)

Other disciplinary sanctions:


6. Oral or written apology from the student
7. Invalidation of a quiz, examination, term paper, or any class work
8. Cancellation of a scholarship
9. Exclusion from co-curricular activities
10. Disqualification from holding or seeking any position either by election or
appointment.

11. Payment on actual damage inflicted


12. Denial of graduation privileges
13. Non-readmission to the school
14. Other sanctions may be recommended and approved by the disciplinary
committee.

3. OFFENSES SUBJECT TO MINOR DISCIPLINARY MEASURES


A. Improper wearing of uniform or not following the prescribed uniform.
B. Improper wearing of school ID, borrowing of school ID and library ID.
C. Improper or indecent dresses/ attire on non-uniform day/s.
D. Wearing T-shirt inside the polo of the boys.
E. Wearing of earrings of the boys/ piercing in any part of the body. Multiple
piercing on ears and any part of the body of the girls.
F. Loitering during class hours/ study period.
G. Eating/ making noise inside the church and library or in any restricted area.
H. Sleeping during class hours.
I. Scattering waste
J. Hair coloring/ no proper hair cut for boys/ untoward hair styling—up two 1 inches
only.
K. Quarrelling/ creating scandals
L. Shouting/ running along the corridor
M. Initiating trouble
N. No wearing of ID for three times
O. Cutting classes or going out of the school without permission
P. Internet usage to the Library instead of email research.
Q. Any other offenses similar or analogous to the above.

A. Frequent unexcused absences and tardiness
B. Gambling of any form
C. Fighting
D. Tampering and hiding the property of the school and that of others
E. Presenting forged or tampered letters, reply slip, and other letters, notices
F. Deliberately delaying or keeping letters/ notices (of vital importance addressed to
parents)
G. Repeated violation of rules on uniform/ minor offenses
H. Smoking inside the school campus or within its immediate vicinity
I. Using vulgar/ immoral language or acting indecently
J. Possessing, selling, transmitting, lighting, or exploring fire crackers of pyrotechnics
K. Cheating of any form
L. Committing an act of plagiarism (copying any literary work/ written document
form publications, research, or projects without permission from the author)
M. Truancy, escaping or skipping/ cutting classes
N. Tattooing in any part of the body
O. Marking on any school property
P. Any offense similar or analogous to any of the foregoing whether committed inside
or off campus where the offender is clearly identified as bonafied student of
MOMA.

5. OFFENSES SUBJECT TO DISMISSAL/ EXCLUSION TO BE IMPLEMENTED BY


THE SCHOOL DIRECTOR
A. Possession or distribution of pornographic materials
B. Forging/ tampering of school records or official documents
C. Stealing others property inside or outside the school
D. Drinking of any form of liquor
E. Possessing, using or pushing any prohibited drug
F. Unauthorized meddling on school’s established operating procedures without the
knowledge of the school head or collecting contributions or items for personal
purpose of others without the authorization from the administration/ head of the
school.
G. Has incurred a total of more than 30 unexcused absences
H. Inflecting injuries to others
I. Inciting rebellion, unrest, violence among students
J. Acts of lasciviousness especially within the school premises

K. Possessing of firearms, poisons, explosives, or any deadly weapon


L. Membership of any gang, fraternity/ sorority or any organization not recognized or
approved by the school authorities
M. Acts leading to public scandal
N. Wearing of the school uniform in public places like market, malls, indecent places
like drinking bars, gambling/ amusement establishments and on public functions
not sanctioned by the school.
O. Unauthorized entry in the school premises and unauthorized use of school
facilities
P. Abusive behavior or discourtesy towards school officials
Q. Repeated/ habitual offenses, students who show no reform/ change in his/ her
misconduct
R. Any other offenses similar to the above

D. Extra-curricular Activities
All bonafide students may participate in any extra-curricular activities held in the
school or outside the school provided that such participation is sanctioned or authorized by
the school authorities.

E. Student Government and Associations/ Clubs


Student government and associations are recognized in the school for the
development of the leadership skill of the students. Students are given the opportunity to lead
and manage according to their capacity. When properly guided, they can become committed
members in the community.
To be able to effectively perform their duties and obligations in their respective
offices, the following guidelines must be strictly followed:
1. Constitution and By-laws ratified by legitimate members of the organization and
shall be approved by the school administration.
2. Record of officers and members shall be filed at the records section.
3. Teachers-advisers shall be assigned to every organization, on club to stand as
coach, mentor, guide, etc. to ensure the orderly performance of the student
officers and to develop their optimum potentiality.
4. Recognition on their leadership/ performance will be given on a yearly basis during
the recognition program.
5. Program of Activities/ Action Plan shall be submitted to and approved by the
school head/ authorized representative.
6. The highest officer of the organization should ask for approval/ permission with the
recommendation of the teacher adviser from the school head in any activity they
shall undertake.
