Title/Description:
Care Of Patients (COP)
Applies to/concerned Areas:
All Arab Hospital departments, both medical & all other supporting
services, involved with the development of policies & procedures

Purpose:
1. To define procedures and processes that ensure patients receive the same level of
specialized or critical care provided to other patients within the hospital.
2. To ensure that patient care provided at AH is based on clear assessment and is
conducted based on an integrated care plan that is periodically updated to reflect
changes in patient condition.

Policy:
1. Arab Hospital (AH) staff shall maintain uniformity of level of care provided to its
patients.
2. Patients with the same health problems and care needs shall receive the same quality of
care throughout the hospital regardless of location within hospital based on acuity,
complexity and need for specialized or critical care and services.
3. Care planning shall be integrated among all healthcare team members participating the
care of patients.
4. Integrated care plane shall be based on the assessment findings.
Procedures:
1. Uniformity of care:
a. AH staff shall ensure that the care provided to similar patient populations in
multiple departments or setting is uniform and results in similar outcomes.
b. Department/service leaders shall collaborate to ensure that the same level of care
is available each day of the week, and in each shift.
c. Uniform patient care is reflected in the following:
I. Access to and appropriateness of emergency care and treatment shall be
provided regardless of the patient’s ability to pay or source of payment.
II. Access to appropriate care and treatment by qualified practitioners shall
be the same every day of the week and at all time.
III. Acuity of the patient’s condition shall determine the resources allocated
to meet the patient’s needs.
IV. The level of care provided to patients (for example, anesthesia care) shall
be the same throughout the hospital.

V. Patients with the same nursing care needs shall receive comparable levels
of nursing care throughout the hospital.
d. Uniform patient care shall result in the efficient use of resources and permit the
evaluation of outcomes of similar care throughout the hospital.
e. Methods for medical technology cleaning, disinfection, and sterilization shall be
coordinated and uniformly applied throughout the hospital.
f. Emergency room patients staying for extended stay due to lack of available beds
and are determined for admission to a critical care unit shall be assessed and
followed up according critical care standards until they are transferred to the
appropriate critical care unit.
g. Sedation and anesthesia procedures shall be standardized and uniform regarding
staff qualification, the medical technology, the supplies, and the monitoring shall
be the same wherever procedural sedation is provided in the hospital.
h. The hospital shall maintain a uniform medication dispending and distribution
system.
i. Hospital staff shall follow uniform documentation of clinical and non-clinical
processes and practices.
2. Care planning:
a. Hospital staff shall collaborate together and use the needs as identified in the
initial assessment and periodic reassessments performed by physicians, nurses,
and other care to meet the patient’s needs.
b. The initial care plan shall be developed within 24 hours after inpatient admission.
c. Patient care plans shall identify and establish measurable goals whenever
applicable to be the selected by the responsible physician in collaboration with
the nurse and other health care practitioners. Measurable goals shall be
observable, achievable targets related to patient care and expected clinical
outcomes.
d. The care planning goals shall be realistic, specific to the patient, and time based
to provide a mean for measuring progress and outcomes related to the plan of
care.
e. A multidisciplinary approach shall be followed in patient care planning through
f. Active participation in the daily round.
g. Direct communication among members of the care team.
h. Access to written plans and progress notes by all members of the care team.
i. Multidisciplinary meetings with or without the patient family.

j. All patient care plans shall documented in the patient’s medical record.
k. Hospital staff shall involve the patient/family in the planning process whenever
possible.
l. Patient care plans shall be revised and updated periodically and as the patient’s
condition changes based on the reassessment findings and on the achievement of
the planned goals.

Revision:
Every 2 years, or as required.
