Transitional Housing Application

Dear Applicant;

Here is the application for transitional housing that you have requested. Getting into transitional housing is a
simple process. Fill out the application and return it to the Spera Recovery Center—that’s the first step.

We will go over your application. If you meet our basic criteria, we will contact you for an interview when a
bed becomes available. It is through the interview process that we determine if you would add to the positive
recovery atmosphere of the house in which the bed is open.

Things to remember:

• Filling out the application and returning it does not mean you will automatically get a bed when
one becomes available—it only gets you on the waiting list for consideration. The average waiting
list ranges from one (1) to three (3) months.

You are encouraged to call Charles Coleman, Chapin Street Project Coordinator during business hours to
inquire about your status on the waiting list. Charles can be reached at 734.485.8725 or 734.669.8265. Leave
a message, and he will contact you regarding your status.

• We welcome you attend local 12 step meetings in the Ann Arbor area, and introduce yourself to
Chapin Street Project House Managers at these meetings. Call us for a list of meetings.

• Once on the waiting list, please call us twice a week to keep your name current on the list. Also,
keep your address and telephone number current with Charles and/or the Spera staff so that we can
contact you when a bed becomes available.
CHAPIN STREET PROJECT
Transitional Housing Rules
1. Residents are expected to abide by all local, state, and federal laws.

2. Any use, possession, or distribution of any illegal drugs, alcohol or drug paraphernalia is
prohibited and will lead to immediate discharge.

3. House Managers must approve any prescription drugs or over the counter drugs taken by
the residents. Any unauthorized prescription or over the counter drugs use is prohibited and
may lead to immediate discharge.

4. Threatening or violent behavior toward staff or other residents will result in immediate
discharge.

5. No weapons are allowed on the property.

6. CURFEW:
Sunday – Thursday 11 PM

Friday – Saturday 1 AM

All visitors must be off the premises no later than: 10:30pm Sun – Thurs

12:30am Fri - Sat

7. The only overnight visitors allowed are Dawn Farm residents (Farm residents, Dawn Farm
Downtown residents). Your House Manager must clear all overnights.

8. All visitors must stay in the common areas

9. All visitors must be pre-approved by the House Manager

10. Residents should respect the confidentiality of all other residents at all times.

11. All residents are expected to do daily chores and to keep their apartment clean.
Dirty laundry must be kept in plastic bags or hamper.

12. Being intimate or overly friendly with neighbors or residents of Dawn Farm (which includes
the Farm, Dawn Farm Downtown, Spera, Aftercare and transitional housing) is not allowed
and may result in immediate discharge.

13. Residents are to attend all house meeting.

14. All residents must attend the required number of AA meetings a week (see sheet). No other
residents from your housing site (or any other Dawn Farm facility) can sign your sheet. Your
sheets are to be brought to the weekly house meetings.

15. Telephones are for local calls only. Long distance calls are not allowed.

16. Please report any needed repairs to your House Manager immediately.
17. Residents must provide a urine sample for drug testing upon request. Denial of the request
will result in discharge. Any positive drug tests will result in immediate discharge.

18. Rooms are subject to search and inspection at any time at House Manager discretion.

19. Personal belongings will NOT be held after discharge or premature departure. Personal
belongings will be disposed of if not taken at time of departure.

20. No gambling is allowed on the premises.

21. Lending or borrowing of monies between any Dawn Farm residents is prohibited.

22. Personal belongings are not to be tampered with or borrowed at any time. Respecting other
people and their property is a part of recovery.

23. No Smoking in the apartments or houses. Smoking in our transitional housing sites can
result in immediate discharge.

24. Monies are refundable on a per diem if you are discharged or leave prematurely.

25. AA service work AND community service work is required of each resident each week.

26. Residents must work DAY shift

27. Failure to find work within 2 weeks may result in discharge.

28. Disruptive behavior may result in discharge.

29. Failure to pay apartments fees may result in immediate discharge.

30. Failure to comply with the rules may result in discharge.

31. No overnights for the first (60) sixty days at the apartments, unless an AA function—
and approved by the House Manager.

32. A $10.00 lawn care/snow removal fee (Cedar Park only) will be collected for the upkeep of
the premises from each resident, cash only.
