JOINT AFFIDAVIT OF DISCREPANCY

WE, Ma. JANET E. MARTINEZ and ANTONIO E. BARENG, both Filipinos, both married,
of legal ages, and residents of Brgy. Tugbungan, Zamboanga City, after having been duly sworn to in
accordance with law, under oath hereby DEPOSE and STATE:

1. That we personally know Maria Socorro M. Bustamante, and her daughter. Maria Elouisa
M. Bustamante, being both our neighbors in Brgy. Tugbungan, Zamboanga City as well as
our long-time acquaintances;

2. That we know for a fact that said Maria Socorro M. Bustamante is the widow, and Maria
Elouisa M. Bustamante is the legitimate daughter of the late Alberto A. Bustamante, Jr.;

3. That in the Government Service Insurance System (GSIS) Information for Membership Form
of said Alberto A. Bustamante, Jr., the name of Maria Socorro M. Bustamante was mistakenly
encoded as “Maricor M. Bustamante”, while the name of Maria Elouisa M. Bustamante was
mistakenly encoded as “Ma. Eloisa M. Bustamante”;

4. That we hereby attest that the names Maria Socorro M. Bustamante and “Maricor M.
Bustamante”, all refer to one and the same person, Ms. Maria Socorro M. Bustamante, widow
of the late Alberto A. Bustamante Jr., and that the names Maria Elouisa M. Bustamante and
“Ma. Eloisa M. Bustamante”¸ all refer to one and the same person, Maria Elouisa M.
Bustamante, legitimate daughter of the late Alberto A. Bustamante, Jr.;

5. That we are executing the foregoing affidavit to state the truth surrounding the circumstances
mentioned above, and for whatever legal purpose this may serve;
