Extracurricular Activities
List any extracurricular activities in which you participated while in professional school.
Provide activity and dates:
Since the commencement of my time in professional school I have made sure to involve myself in a well- rounded assortment of
extracurricular activities as to maintain an ever growing repertoire of skills and experiences.

One of the most enjoyable leadership and advocacy groups I offered my time to was serving as Chair of the Rosalind Franklin
University Class of 2018 Scholl Ambassadors. Some of my happiest memories were formed serving as an ambassador for Scholl
College. I had the privilege to speak with incoming students and serve as testimony. Sharing the pride and fulfillment attained in
becoming a podiatric physician was very special to me. I was also given the privilege to organize the orientation for the 2019
incoming class and serve in leading the “big-little” match process between my class and the class of 2019. Advocacy for our
profession as a whole and partnership within our profession is very important to me and this role allowed me to play a part in
attaining that objective. (May 2015 – Currently)

I involved myself in many philanthropic organizations, including acting as a board member for the Rosalind Franklin Christian
Fellowship Mission which implemented service and activities in the Chicago area which offered service to local retirement homes
and shelters, as well as organizing fund raisers and drives to fund overseas endeavors sponsoring children in underprivileged
environments. (January 2015-Currently)

I often volunteered at the Feed My Starving Children Mission and Hands of Hope Mission in the North Chicago, Libertyville, and
Barrington areas which prepared food and resources for under served populations around the globe. (May and August 2016)

I have also served as an auxiliary staff volunteer at Kishwaukee community Hospital in DeKalb since 2012 aiding in running the
visitor desk of the hospital and stocking exam rooms throughout the hospital.

On a more casual note for the past ten years I have maintained a passion for my interests in music and furniture design and
carpentry. My carpentry skills have been put to good use, during my time spent in habitat for humanity in July 2016. I have also
remained an active musician, even currently performing as a guitarist, drummer, pianist, and vocalist in a variety of settings. (2006-

Personal Statement:
When I have free time I refuse to remain sedentary for long. What I believe truly dictates the success of a person is the motive that
drives that individual to devote him or herself to achieving their goals. It is rare that a person finds something they are truly
passionate about and it is this notion of mine that makes me a fortunate commodity. The opportunity to be a surgeon is the
opportunity to blend the apex of academia with the application and motor skills only a hand full of individuals on the planet have the
ability to deem their gifting as well as their profession. The intrigue and fulfillment I experience each time I am granted new
information or the opportunity to benefit the well-being of my community is overwhelming evidence that I have found what truly
moves me. Tending to the human spirit and well-being relieves me of any burden associated with the effort required of me to attain
the skills necessary to perform at the utmost proficient level as a physician. This is what is priceless to me. The fact that we as
humans are capable of being so intrinsically drawn to something, that naturally brings the best within ourselves to the surface is
truly profound. With confidence, I can vow to be entirely devoted to molding myself into a successful student and future contributor
to the medical field. It was the notion that in this life, medicine allows an individual to connect to another so intimately. No other
profession has the connection between provider and client like a doctor and his patients. Being a surgeon I am given the privilege to
intervene in the most empiric and intricate physical and emotional level. It is a true marvel to me the strides and discoveries we
have made in medicine throughout history, and it will be an honor to be another component of that force.

My Family immigrated to the United States from Italy. I was endowed with many opportunities my family was unable to utilize. Not
only am I the first generation in my family to be born in this country I am also the first to have attended college and have attained a
degree. The power of an education was always stressed to me growing up and only now have I begun to experience the true value
of its application. My family arrived in this country like many others with nothing but an intrinsic drive to better their lives and the
lives of those around them. This notion of always working hard and striving to bring out the best in people was ingrained in my
character thanks to the teachings of my family. Not only do I recognize this degree for its social value but more so, that it is an
extension of society’s faith in me to continue these principles that have been bestowed unto me. The notion from the start has
invigorated me and prompted me to be the best version of myself as an individual, and provider as a future member of the health
care field. Knowing I will be entrusted with the responsibility to physically open up and intervene on a pathology surgically is an
astonishing feat. As is practiced in my family, I vow to always put forth my fullest efforts in any scenario and continually strive for
excellence with the hopes to instill that trait of humanity in every individual I encounter. I will work my hardest in the years to come
to mold myself into the best podiatric physician I can be for myself and my patients, to demonstrate the extent of my appreciation. If
in 30 years I can say I did my best to uphold this promise with myself and the promise I have made to my family and all those who
have put their faith in me and shown support then I have accomplished my goals. If I can make these sources of motivation proud in
the trying years that will define who I become, I will have indeed attained my definition of success.

