Subject: HEALTH & SAFETY SUMMARY OF THE MONTH OF NOVEMBER 2018
“Contractor believes human resources are the valuable assets of a company, we respect social norms, cultural values, as well
as safety protocols and motivate our workforce to maintain the safe working environment to the project sites. We discourage
all kinds of discrimination on the working field and expect sincere efforts from all staffs/workers to develop reciprocal harmony
to contribute for Zero Accident Performance.”
I would like to request all our staff/workers and stakeholders to make execution phase of health and safety policy very
successful. This month remained free from injury, this trend will remain as is, in the coming days. I am thankful to our
management for the sincere support to Health and Safety Department.
Summary of all sites activities are as below:
A. Achievements:
1. Safety Team at Site- Site Health & Safety Officers at all three sites present at the field, performed the best from their level.
Health and Safety Officer regular observed and guided to all Site Health and Safety Officers to make the site free from unsafe
conditions.
2. First Aid Box- At all sites, First Aid Box and emergency gears provided and maintained.
3. Safety Signs- Shortage Safety signage is under printing process; those will be placed once Procurement & Logistics fulfill the
requirement.
4. Toolbox- Site Safety Officers have been conducting Toolbox meeting and site-specific safety orientation. Forms, and training
material and guideline provided to site staff. Altogether 38 toolbox meetings conducted during the month for all 3 sites and
1,011 workers participated in those tool-box talks.
5. Safe Hours: Total Safe Hours for all sites for the month of November is 17,752 where 2,219 workforces contributed for the
same figure. Till Date, Safe Hours for all Site is 91,945 hours as of 30 November 2018.
6. Daily Event- Site Safety Officers are monitoring on daily safety issue and reporting accordingly.
7. Environmental Audit from CMC: The Audit conducted for all construction sites from 29th to 30th November. Contractor’s site
responsible and HASO supported to Environmental Expert to accomplish her Audit providing all information required for
Audit questionnaires.
8. Health & Safety Training: Two hours’ Audio-Visual knowledge sharing program organized at Adarsha Site on 30th November.
The program was organized by the Health and Safety Coordinator of CDM Smith (CMC). Work at height and support system
was taught in the program. Site Inspector, Site Superintendent, Quality Inspectors, HASO, SHASO, were participated in the
program.
9. Weekly Meeting- Health and Safety Officer has been participating on regular weekly and monthly progress meeting to
address the issues raised by participants of the consultant.
10. Emergency Response Plan: As per the requirement of ERP, Emergency Response Team created each site, new arrivals are
also adjusting, emergency equipment has been provided at each site.
11. Reports: Weekly Health and Safety data, monthly data has been providing to CMC. As per instructed by CMC Toolbox
Meetings, Participants, Safe Man-hours, records of injury and site issue logs are including in the reports.

B. SITE ACTIVITIES
KANYA MANDIR SECONDARY SCHOOL
Site safety and security duty out of Saturday and Public Holidays remained to continue during the month. First Aid facility and
Fire Fighting Equipment made available and maintained. Foundation laydown ceremony organized at the site by School
Management Committee on 23rd November. Strip-footing concreting work completed for Zone A. Regular toolbox meeting,
compliance with PPE and safe work procedure followed. No incident, accident, and injury recorded during the month. The details
of the site data are attached as an Annex of this report in a separate sheet.
ADARSHA SECONDARY SCHOOL
Site safety and security situation during the month remain okay at ADSS, no injury, and loss recorded during the month. Brick wall
for the ramp of block no 8 completed, retrofitting work going on, rectification and roof-truss work for the same building going
on. Site Safety supported with safety material by Logistics department. Daily Health and Safety monitoring and toolbox training
conducted regularly. The details of the site data are attached as an Annex with this report in a separate sheet.
SHREE BHAGWATI SECONDARY SCHOOL
Site work activities improved well in this month at this site; Safety and Security duty remained, as usual, no accident, injury, and
loss recorded for the site. Strip footing concrete work completed on building 2A and 2B: Anti Termite Treatment work going on.
Safety induction training as well as toolbox meeting organized as per job nature and site requirement. Safety signs placed on the
construction site. The details of the site data are attached as an Annex with this report in a separate sheet.

C. C. CONCLUSION: From Health and Safety point of view the month of November was GOOD.
