SCOPE AND PURPOSE

This procedure describes the hazardous energy control procedures that must be performed in order to
protect workers potentially exposed to unexpected energization or release of stored energy which could
cause injuries during the servicing or maintenance of machines, equipment or systems, as well as while
working on or near exposed de-energized electrical conductors and parts of electrical equipment.
The purpose is to provide effective controls against the release of hazardous energy of all types, including
but not limited to:

This procedure applies to all CONSORTIUM Members and their Low Tier Subcontractors on the Angola LNG
Tanks Construction project.
Where the words “shall” or “prohibit” are used then the subject shall be mandatory. Any deviation from this
procedure or part of it shall require written permission from the CONSORTIUM Project Director or his
Deputy.

LOTO/ISOLATION: Lock Out / Tag Out
Installation of lock(s) and tag(s) on the energy isolation devices to
ensure that work can be performed safely. The lock(s) and tag(s) ensure
that the energy isolating device(s) and the machine, equipment or
system(s) that they isolate and/or control cannot be operated until the
lock(s) and tag(s) are removed.
ENERGY SOURCE: Any source of hazardous energy or materials. Energy sources include,
but are not limited to; electrical, mechanical, hydraulic, pneumatic,
chemical, radiation, and thermal energies, as well as various forms of
potential energy such as that stored in springs, compressed gases, or in
suspended objects (gravitational).
ENERGIZED: Connected to an energy source or containing residual or stored energy.

DE-ENERGIZED: Disconnected from all energy sources and not containing residual stored
energy.

ENERGY ISOLATION A device that prevents the transmission or release of hazardous energy
DEVICE: or hazardous materials. Examples include, but are not limited to;
restraint blocks, electrical circuit breakers, disconnect switches, slide
gates, slip flanges, or line valves. For LOTO purposes, isolating devices
that provide visible indication of the device’s position are desirable.
ZERO-ENERGY STATE: The lowest achievable energy state (i.e. discharging capacitive and
inductive elements, blocking or totally releasing of mechanical kinetic or
potential energy, etc)
LOCKOUT: The placement of a lockout device on an energy isolating device in
accordance with an established procedure, ensuring that the energy
isolating device and the equipment being controlled cannot be operated
until the lockout device is removed.
TAGOUT: The placement of a tagout device on an energy-isolating device, in
accordance with an established procedure, to indicate that the energy-
isolating device and the equipment being controlled may NOT be
operated until the lockout device is removed.
LOCKOUT/ISOLATION A device that utilizes a positive mean such as a lock, either keyed or
DEVICE: combination type, to hold an energy isolating device in a safe position to
prevent the energization of a machine, equipment or system. Other
Lockout Devices include dead ends (blanks), bolted slip blinds, valve
wheel covers, and chains/lock. All locks require a Danger Tag.
PICW: Person In Charge of the Work.
AP: Authorized Person.
IO (MIO/EIO): Isolation Officer (Mechanical Isolation Officer/Electrical Isolation Officer).
IC (MIC/EIC): Isolation Certificate (Mechanical Isolation Certificate/Electrical Isolation
Certificate).
PTW: Permit To Work.

4. RESPONSIBILITIES

4.1 Consortium member project manager


CONSORTIUM Member Project Manager shall continuously promote adherence to objectives
and requirements of this procedure throughout the Project Team. CONSORTIUM Member Project
Manager shall provide enough resources for its accomplishment.

4.2 Consortium member construction manager


CONSORTIUM Member Construction Manager shall:
• Ensure that the requirements contained in this procedure are implemented for work activities
involving LOTO in every stage of the project.
• Provide enough resources for its accomplishment.
• To ensure that training is provided to the workers according with their roles in LOTO
operations.

4.3 Authorised person (ap)
The AP shall:
• Verify the need of isolation.
• Authorize IO to put in place the necessary isolation.
• Verify the isolation completion.
• Participate to issue the PTW and Isolation Certificate.
• Verify the job is completed and the PTW with the Isolation Certificate is sign off.
• Authorize IO for removal of isolation.
• Ensure the PTW and isolation works are correctly recorded in a Lockout register.

