III. INTRODUCTION Barge bumpers at P-I P/F were inspected during the last under water survey
conducted on 2014 and all barge bumpers were found in fair condition with no signs
of dents or cracks as per the UWS reports.
During the approach of the supply boat M/V Miser Gulf 13 to the platform on
05/04/2016, an impact was occurred between the a/m marine vessel and the
platform barge bumper fixed on leg Bl.
Inspection sector crew directed to the location of incident on 05/04/2016 to evaluate
the current condition and Preliminary Inspection Note was issued on 06/04/2016
with initial findings and recommendations. (See Attachment#1).

IV. FINDING AND During the initial visually survey on 05/04/2016 the following were observed :
OBSERVATION Damage and cracks were observed in the weld seam of the upper fixation
for the subject barge bumper. (See Attachment#3, Figs. 1 & 2).
Dent, deformation and rupture were observed in the subject barge bumper
body itself (18" pipe). (See Attachment#3, Figs. 3 & 4).
After Scaffolding and sand-blasting activities have been finished, thorough inspection
was performed on the subject barge bumper on 23/05/2016 especially on the upper
welded fixation point of the barge bumper to platform main leg (B1), and the
following were observed:-
Crack was observed on the upper weld seam between upper beam support
& the barge bumper. (See Attachment#2 and Attachment#3 Figs. 5 & 6).
Crack was observed on the lower weld between upper beam support & the
barge bumper. (See Attachment#2 and Attachment#3 Figs. 7 & 8).
Dent, deformation and rupture with length of 35 cm were observed on the
barge bumper body (18" pipe). (See Attachment#2 and Attachment#3 Figs.
9 & 10).
Magnetic Particle Inspection (MPI) was performed for all welds between
the barge bumper and leg B1 and there is no sign of cracks. (See
Attachment #3, Figs. # 11 & 12).
UT Thickness Measurements were performed for the support beam and the
barge bumper and found in fair condition except for the ruptured area.
(See Attachment #2)
. DISCUSSION'' As per the MPI Inspection most of the above water welds between leg B1 & barge
bumper were found in fair condition, except where cracks were found in welds
between upper beam support & barge bumper.
UT Thickness Measurements shows that the barge bumper in fair condition and no
metal loss were observed.
All the dent, deformation, rupture and weld cracks were occurred due to the impact
between the supply boat M/V Miser Gulf 13 and the barge bumper at the time of
incident.

In order to keep the integrity of the subject P/F and to avoid any damage or
catastrophic failure consequences, the following are needed to be done:
Underwater inspection for the underwater part of the barge bumper to be
performed to evaluate the condition of the underwater welds between leg B1 &
barge bumper.
All necessary action must be taken to replace the defected portion of barge bumper.
