Atopic dermatitis 異位性皮膚炎

→FLG突變造成皮膚屏障不全，
• gene問題 (Filaggrin, FLG) 導致過敏原通透增加
• 發炎細胞激素( IL-4、IL-13、TNF) →發炎

• 脂肪、ceramides減少 →難以維持濕潤

• 環境刺激
• 可能併發細菌感染或病毒感染

Atopic dermatitis異位性皮膚炎

• 癢、紅斑、鱗狀皮膚、濕疹、滲出液、丘疹 → 異位性
• 多在出生第一年就發病 → 臉、脖子
• 30%成年仍有 →手肘、膝窩、脖子、額頭、眼睛、手
• 80%併發氣喘、過敏性鼻炎→Atopic march、 Atopic triad
• 細菌感染 → 黃色硬皮

Dry Skin (Xerosis) 皮膚乾燥

• 皮膚水分減少 → 角質層細胞不正常流失，搔癢主因

• 環境因素 → 冷風、強風、乾風、熱水澡、喝水不足

• 缺乏ceramides →角質層缺水

• 干擾皮膚角化及脫落
→角質細胞不正常成熟與附著、皮膚吸水性
• 年紀上升 物質減少、皮脂分泌減少(賀爾蒙)

• 其他原因 →長期使用清潔劑、營養不良、角質層物理
性傷害、甲狀腺機能低下、脫水

• 皮膚通透、脂肪酸穿透

Psoriasis 乾癬

• 常有對稱斑塊，且覆蓋一層銀白鱗狀皮膚，會痛或癢，
有時突起的鱗片上有點狀血點
• 可在全身多處產生，常見於手肘外、膝蓋外、腰背面、
頭皮、軀幹、生殖器
• 可能終身有乾癬也可能很快消失

Irritant Contact Dermatitis (ICD)刺激性接觸性皮膚炎

• 發炎、紅斑、腫、癢、刺、燒、痛
• 臉、手背等暴露區較易有
• 常見於特定職業 → 洗碗、做菜、剪髮 ……
• 一段時間後會結硬皮，若避免接觸刺激物，數天內痊癒
• 若仍反覆接觸刺激物，則持續發炎並產生龜裂、鱗狀皮
膚，且可能苔癬化(lichenification)或使皮膚變厚

Allergic Contact Dermatitis (ACD)過敏性接觸性皮膚炎

• 接觸到過敏原而發炎 →第一次接觸常無反應，下次才發
作 (24-48小時內)

• 鎳 (珠寶、衣服、首飾、電子產品中)、橡膠(手套、拖鞋、
鬆緊帶)、染髮劑、鉻酸鹽類(皮革、顏料、漂白劑)、甲醛
(黏膠、化妝品、清潔用品、油漆)

症狀
Allergic Contact Dermatitis (ACD)過敏性接觸性皮膚炎

• 由分布及外觀與ICD區別，但在慢性病人上不好分
• 發炎、腫脹的皮膚上常有丘疹、小囊泡，會癢，慢性也
會苔癬化
• ACD在接觸區周圍也發作 → 也由手或物品傳播

• 適應症：濕疹、皮膚炎、牛皮癬、濕疹性 乾癬、皮膚搔癢症。

• "人人" 保膚洗液
• 洗劑
• 每ml含: Calamine 80 mg、Diphenhydramine HCl 10 mg
• 用前搖勻，3-4次/天，均勻塗抹於患部
• 適應症:緩解搔癢、皮膚刺激、尿布疹
• 連續使用三星期可能併發ACD
• 對藥物個成分過敏者不能使用

• 成分: diphenyhydramine HCl 50mg
• Forms: 膠囊劑
• 適應症：緩解過敏性鼻炎、枯草熱所引起之相關症狀（流鼻水、
打噴嚏、眼睛及喉部搔癢）及過敏所引起之搔癢、
皮膚癢疹。
• Use: 一日3次，每次間隔4小時以上服用。成人每次1粒，
12歲以上適用成人劑量
• Associated side effects: 鎮靜、嗜睡
• Considerations:
• 孕婦及授乳婦不建議自行使用
• 嚴重代謝性肝腎疾病患者不得服用 77

• Selsun Suspension 舒聖懸濁劑
• 成分：selenium sulfide 25mg/ml
• 適應症：脂溢性皮炎，汗斑,頭皮屑
• 使用方法：先將頭髮弄濕，搓揉產生泡沫，讓藥品
停留在皮膚上3mins，之後用清水清洗，反复沖洗確
保清洗乾淨。
• 一個星期裡使用兩次，長期使用必須要資訊醫師。
• 注意事項：只限外用

• 舒淨洗髮精２％Ｗ/Ｗ
• 適應症：減少因黴菌所引起之頭皮屑治療之輔助劑
• 用法：搓揉按摩3-5分鐘，以清水清洗
• 注意事項：
• 長期使用類固醇治療患者，馬上改用此洗髮精可能會有
刺激現象。
• 成分過敏者，不宜食用

• 適應症:治療皮膚表淺性黴菌感染，去角質。

• 洗澡後使用Hydrocortisone 1% ointment 改善紅斑症狀
• 每天用含藥洗髮液清洗頭皮以及臉部，
均勻塗抹到頭皮後，等待5分鐘再洗掉。
• 4個星期後 慢慢將使用含藥洗髮液的次數降低至2-3次/周
• 洗髮液勿接觸眼睛&嘴巴
