1. Introduction
Currently, student clinic of Wolkite university are still store patients record by using paper or card
manual system and also laboratory result was report manually. Nurses need to manually write
down the patient’s information and index the patient medical card. Then, these medical cards are
kept on the organized racks or cabinets. Also laboratory report was throw after it can used this
made that the medical treatment information not keeping well. These works are troublesome and
plaguing. Moreover, patient’s information is not secure.
Now a day, there are several changes in health care sector that leading more doctors to consider
the impact of software’s in their practices. The proposed system is attempted to solve the clinic
service and records management problems that are facing in this student clinics.
Student clinic information system (CIS)is computerized patient record, treatment information,
laboratory report and medicine information system. The main purpose of the system is to reduce
the burden of doctor and nurses and improve the patient record and information management
system. The CIS integrates electronic health record, patient card record, medical lab report,
medicine data, chart of schedule and data consolidation /reporting components that enable clinics
to provide patients with quality care in timely and cost-effective manner. CIS is central repository
of information that can be updated and accessed electronically within a clinic, allowing sharing of
vital patient information between nurses and doctor with security password access. This proposed
system will adopt the system development life cycle as its methodology.

1.1. Statement of the problem


The paper -based system currently in use cause many problems to the user. This kind of paper –
based system is tedious and plaguing. There are a few problems that have been a risen by using
manual system. they are
 Insecure
The medical card is easily exposed to unauthorized user. They can easily get the vital
patient information from clinic because the medical cards are just kept on the rack
without any security rock.
 Time consuming
By using medical cards, times are wasted when the medical card need to pass from
the nurse to doctor and then to dispensatory. Besides that, clinic also needs to spend
times to organize the medical cards from time to time.
 Space
Clinic needs to provide space to store these medical cards. When the quantity of card
increases every year, they need more and more space to store the cards.

 Redundant information
Sometimes a patient can have more than one medical card. This happen when the
patient forgot whether he/she have been visit the clinic or not and people who do
registration did not check properly and just directly use anew medical card.

 Limited capacity
What can be written on the medical card is limited, doctor cannot include other
related information in the card. The card just includes some basic patient
information, diagnosis and simple treatment information.
 Much resources consuming
The slip used for laboratory report and medicine should be throw after usage. This
means it could consume much resources of paper.

1.2. Objective of the project

1.2.1. General objective


The purpose of this project is to change manual clinical service and information to computerized
and web based system.
1.2.2. Specific objective
To achieve the above general objective, the following specific objectives is listed
 Designing a system that enable the organization an easy clinical service monitoring
 Capable of supporting patient ordering and laboratory inventory
 The lab reports and call for card outlined online.
 Card store, doctor report and lab documentation was store on server online.
 To develop electronic health record.
 To facilitate Laboratory information and report
 To facilitate Patient card, treatment and emergency information
 To develop Pharmacy information and medicine record
 To facilitate various report generation.

 Clinic workers schedule and attendance.

 Emergency and referral report generation and reporting.

 To monitor the health status and checkup.

 Drug store inventory and monitoring system.

1.3. Significance of the project
Now a day, everything is computerized, there will be less paper used for keeping records in the
future. There will be a paper -to- digital transition happen. People can have spent less time
searching through file cabinets and racks. They can access the patient information in seconds with
the increase of efficiency of technology. It also eliminates the need of writing and rewriting the
patient information and transcription errors.
Besides that, the privacy and security of the patient personal information is also a significance of
research in this project. The proposed system need to ensure the system is secure enough to prevent
any unauthorized person access to record. The other benefits of the system is to computerized the
current paper-based system, which can automate the process of patient record management,
appointment scheduling, electronic report generating and others. The comprehensive components
enable efficient management of patient record. The proposed system is designed based on the
problems and requirements gather from the target user by interview them.

1.4. Scope
The proposed system is to be used in Wolkite university student clinic. The targets users of the
system are doctors, nurses or any office workers in the clinic. This project is mainly emphasized
on the developing a system for storing electronic patient record and report generating. It also
includes some others functions that can help the target users to improve performance.
Basically this proposed system can be divided in to two modules.

 Electronic patient record module


 Reporting and chart schedule
module
 Patient and workers’ information
 Patient treatment information and card monitoring
 Detail drug store and pharmacy management
 Health training monitoring system.
 To assign ambulance and Bus service
 work of laboratory and experiments
 student patient health checkup and following information
 Emergency and health status information
 Have control over the café of student on the level of food
 Clinic workers schedule and
attendance management system.

The first module is electronic patient record module. This is a database module of the proposed
system. It keeps the patient information and their medical record. The medical record included the
patient medical history, previous diagnosis record and previous treatment with medicine slip. The

second module is emphasizing on detail reporting /laboratory report, referral report and chart of
schedule information that needed by doctor. It provides an easier way to auto-generate report from
existing stored information.

1.5. Feasibility study


1.5.1. Technical feasibility
 The system will be developed by using technologically system development
techniques such as visual studio php without any problems and the group members
have enough capability to develop the project. So the system will be technically
feasible.
1.5.2. Economical feasibility

 The project has being passed through different financial and cost examination. Due to
this it has a good tangible and intangible benefit.
1.5.3. Operational feasibility
Surely the system will provide accurate, active, secured service and decreases labor
of workers and save time and also it is not limited to particular groups or body. To
make the system is efficient we tried to use most available resource of the university
that operates in good manner
1.6. Limitation of the Project
 Because of the proposed system is web based, it can’t be accessed
without network connection.
 This system will be comfortably used by only computer literate people
 May be affected by performance of external factors such as finance etc.
 The system depends on electrical power and network connection.
