AFFIDAVIT OF UNDERTAKING
I, MARJORIE L. BANZUELA, of legal age, Filipino citizen, single and with
address at Nagsiya, Sto. Domingo, Albay, after having been duly sworn in accordance
with law do hereby depose and say:

1. I am applying for Fire Officer 1 at Bureau of Fire (BFP) Region V and the
deadline for the submission of all documentary requirements is on July 6,
2018.

2. One of the requirements of BFP is the Certificate of Passing and Rating

from Professional Regulation Commission (PRC). That said certification will
be issued to me by PRC on July 10, 2018.

3. I hereby undertake to submit the necessary certifications and documents

on July 10, 2018 or upon receipt thereof from PRC.

4. I am executing this affidavit to attest to the truth and veracity of the

foregoing facts and for compliance with the requirement for application as
Fire Officer 1 at Bureau of Fire.
