I think the main deliverables are 1. Operational integrity: Your processes, equipment and

Michael De Klerk • I like to think that we put in this huge amount of effort in order to
produce highly resilient operations, that is operations that can react to and adapt in a
resilient manner to systemic challenges as they happen. In order to do this we must
deliver managed risk, managed change, disciplined operations and reliable assets.
Beneath all that comes all the systems and tools that we use in process safety.

Nelson Viana Pascual • My friends, I agree with your comments, it's critic matter to
adjust the system to the changes, i liked very much the term "resilient", and surely there
are other elements missing, my 3 points are my wide view of the PSM surely with lack
of detail, all have to work together, as example: sadly just this last week I have
investigation meetings with operations and maintenance at both plants for 2 incidents,
Follow Nelson resulting in one mayor injury, one minor, and we had shutdown of a generation block ( 3

units), I saw almost the same in both events, lack of training, lack of experience, excess

of confidence, supervision doesn't supervise, supervision doesn't give expectations on
safety for the work, procedures deficiency ( including safety aspects), above all this Manager
organization pressure for production and resources saving (it's ok but at what cost), CHECK OUT Entry

After providing services for all these management systems either singly or as integrated
system to so many companies over the last 20 years, without clear responsibility and
accountability, linked to performance, and real assessment objectively and consistently,
no system can work effectively and have continuou improvement. So just looking PSM
withou merging into the overall management system, accidents and events will continue
to occur and the system will never be fully effective and efficient no matter how effective
and completeness the hazards are identified and evaluated - these are all technical
matters, for effective, management system must be fully implemented.

I think you hit a bullseye, if safety management system its outside the overall
management system it'll be an orphan, now on the concept of management review of
the management system, adjust the management system i think is like moving and

If you are not having the right position and authority, or the right boss, the right culture,
etc. it is NO Way. There were companies I worked or helped, similarly, but also a few
differences, look for the few different companies - these were excellent companies
having the right atmosphere and culture, and clear responsilities, authorities, and full
support for the boss. These were the real good companies where everything go
according to established management system. Unfortunately, there were too few.
So to improve in EHS, basically, you have to start from improving the culture,
influencing the mentality on the thinking, from the top boss and from the lowest workers,
