PROMISSORY NOTE

FOR VALUE RECEIVED, we, ____________________, and ____________________, of

legal ages, Filipinos, and residents of Nalook, Kalibo, Aklan, do hereby PROMISE TO PAY,
JOINTLY AND SEVERALLY, to ____________________________________, the sum of FIVE
HUNDRED EIGHTEEN THOUSAND AND ONE PESOS AND SEVENTY FIVE CENTAVOS (Php
518,001.75), Philippine Currency, payable in installment at the rate of TEN THOUSAND PESOS
(Php 10,000.00) every 15th day of the month, starting on January 15, 2019, until fully and completely
paid.

In case of non-payment of this note or any portion of it, when due, or any other amount or
amounts due on account of this note, without necessity of any previous demand, the entire obligation
shall become due and demandable, with penalty interest of 2% per month of the outstanding balance,
and if for the enforcement of the payment thereof_____________________ is compelled to entrust
the same to her attorney, we will pay, jointly and severally, 25% of the total amount of obligation as
and for attorney’s fees. And we submit to the jurisdiction of both the Municipal Trial Court and the
Regional Trial Court of Kalibo, Aklan, as the case may be, in the event of litigation arising from this
note.
