HAZOP RECORD - MINUTES Revision 0 Final Issue

Template Revisions
Rev 1 Jun-05 First issue
Rev 2 Feb-06 Second issue. Added Node summary, Categorisation Count and print formatting
Rev 3 Mar-06 3rd issue. Added Guideword naming update macro to allow more flexibility
Rev 4 May-06 Added Logos for printing, data entry etc
Rev 5 Aug-06 Minor formatting changes for printing to PDF
Rev6 Feb-08 Added drawing list, rev history and rev info to reports
Rev7 Dec-08 Added extra columns to Summary (Node Description and name)
Rev 8 Feb-11 Minor updates to formatting, extra guidewords

General Notes
1. Don’t change the structure of the Node sheets as the Macros probably won’t work. A lot of it is based on the structure set up in Blank Node and this being the same for all Individual Node Sheets

BEFORE Workshop
1. Set up project info in sheet "Session Info" first
2. Ensure guidewords to be used are set up in guide word sheet. Only words listed can be used when recording minutes. You can add new columns of guidewords as needed.
3. Don’t leave blank rows in guideword set or blank columns between sets of guidewords.
4. Set up Lists sheet (if using). This has the Categories for the actions and the Action status
5. Set up blank_node sheet as you want it to appear - ie zoom %, hide columns etc. Also hide any sheets that you want to.

TO record minutes in WORKSHOP

1. All data entry is done in Node sheets
2. Go to sheet name "1". Select set of guidewords to be used for first node from drop down list, eg Continuous, Batch etc
3. If Node name / number needs changing (eg to "overview", "Project Discussion etc), change as appropriate. This will rename sheet automatically.
If node name already exists, error message will appear. Try a new (unique) name for the node.
4. Enter Node description and design information details as relevant
5. To add first record, select first guideword from drop down list , eg "General Discussion, High Flow" etc
6. Record minutes as usual
7. To add a new row, Select guideword from the drop down list in the line marked with an asterix *. A new line will appear. Do NOT leave blank rows
8. When finished node, press "Add New Node" Button. Fill in new node number and change guideword set as appropriate
9. DO NOT use Sort Ascending / Sort descending feature in NODE sheets it will mess up the numbering. You can use Filter

Minute ID Numbered automatically NO DATA ENTRY

Action ID Numbered automatically (number is NO DATA ENTRY
generated based on a entry being
made in the "action By Column)

PIDs Enter PID / Rev number details. Enter manually

Each P&ID on new row. One line per PID (limit of 6 - if it becomes a problem we'll change it)
Don't insert rows in this section - it will mess up the macros which are all referenced from the same row in every sheet

Node Enter number as defined in HAZOP Repeating number can't be entered - error message will come up. Choose another number
sessions.

Actions Enter details as required - For each

separate action a new row should
be defined.
Category Select from drop down list as You can set up whatever categories you like in the Lists sheet, the list automatically expands so it can be as many as
required you want.
The Categorisation Count formulas will be updated when the Update Summary macro is run from the summary sheet.

By Enter details as required Note - entry in this field triggers Action Nos. If no action leave this blank

After Workshops
Two options if a project wants to use this for action tracking :
1. Use Node sheets - in which case wee need top give them the entire spreadsheet - wouldn’t recommend this - option 2 is easier
a) Use additional columns in Node sheets for adding action status and details of action taken.
Due Date Enter details as required Can be completed after meeting
Status Select from drop down list as Can be completed after meeting
required
Details of action comp Enter details as required Completed after meeting
b) Use the summary sheet to get an overall report of number of outstanding actions by person etc.

2. Generate Summary sheet and Node Summary . Save only these into a new spreadsheet and give to project. The Node sheets form part of the HAZOP report and are a record of the workshop - provided in PDF only.
3. Update revision history in Session_Info sheet. Latest reviion will appear on report header.

To generate Summary Sheet

Click Update Summary A dialog box comes up when you This OVERWRITES any existing Summary Sheet and also updates the action category count on the Lists page