4.4 Person in charge of the work (picw)


The PICW is the person who is in charge of the work, or actually carrying out the work himself.
He shall ensure that:
• Verify the isolation works.
• The PTW with appropriate certificate and JSEA is correctly and accurately completed and
validated prior to start the work.
• All conditions and precautions specified in the PTW are observed.
• The correct PPE has been issued and is worn or used, the proper preventive measures have
been implemented.
• The actual conditions of the PTW have been met.
• The PTW is displayed in a PTW holder at the workplace.
• All used materials and safety equipment have been cleared from the area.
• After completion of job, PTW is sign off as job completed.
• The copy of the PTW is returned for close out
• All the team members are aware of the PTW contents particularly regarding the work
associated hazards.
• Ensure that all team members involved in the isolation work are skilled and trained.

4.5 Isolation officer (io)


Specific responsibilities are:
• To support the LOTO training whenever necessary.
• To participate in the evaluation of possible energy sources on or near the machines,
equipment, or processes.
• To isolate all energy sources from a system, machine or equipment.

• To verify safety conditions before work is performed.
• To conduct a shutdown of the equipment to ensure that all energy has been isolated and the
machine is safe for operating.
• To place lock(s) and tag(s) on the appropriate zones.
• To ensure that the equipment could be turned on after the removal of the isolation devices.
• To remove the isolation devices and sign off the Isolation Certificate.
• To maintain the LOTO Register updated at all time.

4.6 Consortium member hse manager


CONSORTIUM Member HSE Manager shall:
• Provide assistance on HSE subjects.
• Audit for compliance.
• Verify compliance during inspection on field.
• Update and re-issue procedure based on regulatory revision.

4.7 Affected employees


Their responsibilities are:
• To know that, in no case, their job could be performed under LOTO operations
• To understand the meaning of LOTO
• To verify that no LOTO permit is applied on the equipment/machine they are about to
work on before starting the job

5. PRINCIPLES

Isolations shall be applied to all live mechanical and electrical systems that are to be worked on.
No individual shall attempt to start, energize, use, or operate a piece of equipment that has been locked out
and tagged out after the safety condition checking has been completed.
No individual other than the IO who placed the device and tag shall attempt to remove it, except as noted in
section 6.1.5.
All locks and keys to be used shall be stored in the appropriate shops with exception of each IO personal
lock, which will be controlled by him/her. Every stored lock device shall be identified by unique number
and/or color. If any lockout keys are lost or misplaced, the affected lock(s) shall be removed utilizing the
master key. If the lost or misplaced keys are not found, the lock shall be destroyed. A new PTW with the new
lock shall be issued. Master keys shall be under the responsibility of the CONSORTIUM Site Manager or his
designee.
The personal lockout and any safety tag signifies that it was installed by that task’s Isolation Officer.
The IO safety tag is reserved for the exclusive use of the authorized worker identified on that tag. The
identifying markings shall be marked permanently.
No one shall authorize another person to ignore or violate this procedure.

No person shall remove a lockout device when an unsafe condition exists, until they have corrected the
condition or another person has installed a lockout device.
No IO shall install a LOTO on any system without previous notification to the implicated operational area
Supervisor and/or the Authorized Person. This measure will ensure that operating personnel know the status
of its equipment/systems. For the same a LOTO register shall be implemented and maintained updated.
A check valve cannot be used as an energy isolation device.
When electrical ground system needs to be applied it shall be the last device applied and the first device
removed. ONLY Electrical Isolation Officer such as qualified electrical personnel shall install grounding
devices.
All employees shall receive the appropriate level of training based upon their LOTO duties.
Any employee who observes any apparent violation of this procedure shall immediately notify his Supervisor.

