Assignment: Word-processed report addressing the following points:

1. What was the nature of the specialty area?


The dialysis area was a normal hospital room, but it was set up for 1:1 care
so that the nurse can focus all of the attention on this one patient.

2. What was the role of the registered nurse in this setting?

The role of the nurse is to set up the room for the patient this includes the
machine that is used medications if needed, and fluids. The nurse then
checks the latest lab values and when the patient’s comes more blood is
obtained for lab if ordered. The nurse then is responsible for accessing the
patient’s port for dialysis. During this time the nurse must have of the patient
and the port throughout dialysis therapy.

3. What were the medical diagnoses of the patients who were receiving
dialysis?

The patient had a history of end stage renal failure, and she has breast
cancer that has metastasized.

4. What were the underlying medical problems that lead to the renal problem?

There was no problem that I noted in H&P and at the time of interview patient
was anxious and not a good historian. Amy didn’t remember the initial reason
of her renal failure.

5. What was the expectation of the treatment?


The person of this patient’s dialysis treatment was to get rid of some extra
fluids that the patient was retaining, and cleanse the blood from waste
products.

6. What medications were given in this setting?


No medications were giving to the patient today.

7. What type of hemodialysis grafts did the patients have?

The patient had a IJ and a AV fistula, the fistula was not assessed because
that last time the patient had dialysis it was hard to stop the bleeding she had
to hold the site for 55 minutes.

8. Explain the anatomy of the types of grafts.


AV fistula is created surgically by anastomosis of a large artery and large vein
in the arm.

9. What patient teaching did you see performed? What patient teaching could
have been performed?

The patient teaching that was performed was the overall health of the patient.
The patient really wanted to be discharged, but didn’t have a stable house to
go to, so Amy did patient teaching with regarding how her immune system is
already down and going home to sick people would just make it worst, the Dr.
also reinerated this.

10. How long was pressure held at the arterial and venous needle sites before
hemostasis was achieved?

In this case pressure was held 8 minutes.
