CONTRACT FOR SERVICES OF QUEZON INSTITUTE OF

TECHNOLOGY

Know all Men by these Presents:

This agreement, made and entitled into by and between the Local Government of the
municipality of Quezon, province of Bukidnon herein represented by the Municipal Mayor, herein
referred to as the First Party.

And

Quezon Institute of Technology an accredited educational institution with address at Blgy.

2, Poblacion, Quezon, Bukidnon herein represented by the head, School President/Instructor In-
charge of the subject herein referred to as the second party:

WITNESSETH
Whereas, quality pre service education is a key factor in quality Philippine
education.
Whereas, on the-job training for high school students is one of the most
important and crucial phases in vocational education.

Whereas, the Engineering Department of the Municipality of Quezon is

believe to be capable of providing training for practical electricity.

Whereas, students on the job training will follow strictly the rules and
regulation of the Department where he/she is assigned.
 Whereas students in the job training are required to secure parents/ guardian
weaver, medical certificate and barangay clearance before the scheduled training.

This agreement shall take effect immediately upon signing hereof and
maybe amended or modified upon mutual agreement of both parties and shall
remain enforce until the termination of the training.

In Witness, heron the parties through their representative have signed this
agreement on the date appearing in the acknowledge hereof.