To Whom It May Concern:

It is my pleasure to write a letter for Mr. Giovanni Incandela in support of acceptance into a
residency program. I have had the pleasure of spending many clinic days with Mr.
Incandela over the past 5 years in my office at his request to shadow a “Podiatrist”. I am a
2003 graduate of The Scholl College of Podiatric Medicine and a 2006 graduate of The Inova
Fairfax Hospital Podiatric Residency Program. I am board certified by the American Board of
Podiatric Surgery in Forefoot and Reconstructive Rear foot and Ankle Surgery. I am
currently serving on the Board of Directors for the Illinois Podiatric Medical Association.
When I get to evaluate a student in terms of being a Podiatrist, I examine three questions.
First, can this candidate handle the work load for three years of residency? Second, does
this candidate display a positive outlook, good bedside manner and ethical attitude? Third,
does this candidate have a passion for medicine, in any form that will drive him or her to be
the best physician possible and to provide the kind of care that we all would want for our
loved ones?

I believe Mr. Incandela’s resume speaks for itself, but some of his achievements deserve
special mention. Mr. Incandela has performed well in an impressive academic undertaking
by double majoring in Biology and Health and Human Services, while also maintaining the
necessary curriculum for minoring in Chemistry and Gerontology even before entering
medical school. This trajectory and work ethic did not fall through medical school, taking on
several leadership positions and maintaining a competitive GPA, all while serving his
community through several service outlets. He is very likely prepared for the work load
required for residency.

Mr. Incandela became chair of the ambassadors for his class to help his peers and reinforce
his own knowledge simply because he loves the material. It is those among us who “simply
love” the material and nature of our work that become our fields leading researchers and
lead thinkers.

My observations of Mr. Incandela have been nothing but positive and particularly in regards
to his natural interactions with my patient’s. I have seen several students in my office and
some leave me concerned when presenting them to my private patient population, but Mr.
Incandela has a very professional and compassionate bedside manner. He maintains a
respectable appearance and my patients are quite fond of his warm demeanor and curious
desire to learn.

Mr. Incandela’s passion for medicine and the healing arts are not easily surpassed. His list
starts long before even entering medical school. He was the president of the Pre
Professional Association at Northern Illinois University. In this capacity he shared his
passion for medicine with incoming and undecided students in hopes that they will discover
in themselves what he already knows of himself. This same advocacy and pride for his
passion was again displayed in his role as chair ambassador for his class at Scholl. He was
also the President of Phi Sigma Honors Biological Society where he participated to ensure
the success of events like, “STEM fest” and “Honors Biology Convocation”. His passion for
medicine is also evident in his volunteer work at Kishwaukee Community Hospital. I have
gone on several medical mission trips and fully appreciate the duty to philanthropy that Mr.
Incandela displays.

In summary and based on my experiences with him, I recommend Mr. Giovanni Incandela in
the highest manner and believe he will not only be a top academic mind, but also a warm,
reassuring proponent for his patient’s and their families at times when they are the most ill.

If you have any questions related to this recommendation please feel free to call me at
(630) 352-3700 Ext 4. Also, feel free to email me at doug.afas@yahoo.com I look forward
to calling Mr. Giovanni Incandela a colleague in Podiatric Medicine.

Sincerely,

Dr. Douglas Pacaccio, DPM, DABPS

To Whom It May Concern:

I am honored to write this letter in support of Mr. Giovanni Incandela, he is an accomplished, dedicated
student and well deserving of your consideration.

I have known Giovanni since he matriculated here at RFUMS in August of 2014. It is clear to me, as he
always sat if the front row during my lectures, that he is possessed of a maturity, self-awareness, and
integrity that makes him outstanding among his peers. He regularly participated in class discussions and
never, to my knowledge, came to class unprepared or unwilling to play a constructive role.

In addition, I had the opportunity to observe the quality of his work in the Gross Anatomy Lab over the
course of an entire academic year. The complicated nature of this work requires a student to be
adaptable, self-driven, and persistent. He displayed these traits in excess and provided leadership for
other students in the course

I fully expect that he will continue to lead throughout his time here at Scholl College, as evidenced by his
position as the Chair of the Class of 2018 Scholl Ambassadors, and that he will go on to serve his
profession well upon graduation. I feel very confident that he has great professional promise and will
become a talented and accomplished podiatrist.

I can, without hesitation, strongly recommend Mr. Giovanni Incandela. He will undoubtedly be an asset
to your program

Sincerely,
