MINI PROPOSAL

Challenge TB under coordination with District Health Office of Jayawijaya will do a 10 days Data
Collection of TB cases diagnosed and treated in RSUD Wamena - Jayawijaya District.
The objective of the activity are about:

1. To collect data regarding the number of TB cases diagnosed and treated in RSUD Wamena
2. To evaluate the proportion of unreported TB Cases in RSUD Wamena
3. To identify the associated factors.

The field visit will be planned do in July 2nd to 13th, 2018 in Wamena accompany by DHO (Head of
CDC/Section of TB and TB program Officer). Funding from Challenge TB – activity code 1.1.1 GFASS
code 0996.0030
