PETITION

Undersigned Petitioner, unto the Honorable Court, most respectfully

states and avers: THAT

1. He was appointed as Notary Public for the City of Pagadian and in

the Province of Zamboanga del Sur until 31 December 2018;

2. He possesses the eligibility for commissioning as Notary Public as

shown in his personal circumstances:

3. Petitioner has satisfactorily completed the required course in law at

JH Cerilles State College- College of Law, Pagadian City Campos,
Pagadian City in April 2014;

4. He passed the 2016 Bar Examinations and admitted to practice law

in the Philippines on 23 May 2017. Copy of the Certificate of Bar
Membership is herewith attached as Annex “A”;

5. He is a member of the Philippine Bar in good standing with clearance

from Integrated Bar of the Philippines, Zamboanga del Sur Chapter,
and court clearance from the Regional Trial Court 9th Judicial Region
 Multi Sala Station, Pagadian City and was issued certifications.
Hereto attached as Annexes “B” and “C” is copy of said certification;

6. The undersigned has not been charged nor convicted in the first of
any crime involving Moral Turpitude. Hereto attached as Annex “D”
is copy of Petitioner’s NBI clearance;

7. He desires to be re-commissioned as Notary Public for the City of

Pagadian City and in the Province of Zamboanga del Sur and
possesses all the qualifications and none of the disqualifications;

8. In compliance to the requirements of the Honorable Court for the

Notarial Commission, attached are the following annexes and made
as integral part of this Petition:

a. Three Specimen of his Official Signature as Annex “E”;

b. Three (3) pieces size I.D. pictures as Annex “F”;
c. True impression of Notarial wet and dry seal made pursuant to
law as Annex “G”;
d. Copy of the PTR with O.R. No. _____________________ is
Annex “H”;
e. Copy of the IBP O.R. No. __________________ is Annex “I”

PRAYER

Wherefore, premises considered it is most respectfully prayed of the

Honorable Court that Petitioner be reappointed and re-commission, after
hearing, as notary public for the City of Pagadian and in the Province of
Zamboanga del Sur.

Other just and equitable relief is likewise prayed for.
