Evaluation of pain score in immunotherapy injection


for children with allergic rhinitis

Background: Allergic rhinitis has high comorbid impact on people's lives. Good prognosis and fewer side
e ects were found when patients received standard treatment and immunotherapy than standard
treatment alone. Immunotherapy is given by subcutaneous injection. Fear of needle is the main cause of
non-adherence immunization in children. Pain caused by subcutaneous injection is closely related to the
dose of the injected fluid: the higher the dose of fluid, the higher the pain caused.

Objective: To evaluate pain score in children with allergic rhinitis who get various dose of immunotherapy
injection.
Method: This study was an observational study-case series that took data from pediatric patients with
allergic rhinitis in Dr. Sardjito Hospital. It was conducted from April 2016 to May 2017. A total of 24 subjects
aged 3-18 years were included in the analysis. Pain score was measured using Wong Baker Face Pain Scale,
and the evaluation of pain scores complained by male and female patients were recorded weekly for seven
weeks.
Result: In the first injection(0.1cc), the mean pain score were 2.4 (male) and 0.83 (female). Second
injection(0.15 cc), the mean pain score were 3.13(male) and 1.17(female). Third injection(0.22 cc), the mean
pain score were 2.27(male) and 1(female). Fourth injection(0.32cc), the mean pain score were 3(male) and
0,5(female). Fi h injection(0.48cc), the mean pain score were 2.07(male) and 0.17(female). Sixth
injection(0.72 cc), the mean pain score were 3.8(male) and 1.5(female). Seventh injection(1cc), the mean
pain score were 3.2(male) and 1.67(female). Male patients had the lowest mean pain score at dose of 0.48
cc and the highest at dose of 0.72 cc. Female patients had the lowest mean pain score at dose of 0.48 cc
and the highest at dose of 1cc.

Conclusion: The mean pain score of male patient with allergic rhinitis is higher than mean pain score of
female patient in all immunotherapy injection dose.
