Client personally came to the office seeking for financial assistance to augment her educational
expenses.
IV. FINDINGS:
Jouielyn Joy Muya is 19 years old, single and a resident of Barangay New
Poblacion,Buenavista,Guimaras. Client is a Person with Disability and presently a graduating student at
Guimaras State College taking up Bachelor of Science in Business Administration. With her strong desire
to finish her education she strives hard to reach her goal so that she can help her family as well. Thus,
client opted to seek financial help from the office to help augment their financial problem.
As per interview conducted to the client, she stated that her father is a sole breadwinner in the
family. He earns P 200.00/day which is on and off. That income however, is not enough to cater all their
needs much more to spend for her educational expenses. Client at present is in crisis situation that she
needs of an outside intervention to help her cope with her present problem.
VI. EVALUATION/RECOMMENDATION:
With the above findings it was found out that client is indigent and eligible for financial assistance
thus, worker recommended client to avail Aid to Individual in Crisis Situation in the amount of TWO
THOUSAND PESOS ONLY ( P 2,000.00).

to support and provide their children’s need especially her daughter MC but due to financial crisis they
find difficult where to get their daily expenses during classes. In her case, it is quit sensitive and needs
patience, understanding and care. Through sending MC in school they believed that she can establish
camaderie among others, develop her knowledge and skills too. Thus, client opted to seek financial help
from the office to help augment their financial problem.
As per interview conducted to the client, she stated that her husband is a sole breadwinner in the

almost a year. Client’s health condition is a lifetime medication and treatment. She is in need to undergo
psychiatric intervention every month at Western Visayas Medical Center. With the high cost of medicines
relative thereto, client finds difficulty where to get the amount needed for her medication. Client further
revealed that they are financially exhausted because her medicines is so costly and beyond their financial
capacity. Thus; client opted to financial assistance in order to lessen their financial problem
thus, worker recommended client to avail Aid to Individual in Crisis Situation in the amount of FIVE
THOUSAND PESOS ONLY (P 5,000.00).

IV. FINDINGS:
Emelyn Ferrer is 50 years old, widow and a resident of Barangay Sto.

Rosario,Buenavista,Guimaras. Client is a sole breadwinner in the family. She accepts laundry works to
help augment family income . She earns P 400.00/week which is on and off. Client further stated that her
2 grandson was on her custody since their parents separates for almost 5 years.
As per interview conducted, client revealed that her grandson, John Michael was consulted and
diagnosed of having T/C Cardiac Pathology. The doctor’s advised them that he is in need to undergo
laboratory exams and continuous medication for his fast recovery. Unfortunately, client cannot afford to
sustain his medical needs, therefore, client seeks outside intervention to help medical expenses of her
nephew.
Based on the above findings, client is financially incapable to augment her nephew’s medical
expenses.. Thus, the undersigned respectfully recommends that client be given financial assistance in the

vaccine.
IV. FINDINGS:
Erlinda is an 71 year old, married to Rogelio Elle. They were blessed with five children whom they
have family of their own to support. Without having a regular income the couple encountered lots of
difficulties to meet their needs. Sometimes their children supported them but it is not enough.
As per interview conducted, client revealed that she was bitten by a dog (Right Palmar Area
Category III) . Anti rabies vaccine now a days is so costly and already beyond their financial capacity so,
client seek the assistance of the office. The family at present are in crisis situation that they need of An
outside intervention to help them cope with their present problem.
Based on the above findings, client is financially incapable to augment her anti rabies vaccine..
Thus, the undersigned respectfully recommends that client be given financial assistance in the amount of
ONE THOUSAND FIVE HUNDRED PESOS ONLY.(P 1,500.00)

while her husband, Ronnie is a laborer with an earnings of P 180.00/day. That income however, is not
enough to cater all their needs.
As per interview conducted, client revealed that her daughter, Janine will be going to attend
Bacolod to attend training. Food and transportation expenses relative thereto are already beyond their
financial capacity. The family at present is in crisis situation that they need outside intervention to help
them cope with their present problem.
Based on the above findings, client is financially incapable to augment her daughter’s food and
transportation expenses. Thus, the undersigned respectfully recommends that client be given financial
assistance in the amount of TWO THOUSAND PESOS ONLY. ( P 2,000.00).
