HOW TO QUALIFY:
 Must be a bona fide institutional regular member of the PHA with updated membership payment/in good standing as represented by a fully
paid delegate to the event.

● There is no purchase requirement for a raffle ticket to qualify as this is a thanksgiving gift by the PHA exclusively for registered, paid and in
good standing member institutions.
HOW TO WIN:
● Raffle stubs should be legibly filled out with the Hospital’s Name and registered delegate/Hospital Representative’s Name. Blanks shall
invalidate the raffle entry.
● Drop filled out raffle stub in the designated official drop boxes located at the following :
● 1. at the exit of the Tent City Exhibit area
● 2. outside the COMELEC voting area at the Millennium Salon
● 3. Other locations (to be announced).
● Do not lose your Key to Wheels promo card. Fill it out completely. It will be subject to validation/verification if and when your raffle stub is
drawn.
● Participants are required to visit the exhibit booths of the 8 MAJOR SPONSORS at the Millennium Hall (MH) and the Tent City (TC), namely:

IMPORTANT: All boxes of the 8 major sponsors must be stamped 100%
● Participating hospital’s representatives may visit any 10 of the 47 exhibit booths at the MILLENIUM HALL to have the blank boxes appearing
on the Key to Wheels promo card stamped by the exhibitor’s representative as proof of visit. PHA shall provide the exhibitors with the
official and customized PHA STAMP.
● Participating hospital’s representatives may visit any 24 of the 121 exhibit booths at the TENT CITY to have the blank boxes appearing on the
Win the Wheels promo card stamped by the exhibitor’s representative as proof of visit. PHA shall provide the exhibitors with the official and
customized PHA STAMP.
● Time of Raffle: After the Closing Ceremonies on 16th November, 2018.
● Once a raffle stub is drawn, the registered delegate/hospital representative must present his/her Win the Wheels promo card. The PHA
Secretariat shall then subject it to final verification for compliance of all the requirements. In the event of any noted deficiency, another
raffle stub shall then be drawn and subjected to the same verification process.
● Validation process of the raffle stub: 1. All information are duly filled out; 2. Hospital member is in good standing, 3. the promo card has a
total of 42 STAMPS (no signature allowed)(All 8 Major Sponsors , any 10 of the Millennium Exhibitors other than the major sponsors , any
24 of the Tent City Exhibitors other than the Major Sponsor) and 4. the person claiming the prize is the registered delegate/hospital
representative as indicated in the drawn “Win the Wheels” card/stub (no proxy).
HOW TO CLAIM YOUR PRIZE:
● The PHA shall issue a “Certificate of Prize” to the GRAND RAFFLE INSTITUTIONAL WINNER that can be presented to the Toyota Abad Santos
Manila Branch.
● All obligations other than the payment of the BIR final tax on winning, pertaining to the TOYOTA HI-LUX, e.g. LTO registration, insurance, etc
shall now be the car owner’s (WINNER) responsibility.
● For full enjoyment by the winner of the Grand Prize Win the Wheels, the corresponding “final tax on winning” shall be paid for by the PHA
(BIR Form 1601F).
DISCLAIMER:
Dropping the raffle stub into the drop box is sufficient manifestation that the registered delegate/hospital representative agrees to have read,
understood, and commits to willfully abide by the herein promo mechanics of the KEY TO WHEELS grand raffle promo.
The PHA undertakes to abide by the grant of the “Certificate of Prize” in the name of the organization’s Institutional Regular Member Winner and
shall not entertain any claim to the prize by an individual or non-regular PHA Institutional Member.
