Intern

This final evaluation will aid the college in evaluating the student’s performance
during the internship. Please review this form with your intern, sign it, and
return it to her.

A. What did the intern do?

1. Activities for which the intern was primarily responsible.

During Simi’s time with SSH, she played a key role in a project focused on
improving the patient experience through advancements in technology. Simi
developed and administered a survey that asked our patients a number of
questions relative to their use of technology, their social habits, and their
wants/needs about how we could integrate technology into their care. She
analyzed the data, did research on technology in healthcare settings, and
developed an impressive presentation which she presented to members of our
senior leadership team. In addition to this project, she spent time in our patient
care areas learning how we operate and talking to patients and colleagues about
their experiences. Simi also created a comprehensive budget reconciliation
workbook for my department to help us manage our monthly budgeting more
efficiently.

2. Activities in which the intern participated with others

In addition to the above mentioned project which she spent time with members of
our IT departments and patient care areas, she also attended a number of
operational meetings with me where she was able to interact with hospital
leaders, and she spent time rounding in patient care areas with members of my
team. She also had the opportunity to have 1:1 meetings with our CEO and CFO.

3. Activities in which the intern was primarily an observer.

Simi attended some meetings as a guest and not a participant, she also visited a
number of our administrative and business offices for tours and spent some time
in our Continuous Improvement (Lean) lab.

B. Overall, how would you evaluate this intern’s performance as a worker?

Appraise the intern’s approach to her work: asks relevant questions, is
resourceful, is articulate, listens well, has common sense, knows her subject,
learns from experience.

I wish all young women her age were as mature, well rounded, professional,
focused and hard-working as she is. From day one I was impressed with her
confidence and strong desire to learn. She exceeded all expectations and
became well known here – so much so that after she left this week I had senior
leaders asking where she was and when she would be back! She actively sought
feedback, was self-motivated and was always prepared. She has a very bright
future. She gets a solid A+ from me!

C. During the internship, in what areas did the intern demonstrate an increase in
knowledge and/or skills (technical or behavioral)?

One thing I really admired about Simi was her dedication to learning as much as
possible about not just the work she was doing, but about the current state of
healthcare and also how what she was learning in school connected to how we
operate at SSH. She spent a lot of time self-educating by reading publications,
and watching webinars and videos about healthcare. The project she was
working on was very foreign to her when I first presented it to her – and by the
next time she came in…she knew more than me! She left here armed with a
whole new understanding of healthcare and hospital system operations. And I
think she developed a real interest in healthcare management too.

D. Would you take another intern from the Simmons Internship Program?

Yes ____X______ No _________, because:

E. How would you evaluate this intern’s overall performance? Select the
appropriate category from the list provided, and provide a brief
explanation.

Overall Performance Criteria

Outstanding Self-starter. Thorough grasp of job and related factors.
Assumes new duties and adjusts to changed conditions quickly
and easily. Performs job functions conscientiously. Extremely
reliable and dependable.
Good Generates expected volume of work. Often exceeds what is
expected. Requires occasional assistance. Reliable and
dependable.
Satisfactory Adequate grasp of essential and related factors. Generates
required volume of work in accordance with prescribed
standards.
Needs Minimum acceptable performance. Definite need to show
Improvement improvement in near future or would be demoted or dismissed
from position. Often requires supervision.
Unsatisfactory Volume and quality of work frequently below standard.
Continued performance at this level would definitely be a cause
for dismissal. Shows little desire to improve.
I think my explanations above say it all – but in summary…Simi is a gem! I’d hire
her in a heartbeat. As a Simmons Grad myself (as was my Grandmother) I may
be a bit biased, but over the years I am always impressed with the many
students I have worked with and mentored from there – but by far Simi has been
the most impressive.

Thank you for the opportunity to work with Simi.

Thank you for your assistance in furthering the education and training of this
student.
