6. Endometriosis
Endometriosis and infertility

Endometriosis is a common disease affecting 10% of
reproductive-aged women. Its chronic inflammatory condi-
tion causes dysmenorrhea, dyspareunia, chronic pelvic pain and
infertility. The pathology of endometriosis and its inflammatory
reaction makes 50% of women with endometriosis are infertile,
The different theories are discussed with different Diagnostic
approaches and theses provides insights into possible mechanisms
of infertility, especially in women with milder forms of the disease.
Endometrioma or its treatment may lead to reduction in the
ovarian reserve that can have a further reduction in the fertility.
The ovarian reserve is affected either by the long term chronic
inflammatory effect of endometrioma or the surgery of ovarian
cystectomy. Ovarian reserve plays an essential role in the success
of management. Treatment of endometriosis associated with infer-
tility, includes surgical management which is usually laparoscopic
is cost-effective and offers the potential for improvement in cycle
fecundity. Many of the medical treatment may interfere with
the management of infertility and ovarian stimulation and may
unnecessarily delay further fertility therapy. Medical management
has not been proven outside of in vitro fertilization.

European society of contraception and
reproductive health care – educational project

Introduction: The European Society of Contraception and
Reproductive Health Care is a multidisciplinary society dedicated to
research, education and training, implementation and advocacy in
the field of contraception and reproductive health care for women
during their reproductive years. Its members are physicians, mid-
wives, nurses, social workers, psychologists.

As part of the educational commitment ESC is developing an
educational program aiming at disseminating knowledge, skills and
women friendly attitudes across Europe.
Methods: A group of experts evaluates different training pro-
grams existing in Europe including the program of the UK Faculty
of Sexual and Reproductive Health, the Master program of the
Spanish Society of Contraception and at the same time uses the
Delphi methodology to define the content of the knowledge part,
the specific communication and technical skills to be taught, and
the attitudes to be promoted.
Results: The knowledge part of the program includes basics
(reproductive physiology, STIs, Sexual health, epidemiology,
demography, ethics); contraceptive methods (mechanism, efficacy,
safety, tolerability, additional benefits); contraceptive counselling
and care for women with medical conditions; contraceptive coun-
selling and care in different life phases (adolescence, postpartum,
premenopause).
The skills part includes technical skills (insertion of IUD or
Implant), patient centered communication, directive counselling
and shared decision making.
Awareness of attitudes shall be trained by case discussion and
role play.
Conclusion: The Educational Project of ESC can serve as a model
for multiprofessional training in contraception, sexual and repro-
ductive health across Europe and thus serve as a basis for a future
exam.

Sexual rehabilitation after gynaecological
surgery

Introduction: Gynaecological surgery can impact female sexual
response on various levels:
Physical: Destruction of genital organs, disruption of nerve sup-
ply, dysregulation of the endocrine response, visible disfigurement,
disability and pain.
Psychological: Change in body image, anxiety, depression.
Interpersonal: Withdrawal, difficulties in communication,
changes in partner dynamics.
