CONSTRUCTION OF TWO-STOREY ACADEMIC BUILDING (EVSU MAIN-CAMPUS)


CHECKLIST OF TECHNICAL & ELIGIBILITY and FINANCIAL REQUIREMENT FOR
BIDDERS

Technical & Eligibility Component Envelope


The Technical & Eligibility Component Envelope shall contain the following:
Class “A” Documents:
1. Registration certificate from the Securities and Exchange Commission (SEC), Department of Trade and
Industry (DTI) for sole proprietorship, or Cooperative Development Authority (CDA) for cooperatives, or any
proof of such registration as stated in the BDS;
2. Mayor’s permit issued by the city or municipality where the principal place of business of prospective bidder is
located;
3. Statement of all its ongoing and completed government and private contracts within (10) years from the
submission of bids, unless otherwise stated in the BDS, including contracts awarded but not yet started, if any.
The statement shall include, for each contract the following:
3.1. name of the contract;
3.2. date of the contract;
3.3. contract duration;
3.4. owner’s name and address;
3.5. nature of work;
3.6. contractor’s role (whether sole contractor, subcontractor, or partner in a JV) and percentage of
participation;
3.7. total contract value at award;
3.8. date of completion or estimated completion time;
3.9. total contract value at completion, if applicable;
3.10. percentages of planned and actual accomplishments, if applicable;
3.11. value of outstanding works, if applicable;
3.12. the statement shall be supported by notices of award and/or notices to proceed issued by the
owners; and
3.13. the statement shall be supported by the Constructors Performance Evaluation System (CPES)
rating sheets and/or certificates of completion and owner’s acceptance, if applicable;
4. Unless otherwise provided in the BDS, valid Philippine Contractors Accreditation Board (PCAB) license and
registration for the type and cost of the contract for this Project;
5. Audited financial statements, showing, among others, the prospective total and current asset and liabilities,
stamped “received” by the BIR or its duly accredited and authorized institutions, for the preceding calendar year
which should not be earlier than two (2) years from the date of bid submission;
6. NFCC computation or CLC in accordance with ITB Clause 5.5; and
Class “B” Document:
7. If applicable, valid Joint Venture Agreement (JVA) or, in lieu thereof, duly notarized statements from all the
potential joint venture partners stating that they will enter into and abide by the provisions of the JVA in the
instant that the bid is successful shall be included in the bid.

TECHNICAL DOCUMENTS
1. Bid security as prescribed in ITB Clause 18. If the bidder opts to submit the bid security in the form of:
1.1. a bank draft/guarantee or an irrevocable letter of credit issued by a foreign bank, it shall be
accompanied by a confirmation from a Universal or Commercial Bank; or
1.2. a surety bond accompanied by a certification coming from an authorized Insurance Commission
that a surety or insurance company is authorized to issue such instrument;
2. Project requirements, which shall include the following:
2.1. Organizational chart for the contract to bid;
2.2. List of contractor’s personnel (viz, Project Manager, Project Engineers, Material Engineers, and
Foremen), to be assigned to the contract to be bid, with their complete qualification and
experience data; and
2.3. List of contractor’s equipment units, which are owned, leased, and/or under purchase agreements,
supported by certification of availability of equipment from the equipment lessor/vendor for the
duration of the project;
2.4. Site Inspection Certificate
2.5. S-curve
2.6. PERT CPM
2.7. Construction Schedule;
2.8. Health and Safety Program; and
3. Sworn statement in accordance with Section 25.2(b)(iv) of the IRR of RA 9184 and using the form
prescribed in the Section IX Bidding Forms
DOCUMENTS COMPRISING THE BID: FINANCIAL COMPONENT
 Unless otherwise stated in the BDS, the financial component of the bid shall contain the following:
 Duly signed Bid prices in the Bill of Quantities
 Duly signed Detailed Estimates
 Duly signed Cash Flow by Quarter and Payment Schedule
 Unless otherwise stated in the BDS, the financial component of the bid shall contain the following:
 Financial Bid Form in accordance with the form prescribed in
Section IX Bidding Forms; and
 Any other document required in the BDS.
 Unless indicated in the BDS, all Bids that exceed the ABC shall not be accepted.