6. PROCEDURE

A JSEA with the PTW shall be established to lockout the energized device. Each Lockout/Tagout operations
shall be archived into the Lockout/Tagout Register by each CONSORTIUM Member. The Lockout/Tagout
Register is very important and allows an overview of all isolation works on site for a better coordination and
risk reduction.
Refer to the Attachment 5: Example of the Lockout/Tagout register.
The steps outlined in this section will serve as a guide for the accomplishment of the requirements of this
procedure.

6.1 General procedure

6.1.1 Lockout/Tagout
Step Required Action
Preparation and Notification: The IO in conjunction with the PICW will
determine all potential sources of hazardous energy and then develop a the
PTW for isolating the equipment.
For every LOTO operations, isolation details (Part 5 of the Certificate) shall be
filled in by Isolation Officer and approved by PICW.

Appropriate system lockout / tagout device(s) shall be provided by each
CONSORTIUM Member.
After that the IO shall identify all the affected employees and notify them that a
lockout and/or tagout system is going to be utilized and the reason for its use.
This notification can be verbal.
Shutdown: If the machine or equipment is operating using the safe and normal

stopping procedures.
Isolation: The IO shall physically locate each energy isolating device that is
needed to control the energy of the machine or equipment and operate it in the

proper order listed on appropriate certificate (Part 5 of the Certificate) to isolate
the machine or equipment from the energy source.

Step Required Action
Lock and Tag Application: The IO shall attach his/her personal lock, and
properly sign and date his/her personal tag to the energy isolating device and
hold it in a “safe” or “off” position. After removing each key from the lock, the

authorized employee will keep the keys and must prevent them from being lost
or misplaced. The PICW in conjunction with the IO will ensure the proper LOTO
system application.
Verification of Isolation: When all energy isolation devices have been
properly de-energized and locked/tagged out, the authorized employee shall
verify the mechanical integrity of the energy isolation device and the de-
5 energization of the machines, equipment, or processes (zero-energy state).
Perform required tests specific to the machines, equipment, or processes, to
ensure that isolation has been achieved and that any stored hazardous energy
is relieved.
Keeping a Lock and Tag in Place: Every person working on the equipment
6 shall keep his or her personal lock and tag on (in some cases only one tag may
be sufficient) until he or she has finished working or near the equipment.
7 Perform the required work safely (if required)

6.1.2 Return to service


Upon completing the work, the authorized employee shall take the following steps to restore the
energy to the machine or equipment:

When all work on the system is completed and prior to energize the system,
1 the IO shall ensure that all machines, equipment, systems, and areas are clear
without personnel and equipment.
Check to ensure that machine or equipment components are operationally
2 intact, and there is no mechanical hazard. If any doubt, the PICW of the work
activity will be asked for help in this determination.
Notify operators and other affected employees that the machine or equipment
3 is ready to be energized and the lock and tag is going to be removed. The
PICW shall also be involved.
The IO shall remove their personal locks and tags from all energy isolation
devices. The system or equipment shall be energized in the proper order noted

on the Isolation Certificate (Part 5 of the Certificate) by the Isolation Officer.
The system shall then be put back into service as necessary.

A lock or tag shall not be removed by anyone other than the individuals who installed it. See
section 6.1.5.

6.1.3 Exceptions to procedure


There are occasions where specific isolations are not required, specifically when the following
elements exist:

• The machine or equipment has no potential for storing residual energy or re-accumulation of
stored energy after shut down which could endanger employees.
• The servicing or maintenance does not create hazards for other employees.

6.1.4 Alternative energy control methods


If an energy isolation device is physically incapable of accepting a lock, a tag-out system shall be
used which will offer full employee protection similar to that of a lockout system.
The tag-out system includes all of the steps of this procedure except the actual use of a lockout
device on that particular energy isolation device.
Tags shall be placed where they are clearly visible to potential machines, equipment, or
processes users and shall indicate that the operation or movement of the energy-isolating device
is prohibited.
Additional means to be considered as a part of the workers protection shall include the
implementation of additional safety measures such as the removal of an isolating circuit element,
blocking a controlling switch or the removal of a valve handle to reduce the likelihood of
inadvertent energization.

