IN THE

3RD DISTRICT COURT - SALT LAKE

COUNTY OF SALT LAKE, STATE OF UTAH

State of Utah

vs.
Affidavit of Probable Cause
CHELSEA WATROUS COOK
Date of Birth: 10/04/1986
Arrestee

On 11/25/2018 18:58 the defendant was arrested for the offense(s) of:

Offense Date Offense Description Statute Gov Code Severity DV

1 11/25/2018 AGGRAVATED MURDER 76-5-202 UT F1 No

I believe there is probable cause to charge the defendant with these charges because:
On November 25, 2018 at approximately 6:58 p.m., Unified Police Department responded to a shooting at the
address of occurrence. Officer entered the apartment and discovered the witness pinning his ex-wife (A/P)
against the wall. The witness alerted officers that he had a handgun in his waistband and informed officers that
A/P had just shot his girlfriend/victim. Officers found the victim with a least two gunshots to the torso. The victim
was transported to a local hospital where she eventually succumbed to her injuries.

Witness told investigators that A/P stopped by the apartment to deliver cold medicine to one of their children.
A/P entered the apartment while the witness was out in the parking lot. A/P was told to leave the apartment, but
she refused to listen. A/P remained unlawfully inside of the apartment during this time. A/P then locked herself
in the bathroom while A/P was on the phone with the 911 operator. A/P then exited the bathroom and walked
over to her coat. A/P then suddenly pulled out a handgun and fired three to five rounds at the victim. The victim
fell back and landed on the couch.

At this point, witness was able to take the handgun away from the A/P. The A/P then walked over to their
children and sat on the recliner chair while the witness tried to help the victim. The A/P then stood up and
started to walk back to her coat, that’s when the witness grabbed A/P and pinned her against the wall until
officers arrived.

Witness added that their children (three-year-old twins) were present during the entire ordeal. A/P invoked her
right to counsel after being detained by officers.
