JUDICIAL AFFIDAVIT OF
JOHN SINA

I, JOHN SINA, of legal age, married, and living at #133 Brgy. Nueve
de Pebrero, Mandaluyong City, the complainant in the instant case, after
having been duly sworn, hereby attests that:

PRELIMINARY STATEMENT

The person examining me is Atty. Arnel Arboleda with office address

at #123 Barangka Drive, Brgy. Barangka, Mandaluyong City. The
examination is being held at the same address. I am answering his questions
fully conscious that I do so under oath and I may face criminal liability for
false testimony and perjury.

PURPOSE: This affidavit/testimony is being offered to prove the identity of

the accused.

1. Q. Please state your name and other personal circumstances for the
record.
A. I am John Sina, 44 years old, complainant in this case.

2. Q. Where were you on December 15, 2018?

A. I was on duty as security guard from 8:00AM to 4:00PM at
HAPPY SHOPPING CENTER.

3. Q. Do you remember any significant event that day?

A. There was a bombing incident happened.

4. Q. Can you tell me what were your recounts of the events?

 A. Yes. Prior to the event, at about
8:50 AM, while I was having my
roving at main entrance of the
mall, I noticed that, a man at his
middle age was wandering at the
area. He ask me of the time the
mall opens and I told him that
9:00 AM. Then he left and said
that he will just look for a
Comfort Room. After he left, I
noticed that he left his bag on the
chair. When he did not come back
after five minutes, I went to look
for him. While looking for him,
the bomb exploded.

5. Q. Then happened next?

A. The police came, they ask me to look at their rogue gallery. I
identified. The one who left the bag as the same person whose
names appears as Alfredo R. Geronimo also known as “Ka
Alfred”.

6. Q. I there anything that you want to tell me?

A. No more for now sir.
