Ask client to hold the bad blanket while the health care provider remove the top linen.
Review chart for any limitations in physical activity and discuss the procedure with the client.
Put on gloves, if necessary. Fold the washcloth like a mitt on your hand to facilitate handling of cloth
Close curtains around the bed and close the door of the room (if necessay). Then adjust the room temperature if
needed.
Offer client bedpan or urinal. (If necessary)
Perform hand hygiene (Wear gloves if necessary).
Lower side rail nearer to you and assist client to side of bed closes to you then raise bed to a comfortable working
height.
Loosen top lines from foot of the bed; place bath blanket over the top linen.
Assess the client preference for bathing practices, frequency of bathing, time of the day preferred, type of hygiene
products used
Place soiled linen in laundry bag or hamper. If linen is to be reused, fold it over a chair. Avoid used linen from
coming in contact with your clothing.
Lay a towel across client’s chest and on top of bath blanket.
Remove client’s gown, undergarment, jewelry and keep bath blanket in place.
If client has an Intravenous (IV) tube, removed gown from arm without IV first, then lower IV container and sliide
gown covering affected arm over tubing and container. Rehang IV container and check flow rate.
Repeat procedures 26 - 29 for the nearer leg and foot. An option for the shorter healthcare provider who areprone to
back strain might be to bathe one side of the client and move to the other side of the bed to complete the bath.
Remove pillow if allowed and raise head of bed 30 - 45degrees. Place bath towel under client’s head
Raise side rail. Fill basin with a sufficient amount or two-thirds full of warm water (110 - 115 F) or as client desire.
Then have client place fingers in water to test temperature tolerance. Change water as necessary throughout the
bath.
Wash client’s eyes with plain warm water. Use different section of mitt for each eye. Move mitt from inner canthus to
outer canthus. Dry eye thoroughly but gentle.
Ask client about preference for using soap on face. Wash, rinse and dry well forehead, cheeks, nose, neck and ears.
(Men may wish to shave at this point or after bath) Clean pinna (The projection part of the external ear) with cotton-
tipped applicators.
Expose arm farthest from the healthcare provider. Place towel lengthwise under the client’s arm. Bathe arms using
long, firm strokes. Starting from distal to proximal areas (hands to axilla). Supporting arm, raise it above the client’s
head if necessary to bathe the axilla. Pat dry the arm and axilla.
Place a folded towel on the bed next to client’s hand and put basin on it. Soak client’s hand in basin. Wash, rinse, if
necessary and dry hand. Apply appropriate emollient.
Repeat actions 19 and 20 for the arm nearer you. An option for the shorter healthcare provider who areprone to back
strain might be to bathe one side of the client and move to the other side of the bed to complete the bath.
Document assessment, record and report findings..
Place a folded towel on the bed next to client’s hand and put basin on it. Soak client’s hand in basin. Wash, rinse, if
necessary and dry hand. Apply appropriate emollient.
With one hand, lift edge of towel away from chest. With the mitted hand, bathe chest using long, firm,upward strokes
for male clients.
For female client, take special care to wash skinfolds under the breast. Lift breasts if necessary. Keep chest covered
between wash periods. Dry well.
Cleanse foot, making sure to bathe between toes. Dry well.
Bring all the necessary materials to the bedside table or over bed table.
Help patient put on a clean gown and assist with the use of other personal toiletries, such as deodorant or
cosmetics..
Lower side rails. Position the client in prone or side-lying (as applicable). Keep client draped by sliding bath blanket
over shoulder and thigh. Put on gloves (if not done on step 6)
Return bath blanket to original position and expose the farthest leg. Bend client’s leg at knee by positioning your arm
under. While grasping client’s heel, elevate leg from mattress slightly and slide bath towel lengthwise under leg..
Cover client with bath blanket, raise side rails for client safety. Change water.
Lower side rails. Position the client in prone or side-lying (as applicable). Keep client draped by sliding bath blanket
over shoulder and thigh. Put on gloves (if not done on step 6)
Then wash, rinse and dry the back from neckline down to buttocks.Use long, firm, downward strokes. Pay particular
attention to cleansing between gluteal folds, and observe for any redness or skin breakdown in the sacral area. If
client can tolerate, give back massage.
Reposition the patient in supine position. If necessary perform perineal care. Remove gloves.
With one hand, lift bath towel. With mitted hand, bathe abdomen, giving special attention to bathing umbilicus and
abdominal folds. Stroke from side to side. Keep abdomen covered between washing and rinsing. Dry well.
Dispose all used supplies and return materials to its proper place.
Unless contraindicated, use long, firm upward strokes in washing from ankle to knee and from knee to thigh. Dry
well.
