Starting with the weighted integral form, we first split the integral into two integrals.

Then we perform
integration by parts on the first integral (it is not necessary to do it on the second integral in this case).
We define u to be the weighting function, and dv to be the term with the second derivative. Then we
find du and v using differentiation and integration, respectively. The equation on line 6 is the formula
for integration by parts. Substituting in with our values for u, v, du, and dv, we obtain the final result.
The final line is just rearranging terms to obtain the same form as in the lecture slides.
