HAZOP (Hazard and Operability)

DEFINITION
 Hazards and Operability- is defined as: “The application of a formal systematic critical examination of the process and
engineering intentions of the new facilities, to assess the hazard potential of mal-operation or malfunction of
individual items of equipment and the consequent effects on the facility as a whole”.

OBJECTIVE
 HAZOP is to assess the hazard potential of mal-operation or malfunction of equipment and the consequent effects on
the facility.
 It attempts to identify how a process may deviate from the design intent.

WHY IT’S REQUIRED

 HAZOP study Assesses engineered controls (ex: automation), sequences of operations, procedural controls (ex:
human interactions) etc.
 Review different operational mode with respect to start up, shut down, emergency etc.
 Identify hazards during the operation life of the plant with respect to Modifications, change on operation window,
critical control loops, change in composition etc.

 Major modifications and changes in facilities.

 Experience from accidents, analysis of causes.
 Organizational changes.
 Changed regulatory requirements.
 Carrying Out Changes of Logic of Interlocks Set Point and Process Conditions.
 To identify potential hazards in the plant.

 M.B. LAL COMMITTEE RECOMMENDATION

Section 10.4. states the following
 Carry out HAZOP and Quantitative Risk Assessment (QRA) on large sized installations through well qualified agency
and implement recommendations forthwith.
 HAZOP study Assesses engineered controls (ex: automation), sequences of operations, procedural controls (ex:
human interactions) etc.
 Review different operational mode with respect to start up, shut down, emergency etc.
 Identify hazards during the operation life of the plant with respect to Modifications, change on operation window,
critical control loops, change in composition etc.
 Case study to highlight the importance of HAZOP.
