1- Finalized Form layout with adherence to corporate identity.

In JIRA out of 5 forms only 2 layouts attached shown is in portrait format. But mentioned
different titles for COI forms:
1. Certificate of insurance
2. Policy of insurance
3. Marine Insurance Policy
4. Premium Invoice
5. Certificate of Insurance and Premium Invoice

As per now the form is not as per CI guideline. Below is attached pdf with highlighted section
that are not as per CI guidelines.

Please confirm if we can use old layout or provide new layout which is as per CI guidelines.

2- Form Triggering mechanism and it is setup.

Trigger would be same as invoice.

3- Are the output methods defined clearly? i.e. Screen, Print, Fax, Email or Any other outputs mentioned?
Need clarification on how all the forms are triggered via print/mail- if the output types will be
different for all the form then we need 5 output types to be defined.

4- Field mapping of each field which are on output.

Field mapping is required from functional in detail. Please check the attachment.

5- Custom driver program or is there any standard driver program?

Is there any standard program is present to trigger the form or do we need to make a custom
program? Please confirm

6- Restriction of one time print and later it should be Test.

Need clarification.

7- As communicated, We would be developing the form in English and later it would get translated into other
language. Are language translations and currency conversions required?

8- Development and execution dependencies and the steps of execution

Please provide.

9- Standard text to be used are addressed?

Need details on how many standard texts are required and where we need to provide it.
