1 SURVEY SUMMARY

Axess carried out an initial baseline survey from 29th October to 31st January 2018. This field service report
covers all lifting equipment inpsected visually and/or with the implementation of NDT methods.

One of the main work scopes during the baseline campaign is to conduct load test on DSME Fabricated
Padeyes rated 2t and below. The mentioned scope was completed prior arriving in Cape Town.

During the final leg of the transit to G.O.M, a list of quarantined lifting equipment without certification was
submitted to the Chief Mate, Senior Materials Coordinator and Crane Operator to render their assistance in
contacting the relevant lifting equipment vendors and/or manufacturers to obtain copies of the certification,
which has proven to be highly effective, as it has significantly reduced the number of non-conformance
equipment because of missing certificates.

2 INTRODUCTION

The primary objective of the survey were to carry out an Initial inspection of lifting equipment onboard the vessel
Deepwater Poseidon which was built at DSME shipyard in Geoje island, South Korea.

All inspection was carried out in accordance with Lifting Operation and Lifting Equipment Regulation, with an
Internationally recognised standard such as British Standard (BS), DNV 2.22 Standard for Certification for Lifting
Applicances and DNV-OS-H205 Lifting Operations, which are used as references to verify and justify the
requirement for the testing and examination and in overall, the fundamentals of the whole inspection process.

3 INSPECTION BASIS

3.1 Regulations
· Lifting Operations and Lifting Equipment Regulations (SI2307) 1998

· Transocean Lifting & Rigging Standard HQS-HSE-STD-01

3.2 Procedures and Standards


· Transocean Pad eye procedure HQS-OPS-EST-674-01

· DNV Standard for Certification No.2.22 Lifting Appliances, Oct. 2013

· All relevant British Standard (BS) remarked within the report for testing and examination and equipment
status for non compliances.

4 Testing and Examination

As mentioned in Survey Summary, one of the main scopes for the baseline Lifting Equipment Certification is to
load test Padeyes rated 2t and below. Load Test and NDT were carried out in accordance to Transocean in-
house procedure with DNV Standard 2.22 used as reference in relation to Padeye’s designs as well as to correlate
the Testing and Examination to an internationally recognised standard.

Apart from the above Padeye testing, Axess also carried out load test on Chain/Lever Hoists, Swing Jibs and
Thread Protector Basket based on the requirements set out as per BS 3243/BS 4898, BS 7333 and BS 12079
respectively.

5 Findings Log

All non conformance equipment may it be quarantined or rejected are reflected in Equip database. A separate
Findings Log showing a general picture of equipment provides an idea to Transocean in the event of follow ups
to remove/replace equipment which are currently in use.

6 EQUIP

End user can easily search or filter a piece of lifting equipment in Bridge EQUIP™ database. This software is
password protected and can be accessed by logging on to https://bridge.axess.no/ Once logged on, details ranging
from past and current surveys, date of inspection and equipment status can be viewed.

7 Closing Comment

Axess will like to thank the personnel onboard who have collaberated and rendered their assistance to us throughout
the duration of the baseline campaign.