6.1.5 Removal of locks and tags by other persons


There may be times when the isolation device needs to be closed out to put equipment back into
service when an IO still on the LOTO is off-site and cannot be located.
Removal of an IO lock out and tag out without his/her signature shall require a review by his/her
CONSORTIUM Construction Manager and a physical register (Refer to the Attachment 1:
Alternative authorization for removing locks and tags ) shall be filled.
The Isolation Officer’s Supervisor shall attempt to reach him/her to determine if the LOTO may be
closed. If the IO indicates that the LOTO may be closed, the IO shall return to the worksite to
follow the normal LOTO removal procedure.
If the authorized employee cannot be contacted, cannot return to the facility, or is incapacitated,
the CONSORTIUM Construction Manager in conjunction with the CONSORTIUM Member HSE
Manager may authorize removal of lockout and/or tagout device(s). In that case all potentially
affected employees shall be notified; moreover a HSE Supervisor at least shall be involved in the
lock removal process.

6.2 Mechanical isolation

Refer to the Attachment 2: Mechanical Isolation Certificate.

6.2.1 Raising a Mechanical Isolation Certificate (MIC)


When it has been identified that work on an isolated system, the PICW shall request a
Mechanical Isolation Certificate to the Mechanical Isolation Officer. Once this has been raised
then the Mechanical Isolation Officer shall complete the Isolation Certificate.

The following information shall be noted:

• The number and type of PTW for which the isolations are required

The MIC shall be attached to the relevant PTW before starting to work on an isolated system.

6.2.2 Removal of isolation and cancellation of the MIC


The PICW shall issue a new PTW and submit the PTW to the AP. The MIC shall be signed off by
the IO and the AP when the work on isolated system has been completed. The MIO shall remove
the isolations. The lockout register shall be updated accordingly.

The AP signs the completion of work section of the PTW to close it.

The AP shall verify that the work is complete and ensure all copies of the MIC, the LOTO
identification label along with the accompanying PTW are returned to the CONSORTIUM Permit
Officer for record.

6.3 Electrical isolation

Refer to the Attachment 3: Electrical Isolation Certificate.

All electrical isolations that need to take place shall be carried out under the authority of a PTW.
The EIO shall identify the necessity for isolations and the method of isolation on a system.
Where there are multiple isolations, reference shall be made to the relevant identification
drawings. If multiple personnel are to work on a system, then a LOTO BOX shall be used
whereby the key to an isolation padlock and a copy of the Isolation Certificate shall be placed
inside the LOTO BOX and the box secured by the use of personal danger tags and individual
padlocks.

6.3.1 Raising an Electrical Isolation Certificate (EIC)


An application for PTW shall be raised by a PICW indicating that work on isolated systems is
required to the Authorized Person. The AP after ensuring the need for working on isolated
systems will authorize the EIO for the necessary isolations. After verification of the PICW and the
EIO, they shall sign on the EIC then the work could start.

6.3.2 Removal of isolation and cancellation of the EIC


The PICW shall issue a new PTW and submit the PTW to the AP. The EIC shall be signed off by
the IO and the AP when the work on isolated system has been completed. The EIO shall remove
the isolations. The lockout register shall be updated accordingly.

The AP signs the completion of work section of the PTW to close it.

The AP shall verify that the work is complete and ensure all copies of the EIC, the LOTO
identification label along with the accompanying PTW are returned to the CONSORTIUM Permit
Officer for record.

6.4 Isolation identification label


Each of the points of isolation shall be identified by a LOTO identification label, which shall be
attached to the point of isolation at the time when the isolation is applied and removed when the
de-isolation takes place.
Refer to the Attachment 4: Example of isolation identification label “DANGER - DO NOT
OPERATE”.
