Out Of Stock (OOS)

Notification Form

This form is to be used in the event of a store OOS or product issue that prevents a demo from happening. Forms may be submitted by the
scheduling contact or brand ambassador contact.
To submit the OOS Notification Form:
1. Fill out this form and take a picture of the shelf and shelf tag.
2. Once logged into the Supplier Portal, click the feedback button in the top left of the page. Attach this completed form and picture within 48
hours of the demo.
3. Interactions will communicate the information to Whole Foods Market.

Credit requests will be considered on a case by case basis and are not guaranteed.
Reschedules will be considered based on availability
